Lipolysis and localized slimming technologies are the process of breaking down stubborn fat deposits—formed due to genetic predisposition, hormonal changes, or lifestyle factors and resistant to diet or exercise—through physical or chemical methods and eliminating them from the body. Rather than being an obesity treatment or a weight-loss tool, this medical process reshapes body contours by converting stored intracellular triglycerides into free fatty acids and initiating metabolic cleansing. Removing localized excesses that disrupt tissue architecture and aesthetic proportions aims to achieve a healthier, firmer, and more proportionate body contour through the controlled management of adipose tissue, an active organ.

What Is Lipolysis and Localized Slimming?

Lipolysis and Localized Slimming refers to medical and aesthetic applications aimed at reducing stubborn adipose tissue accumulated in specific areas of the body. The lipolysis procedure aims to break down fat cells using various methods such as injections, laser, ultrasound, or radiofrequency. Localized slimming applications aim to make the body contour appear more balanced. The appropriate method should be determined by a specialist physician after evaluating the person’s fat distribution and overall health status.

How do stubborn fats in our body turn into energy through the lipolysis process?

Our body’s survival mechanism is built on storing energy for days of scarcity. However, in modern life, this storage leads to unwanted appearances around the waistline or hips. Lipolysis is the process of converting packaged fats (triglycerides) stored in our body into usable fuel. We can liken this process to melting a block of butter and using it as energy.

In the chemical kitchen of this conversion, a highly delicate equation operates. When a triglyceride molecule reacts with water, it breaks down and free fatty acids emerge. We can formulate this biochemical reaction as follows:

The essence of this seemingly complex formula is this: When the right stimuli are given, the fat cell empties its contents. These released fatty acids are transported to the liver via blood and lymph circulation. Our liver either burns these wastes to produce energy or metabolizes them and removes them from the body. Every intervention we perform clinically and surgically is essentially about triggering this natural process—which the body can do on its own—using external sound waves, heat, or enzymes.

In which areas is the injection lipolysis method effective?

For patients who do not want to go under the knife or whose problem is very localized, injection methods—namely enzymatic lipolysis—are a very attractive option. The main substance we use in this method is actually a natural enzyme found in the structure of cell membranes in our body and derived from soybeans. When we inject this enzyme under the skin, we gently dissolve the wall of the fat cell and allow its contents to leak out.

This method delivers excellent results in small and stubborn areas that require “pinpoint” treatment rather than large areas. In this protocol, which generally lasts 2 to 4 sessions, we wait a few weeks between each session to allow the body to clear that area. You can return to your social life immediately after the procedure, but completing the process requires some patience.

The most commonly preferred areas for injection lipolysis are:

  • Submental area
  • Inner knee fat deposits
  • Bra-line bulges
  • Small lower-abdomen bulges
  • Underarm accumulations
  • Cheek slimming

How does cold lipolysis destroy fat cells?

The story of cold lipolysis (Cryolipolysis) technology is quite interesting: scientists developed this technology after noticing fat loss in the cheeks of children who ate too much ice cream. The principle is simple: fat cells are far less resistant to cold than our skin or muscles. We use this sensitivity to our advantage.

During the procedure, the device vacuums the targeted fat pad and cools it down to approximately -10 degrees. While your skin remains protected with special pads, the underlying fat cells go into shock and crystallize. The crystallized cells lose their viability and are recognized by the body’s “scavenger cells,” called macrophages. In the following weeks, the lymphatic system slowly eliminates these dead cells from the body. The result is not seen immediately, but you will clearly notice the slimming in that area after 2-3 months.

Why is Vaser Liposuction different from classical methods?

In our plastic surgery practice, liposuction has evolved from a crude fat-removal procedure into the art of “High Definition” body contouring. Vaser technology is the most important part of this revolution. Vaser uses ultrasonic sound waves. If we liken the classical method to breaking branches to pick apples from a tree, Vaser technology is like shaking the tree to make only the ripe apples (fats) fall:

Thanks to this selective technology, we do not harm surrounding tissues, vessels, or nerves while removing fat. Sound waves target only fat cells and liquefy them into a “milky consistency.” This allows us, as surgeons, to work much more precisely and enables our patients to experience a much more comfortable recovery process. In addition, since the removed fats preserve their viability, they can be transferred as a filler material to areas such as the buttocks or face if needed.

The main advantages Vaser technology provides to patients are:

  • Less bruising
  • Minimal swelling
  • Fast recovery
  • Less pain
  • Smoother skin surface
  • Natural appearance

How does laser lipolysis provide skin tightening?

If a patient has some skin laxity, removing fat alone is not enough; the skin also needs to adapt to the new volume. This is where laser lipolysis comes in. Laser energy works in two directions: it bursts the membrane of the fat cell with heat and also heats the subdermal layer, creating an “ironing” effect.

This heat effect stimulates collagen fibers in the lower layer of the skin and causes them to contract. Just as a wool sweater shrinks in hot water, the skin’s collagen fibers also tighten with heat. This feature gives us a great advantage especially in the submental area, underarms, or loosened abdominal skin after childbirth. However, because the heat of the laser is strong, it must be applied by experienced hands with correct energy settings to avoid tissue burns.

Who is not suitable for lipolysis applications?

If half of a successful result is applying the right technique, the other half is choosing the right patient. Although these methods are technological marvels, they may not be suitable for everyone. Our primary rule is that these procedures are not a weight-loss method or an obesity treatment. In patients with a Body Mass Index (BMI) over 30, we recommend losing weight before localized contouring.

In addition, because these are procedures that impose a metabolic load, the body’s general health status must be adequate. It should be remembered that the broken-down fats will be eliminated by the liver and kidneys. Therefore, we avoid these procedures in individuals with serious organ function problems.

Situations in which lipolysis and localized slimming procedures are not performed are:

    Pregnancy

    • Breastfeeding period
    • Severe heart failure
    • Liver failure
    • Kidney failure
    • Active infections
    • Uncontrolled diabetes
    • Blood clotting disorders

What should be considered during the postoperative recovery process?

The period after surgical or non-surgical lipolysis procedures is at least as important as the procedure itself. We have given your body a new shape; now your body needs to memorize this new shape. After surgical procedures such as Vaser or Laser, our patients can usually return to their daily routines within 3-7 days. However, tissue healing continues internally.

In this process, our greatest helpers are compression garments. Think of the garment not only as a compression tool but as the new mold of your body. Wearing a garment is essential to keep swelling under control and to allow the skin to adhere smoothly to the underlying tissue. In addition, since there are cellular wastes that must be eliminated from the body, it is necessary to support metabolism.

What we expect from our patients during the recovery period is:

    • Regular use of compression garments
    • Drinking plenty of water
    • Light walks
    • Lymphatic drainage massage
    • Salt restriction
    • Avoiding smoking
