Cheek sagging is an aesthetic and structural skin problem that occurs when cheek tissues shift downward as a result of aging, collagen loss, and reduced skin elasticity. It reduces the definition of facial contours, deepens the nasolabial folds, and creates a loose appearance in the lower face due to volume loss.
The symptoms of cheek sagging appear as loosening of the cheek tissue, deterioration of the facial oval, and loss of jawline definition. As the subcutaneous fat tissue shifts and the connective tissue weakens, volume loss develops in the midface, and the face may take on a more tired, aged, and sagging appearance.
The causes of cheek sagging include the natural aging process, decreased collagen and elastin production, rapid weight changes, and sun damage. Genetic predisposition, smoking, and inadequate skin care may also contribute to the deterioration of skin elasticity and lead to early sagging of the cheek tissues.
Cheek sagging treatment methods are planned according to the degree of sagging and the patient’s skin structure. Filler applications, thread lift techniques, radiofrequency, and ultrasound-based tightening methods are among the non-surgical options, while facelift surgery may provide a permanent and effective solution in advanced cases.
| Definition | Cheek sagging is the loss of definition in facial contours caused by the downward displacement of cheek tissues due to factors such as aging, gravity, reduced skin elasticity, displacement of fat tissue, and collagen loss. |
| Symptoms | Downward sagging of the cheeks, loss of jawline definition, deepening of the nasolabial folds, sagging at the corners of the mouth (jowl formation), and a tired and aged facial appearance. |

What Is Cheek Sagging?
Cheek sagging is the condition in which the cheek tissues loosen and shift downward due to aging, reduced skin elasticity, and the effects of gravity, causing the facial contours to lose definition. Reduced collagen and elastin production, weight changes, and genetic factors may accelerate this condition. It may create a tired and aged appearance on the face. Skin-tightening applications, filler procedures, or surgical facelift methods may be preferred for treatment.
Anatomical Foundations of Cheek Sagging
The main structures that provide firmness and a youthful appearance to the cheeks are as follows:
- Skin: The main components that provide skin elasticity are collagen and elastin. As age advances, the production of these proteins decreases, and the existing proteins also deteriorate structurally. This causes the skin to lose its ability to stretch and return to its original form.
- Fat Pads (Buccal Fat Pads): These fat pads, which give the cheeks their full and rounded appearance, shift from their original position over time under the effects of gravity and contribute to sagging.
- Muscles: The facial expression muscles may lose their tone over time. This relaxation reduces the support beneath the skin and increases sagging.
- Connective Tissue: The connective tissue that holds the skin, fat, and muscles together also loosens and stretches with aging, causing an overall feeling of sagging.
The development of cheek sagging is a complex process that results from the interaction of these anatomical elements. Thinning of the skin, displacement of fat tissue, and reduced muscle support combine with the effects of gravity and lead to noticeable sagging in the lower part of the face. This condition may make a person appear more tired, sad, or older than they are.
Causes of Cheek Sagging
The best-known cause of cheek sagging is aging, but there are many different underlying reasons for this condition. Our genetic inheritance, lifestyle choices, and environmental factors determine how early and how noticeably this aesthetic change appears on the face.
- Aging Process
Aging is the most fundamental and inevitable cause of cheek sagging. As the human body ages, collagen and elastin production naturally decreases. These two proteins are the main building blocks that preserve the firmness, elasticity, and youthfulness of the skin. Collagen provides structural support to the skin, while elastin allows the skin to return to its original state after being stretched. As these proteins decrease, the skin becomes thinner, loses its elasticity, and begins to sag downward under the effects of gravity. The facial fat pads may also gradually shift from their natural positions because of gravity and accumulate in the lower parts of the face, making the appearance of sagging even more noticeable. Reduced muscle tone also weakens the support beneath the skin and creates a basis for sagging.
- Genetic Predisposition
Some people may respond to the aging process earlier and more noticeably because of their genetic structure. If you observe early facial sagging or deep lines in your family members, you may also have a similar predisposition. Genetic factors may affect the skin’s ability to produce collagen and elastin, the distribution of fat tissue, and bone structure, determining the rate and pattern of facial aging. Therefore, genetic inheritance may significantly influence when and to what extent cheek sagging occurs.
- Harmful Sun Rays (UV Damage)
The sun’s ultraviolet (UV) rays are among the greatest enemies of the skin. UV rays damage the collagen and elastin fibers in the skin and accelerate their breakdown. This causes the skin to age earlier, lose elasticity, and develop more noticeable wrinkles and sagging. Spending long periods in the sun without protection over many years may significantly increase signs of aging such as cheek sagging. Using sunscreen is one of the most effective ways to reduce these harmful effects.
- Smoking and Alcohol Consumption
Smoking is one of the most harmful habits that accelerates skin aging. The chemicals in cigarette smoke narrow the blood vessels and reduce the amount of oxygen and nutrients reaching the skin. This causes the skin to look paler and more lifeless. Smoking also disrupts collagen and elastin production, causing the skin to lose elasticity and sag. Alcohol consumption may also cause fluid loss in the body, resulting in dry and more wrinkled-looking skin. Excessive alcohol consumption may negatively affect overall health and indirectly impair skin health.
- Rapid Weight Loss or Gain
Sudden and rapid weight changes may trigger cheek sagging. When weight is gained, the skin expands, and this may cause fat accumulation, especially in the cheeks. Rapid weight loss afterward may cause the skin to become loose and sag before it has enough time to recover. When the skin loses the volume to which it had expanded, it may not regain its former firmness, and this appears as sagging in the lower parts of the face. A slow and controlled weight-loss process gives the skin more time to adapt to these changes and reduces the risk of sagging.
- Inadequate Nutrition and Fluid Consumption
Obtaining sufficient vitamins and minerals required for the healthy functioning of the body is also critically important for skin health. A balanced diet consisting of fruits and vegetables rich in antioxidants helps protect the skin against the harmful effects of free radicals. Inadequate nutrition, especially deficiencies in antioxidants such as vitamins C and E, may negatively affect collagen production and cause the skin to age more quickly. In addition, insufficient water intake may cause the skin to become dry, lose elasticity, and appear more wrinkled.
- Skin Care Habits
Incorrect or inadequate skin care may accelerate cheek sagging. Failing to cleanse, moisturize, and protect the skin from the sun regularly causes the skin to deteriorate and lose elasticity over time. Using products containing harsh chemicals or rubbing the skin excessively may also impair the skin barrier, increase sensitivity, and trigger signs of aging. A regular care routine using gentle products that support the natural structure of the skin may help delay signs of sagging.
Cheek Sagging Treatment Methods
Today, there are many effective surgical and non-surgical treatment methods for cheek sagging. Determining which method is most appropriate for you depends on the degree of sagging, your skin structure, your expectations, and your general health condition. You can create the most suitable personalized treatment plan by consulting a specialist.
Non-Surgical Methods
Non-surgical methods are generally less invasive, provide shorter recovery times, and are more affordable. These methods are ideal for mild to moderate sagging.
Fillers (Hyaluronic Acid, Calcium Hydroxyapatite, etc.)
Fillers are among the most popular non-surgical methods used to correct facial volume loss and fill the hollow areas caused by sagging. These substances, which are generally based on hyaluronic acid, add volume when injected into the skin, smooth fine lines and deep wrinkles, and reshape facial contours. When applied to the cheeks, they may create an upward lifting effect and reduce the appearance of sagging by restoring lost volume and supporting the skin tissue. Other fillers such as calcium hydroxyapatite may also stimulate the skin’s natural collagen production and provide long-term tightening.
- How Does It Work? Fillers are injected beneath the skin to restore lost volume and support the skin tissue. This makes the face appear fuller and younger. When used in the cheek area, they create a lifting effect by filling the hollows caused by sagging.
- Application Process: It is generally a procedure completed within a few minutes under local anesthesia. Mild redness or swelling may occur after the injection, but this usually resolves within a few days.
- Advantages: Rapid results, minimal recovery time, natural-looking results, and reversibility for certain fillers.
- Disadvantages: Temporary effect, generally lasting 6–18 months, cost, and a rare risk of bruising, swelling, or infection.
Botulinum Toxin (Botox)
Botox is an injectable treatment that works by temporarily preventing the movement of facial expression muscles. Although it is not used as a direct treatment for cheek sagging, it may improve the overall appearance of the face by softening lines in other areas, such as forehead lines and crow’s feet, and slightly lifting the eyebrows. This indirect effect may contribute to a more rested and youthful appearance. For concerns such as more noticeable nasolabial folds caused by cheek sagging, Botox may help make these lines less noticeable by reducing muscle activity.
- How Does It Work? Botox blocks the signals transmitted from nerve endings to the muscles, allowing the muscles to relax temporarily. This reduces the appearance of wrinkles and fine lines.
- Application Process: It is injected into the targeted muscles using very fine needles. The procedure takes a few minutes and is generally painless. Results begin to appear within a few days.
- Advantages: Reduces wrinkles, makes the face look more rested, rapid application, and minimal side effects.
- Disadvantages: Temporary effect, generally lasting 3–6 months, unnatural facial expressions when administered in excessive doses, and rare side effects such as eyelid drooping.
Laser Treatments (Fractional Laser, RF Laser)
Laser and radiofrequency (RF) technologies stimulate collagen production and help tighten the skin by heating the deeper layers of the skin. Fractional lasers create microscopic damage on the skin surface and trigger the skin’s self-renewal process. RF energy reaches the deeper layers of the skin and causes collagen fibers to contract and restructure. These treatments increase skin elasticity and improve mild sagging, contributing to a firmer appearance of the cheeks.
- How Does It Work? Laser and RF energy heat and stimulate the collagen fibers in the deeper layers of the skin. This heating triggers collagen production and causes existing collagen fibers to contract, leading to skin tightening.
- Application Process: It is applied in sessions. A mild sensation of heat may be felt during the procedure. There may be several weeks or months between sessions. Multiple sessions may be required for complete results.
- Advantages: Increases skin firmness, reduces fine lines and wrinkles, improves skin tone, and is less invasive than surgery.
- Disadvantages: Multiple sessions may be required for complete results, temporary redness, swelling, or tenderness may occur, and it may be costly.
Ultherapy (HIFU – Focused Ultrasound)
Focused ultrasound (HIFU) technology sends ultrasound energy to the deeper layers of the skin, stimulating collagen production and tightening the skin. This method may target the layer beneath the skin called the SMAS, or superficial musculoaponeurotic system, without surgical intervention. The SMAS layer contains the facial muscles and connective tissue and is also tightened during facelift surgery. HIFU heats this layer, causing it to tighten and create a lifting effect.
- How Does It Work? Focused ultrasound waves deliver energy to specific depths of the skin, generally up to 4.5 mm, corresponding to the SMAS layer. This energy creates microscopic heating zones in the targeted tissue. The body’s natural healing process triggers new collagen production in these areas, causing the skin to tighten and lift gradually.
- Application Process: A single session is generally sufficient. A mild stinging or heat sensation may be felt during the procedure. Daily activities can be resumed immediately after the session. Results generally begin to become noticeable within a few weeks, and the full results appear within 2–3 months.
- Advantages: Non-surgical facelift effect, ability to target the SMAS layer, single-session application, and minimal recovery time.
- Disadvantages: Temporary redness or tenderness in some people, it may be costly, results may vary from person to person, and it is generally more effective for moderate sagging.
Thread Lift Methods (Spider Web Aesthetics, PDO Threads)
Thread lift methods involve lifting and tightening the skin with special threads placed beneath the skin. These threads are made from biocompatible materials that dissolve beneath the skin over time, generally PDO, or polydioxanone. Small cones or barbs on the threads hold the skin and create an upward lifting effect. In addition, the controlled injury created by the threads in the skin stimulates collagen production and provides long-term tightening. This method is effective in defining the cheeks and jawline.
- How Does It Work? Specially designed biocompatible threads with small cones or barbs are placed in specific layers of the skin. These cones grip the tissues beneath the skin and create an upward lifting effect. The presence of the threads also stimulates the body’s natural collagen production.
- Application Process: It is performed under local anesthesia. The placement of the threads may take 30–60 minutes depending on the degree of sagging and the treated area. Mild swelling, bruising, or pain may occur after the procedure.
- Advantages: Immediate lifting effect, less invasive than surgery, stimulates collagen production, and may provide a natural appearance.
- Disadvantages: Temporary effect due to the threads dissolving over time, generally lasting 1–2 years, rare risks of infection, asymmetry, or thread visibility, and skin dimpling in some people.
Skin Rejuvenation and Tightening Devices (Gold Needle RF, Cryolipolysis, etc.)
Devices such as gold needle RF, or radiofrequency, deliver RF energy to the deeper layers of the skin through microneedles, providing both skin rejuvenation and tightening. This method is effective for acne scars, wrinkles, and mild sagging. Methods such as cryolipolysis mainly target localized fat but may also provide indirect benefits that help tighten the skin. These devices support the skin’s natural collagen and elastin production, helping achieve a firmer and younger appearance over time.
- How Does It Work? Gold needle RF creates controlled damage in the skin using microneedles while simultaneously delivering RF energy to the dermis. This dual effect triggers the skin’s natural healing process and increases collagen and elastin production, resulting in skin tightening and renewal.
- Application Process: Several sessions are generally required. A specific interval, usually 4–6 weeks, is allowed between sessions. Mild pain or discomfort may be felt during the procedure, so a local anesthetic cream may be used.
- Advantages: Increases skin firmness, reduces fine lines and wrinkles, improves skin texture, and is effective for acne scars and pigmentation.
- Disadvantages: Requires multiple sessions, temporary redness, swelling, or bruising may occur, and it may be costly.
Surgical Methods
Surgical methods generally provide the most effective solutions for more pronounced sagging. These methods offer longer-lasting results but involve longer recovery times and higher risks.
Facelift Surgery (Rhytidectomy)
Facelift surgery is a comprehensive surgical procedure performed to achieve a younger and firmer appearance by tightening sagging skin and soft tissues in the face and neck. This operation is generally performed to correct sagging in the middle part of the cheeks, looseness along the jawline, and sagging in the neck. The surgeon lifts and repositions the skin and underlying tissues, defines the facial contours, and significantly reduces the signs of aging.
- How Does It Work? The surgeon reaches the skin and the underlying muscle and fat tissues through incisions generally made in inconspicuous areas such as in front of and behind the ears. Excess sagging skin is removed, and the loosened muscles are tightened and reshaped. The skin is then closed again under tension.
- Application Process: It is performed under general anesthesia and may take several hours. A hospital stay of one or two days may be required after surgery. The recovery process takes several weeks, during which swelling, bruising, and pain may occur.
- Advantages: Provides the most effective and permanent results, significantly corrects facial sagging and deep wrinkles, and defines facial contours.
- Disadvantages: It is the most invasive method, involves a longer recovery period, higher risks such as infection, nerve damage, and scarring, is costly, and carries a risk of an unnatural appearance if not performed correctly.
Mid-Facelift
A mid-facelift is a facelift technique that specifically targets sagging in the middle part of the cheeks and under-eye bags. This operation improves the overall facial structure by making the cheeks appear fuller and more lifted. It is generally performed together with eyelid surgery, or blepharoplasty.
- How Does It Work? A mid-facelift is generally performed through incisions made during eyelid surgery. Through these incisions, the soft tissues in the middle part of the cheeks are lifted upward and repositioned. This makes the cheekbones more prominent and reduces under-eye bags.
- Application Process: It is generally performed under local anesthesia or mild sedation. The recovery period is shorter than that of a full facelift.
- Advantages: Targets sagging and hollowness in the middle part of the cheeks, provides a younger and more rested appearance, and reduces under-eye bags.
- Disadvantages: It may not provide results as comprehensive as a full facelift, requires a recovery period, and carries surgical risks.
Facial Skin Rejuvenation (Dermabrasion, Deep Chemical Peeling)
Methods such as dermabrasion and deep chemical peeling remove the upper layers of the skin mechanically or chemically, allowing fresher, smoother, and firmer skin to emerge from underneath. These methods may be effective in treating fine wrinkles and superficial sagging. However, they may not be sufficient on their own for pronounced cheek sagging and are generally used in combination with other methods.
- How Does It Work? Dermabrasion removes the upper layer of the skin by abrading it with a brush or a sandpaper-like device. Chemical peeling removes the upper layer of the skin through controlled burning with strong chemicals applied to the skin. This peeling process triggers the skin’s self-renewal process and reveals smoother and firmer skin.
- Application Process: It may be performed under local anesthesia or sedation. Recovery time varies depending on the depth of the procedure and may take several weeks. Redness, swelling, and tenderness may occur during this period.
- Advantages: Smooths the skin surface, reduces fine lines and superficial spots, and improves skin tone.
- Disadvantages: It is not effective alone for pronounced sagging, the recovery period is long, there is a risk of infection and scarring, and it increases sun sensitivity.
Recovery Process and Care After Cheek Sagging Treatment
The recovery process varies depending on the treatment method. Non-surgical methods generally provide faster recovery, while surgical procedures require a longer rest period.
After Non-Surgical Methods
- Immediately Afterward: Mild redness, swelling, tenderness, or bruising may occur. These effects generally resolve within a few days.
- Daily Activities: Most patients can return to their normal activities immediately after the procedure or within a few days.
- Care: It is important to use the creams recommended by your doctor, protect the skin from the sun, and drink plenty of water.
After Surgical Methods
- Hospital Stay: Depending on the type of surgery, it may be necessary to stay in the hospital for one night or longer.
- Recovery Process: Swelling, bruising, pain, and numbness may occur during the first few weeks. Facial bandages or compression garments may be used.
- Follow-Up Visits: Your doctor will call you for follow-up examinations at specific intervals. Sutures are generally removed within 1–2 weeks.
- Activity Restrictions: Heavy exercise, strenuous movements, and activities that strain the face should be avoided for several weeks.
- Wound Care: It is important to use the wound care products recommended by your doctor to support scar healing. Complete recovery and the appearance of the final results may take several months.
Long-Term Results and Maintenance
Your lifestyle choices are extremely important for helping the treatment remain effective and preventing future sagging.
- Sun Protection: Using high-factor sunscreen every day is one of the most effective ways to prevent skin aging and loss of elasticity.
- Healthy Nutrition: A balanced diet rich in antioxidants supports skin health and resilience.
- Adequate Fluid Consumption: Drinking at least 8 glasses of water per day is important for keeping the skin moisturized and elastic.
- Smoking and Alcohol: Avoiding smoking and limiting alcohol consumption slow the skin aging process.
- Skin Care: Gently cleansing and moisturizing your skin and maintaining regular care help the skin remain youthful.
- Weight Control: Avoiding sudden weight gain and loss prevents excessive stretching and sagging of the skin.
