Lipoabdominoplasty is a combined surgical technique in which tummy tuck (abdominoplasty) and liposuction procedures are performed together. It provides more effective and aesthetic results by eliminating both skin sagging and regional fat excess in the same session.

With this method, excess fat in the abdominal area is removed with liposuction, while sagging skin and loose muscle structure are corrected with abdominoplasty. Thanks to the dual intervention, both a slimmer waistline is achieved and the abdomen becomes flatter and gains a tighter appearance.

Lipoabdominoplasty is particularly suitable for women whose body shape has changed after childbirth and for individuals who experience abdominal deformity after weight loss. During the operation, the navel can be repositioned; the results are long-lasting and satisfying.

The recovery process usually takes 2–4 weeks; during this period, the use of a special corset is recommended. Edema and bruising may occur in the first days, but they decrease over time. The patient can usually stand up within a few days; full results appear within a few months.

What Is Lipoabdominoplasty (Tummy Tuck + Liposuction)?

Lipoabdominoplasty is an aesthetic operation that combines the surgical removal of excess skin and fat tissue in the abdominal area in the same session and shaping with liposuction. This method aims to correct sagging and looseness that occur especially after childbirth, weight fluctuations, or aging. While the abdominal muscles are tightened, regional fat excess is also reduced, resulting in a flatter, tighter, and more contoured abdominal appearance.

Who is the ideal patient profile for Lipoabdominoplasty and to whom is it not applied?

As with every surgical procedure, the basis of success in lipoabdominoplasty is correct patient selection. First of all, the following distinction must be made very clearly: This surgery is not a weight loss method or an obesity treatment. It is performed to improve body contours, not to reduce the number on the scale.

The ideal patient profile consists of individuals who are in good overall health but cannot get rid of fat accumulation and skin sagging in the abdominal area despite diet and exercise. Body Mass Index (BMI) is an important guide for us at this point. Ideally, a BMI value below 30 is preferred. In patients above this limit, both anesthesia-related risks may increase and wound healing problems may occur. Therefore, for patients who are in the process of losing weight, planning surgery after reaching their target weight is the healthiest approach.

Some common characteristics of the most suitable candidates for this surgery are as follows:

  • Those with a body mass index below 30
  • Those with significant sagging of the abdominal skin
  • Those with regional and stubborn fat accumulation
  • Those whose weight has been stable for the last 6 months
  • Those whose general health condition is suitable for surgery
  • Those with realistic expectations

In addition, patients who have undergone bariatric surgery such as sleeve gastrectomy and lost a large amount of weight also benefit greatly from this procedure. These patients usually have severe skin sagging in the form of an “apron.” Lipoabdominoplasty helps remove this excess skin and allows the body to regain its new form. However, in this group of patients, a detailed vitamin and mineral screening must be performed before surgery.

For women planning pregnancy, the situation is slightly more strategic. Medically, there is no obstacle to becoming pregnant after this surgery. However, since pregnancy can loosen the repaired abdominal muscles and stretched skin again, it may affect the aesthetic result. Therefore, patients planning to have children in the near future are advised to postpone the operation until after childbirth.

Is smoking an obstacle to Lipoabdominoplasty surgery?

This issue is the most critical point of the surgical process and one that cannot be compromised. In plastic surgery literature, nicotine is considered the greatest enemy of surgical success. In operations such as lipoabdominoplasty where large tissue surfaces are reshaped, tissue oxygenation is of vital importance.

Cigarettes and nicotine products create a strong vasoconstriction effect in the body. Even after smoking a single cigarette, the capillaries in the body narrow and blood flow to the tissues decreases for hours. During tummy tuck surgery, while the vessels feeding the lower part of the skin reorganize, the tissue needs blood and oxygen like a flower needing water. In a patient who smokes, these nutrition pathways are blocked, and the risk of tissue death (necrosis) at the far ends of the surgical line increases dramatically.

This risk is not limited to cigarettes; electronic cigarettes, nicotine patches, or nicotine gums also create the same danger. The issue is not the smoke but the effect of nicotine on blood circulation.

Serious complications that nicotine use may cause include:

  • Tissue necrosis
  • Delayed wound healing
  • Opening in the suture line
  • Wound infection
  • Prominent and poor scar quality
  • Fat necrosis

For this reason, patients scheduled for surgery must completely stop all nicotine products at least 4 to 8 weeks before the operation. Passive smoking should also be avoided during this period. In fact, to ensure surgical safety, if the nicotine metabolite “cotinine” is detected in urine tests performed on the morning of surgery, the operation must be canceled to avoid risking patient safety. This rule is the most important precaution taken to protect the patient’s health.

What is Scarpa fascia and why is it preserved in the Lipoabdominoplasty technique?

One of the most important innovations that the lipoabdominoplasty technique has brought to modern medicine is the preservation of the “Scarpa fascia.” Many patients may hear this term for the first time, but it is one of the main factors determining postoperative comfort. Scarpa fascia is a thin but extremely strong membrane-like connective tissue located deep within the fat tissue of the abdominal region.

In older classical tummy tuck operations, this membrane was usually cut and removed. However, scientific studies have proven that preserving this structure surprisingly accelerates the recovery process. Just below the Scarpa fascia are the body’s lymphatic drainage channels and deep veins. When this structure is preserved, the body’s natural fluid drainage system also remains intact.

The advantages of preserving the Scarpa fascia are as follows:

  • Reduction of seroma risk
  • Shorter duration of drain use
  • Faster recovery process
  • Minimal loss of sensation
  • Increased tissue support

The issue of sensory loss is especially important for patients. After classical operations, patients may sometimes experience numbness in the abdominal area for months. However, in the lipoabdominoplasty method where the Scarpa fascia is preserved, the nerves carrying skin sensation are not damaged, so normal sensation returns much more quickly. In addition, this strong tissue carries the load on the suture line, acts as an “internal bra” against gravity, and prevents the scar from widening over time.

Can abdominal muscle separation (Diastasis) be treated with Lipoabdominoplasty?

Absolutely yes, and this is one of the most important functional benefits of the procedure. Especially pregnancy or excessive weight fluctuations can cause the “rectus abdominis” muscles, which form the front abdominal wall and are known as the six-pack muscles, to separate sideways. This condition, medically called “rectus diastasis,” does not resolve on its own no matter how much exercise is performed.

Patients usually apply with the complaint: “No matter how much I diet or do sit-ups, my abdomen still looks swollen as if I were pregnant.” The reason is not fat accumulation but the loosening of the “internal corset” structure that holds the abdominal organs. When the muscles separate, the internal organs protrude outward.

During lipoabdominoplasty, this muscle structure under the skin is exposed along its length. The separated muscles are brought together in the midline and sutured with strong, non-absorbable stitches. This process is called “plication.” This procedure restores the abdominal wall to its anatomically correct position and reduces the internal abdominal volume. The result is not only a flatter abdomen but also a more defined waist curve and the emergence of the “hourglass” shape.

Benefits provided by muscle repair include:

  • A flatter abdominal profile
  • A more defined waist curve
  • Correction of posture problems
  • Reduction of back pain
  • Possibility of umbilical hernia repair

Thanks to this repair, patients not only improve their aesthetic appearance but also gain increased core stability and noticeable relief in chronic back pain.

What are the stages of Lipoabdominoplasty surgery?

The surgical process consists of four main stages that progress in harmony with each other and each contributes to the excellence of the final result. This is a team effort and each stage is managed meticulously.

The first stage is comprehensive liposuction. Before the surgical incision is made, special solutions are injected into the abdominal region, waist area, and back to soften the fat tissue. Then the fat is broken down and aspirated using modern technologies such as Vaser (ultrasonic) or laser. This stage thins the tissue in a “tunnel-like” manner while preserving the vascular and nerve network under the skin. Thus the skin becomes freed and gains mobility without being traumatically separated from the underlying layer.

In the second stage, a low-level incision planned to remain within the underwear line is made. Through this incision, a controlled dissection is performed only along the midline where muscle repair will be performed. The navel is preserved on its stalk and separated from the surrounding tissue.

The third stage is the muscle repair mentioned above. The abdominal muscles are tightened, the internal abdominal volume is reduced, and the waist indentation is maximized. At this stage, the abdominal wall is essentially reconstructed.

The final stage is suturing and closure. Here, one of the most important advantages of modern surgery is the use of “progressive tension sutures” (quilting sutures). In the past, the skin was stretched and stitched only from the lowest part, which caused empty spaces to remain in between. Now the stretched skin is gradually fixed to the underlying fascia with special sutures. This technique prevents dead space formation, prevents fluid accumulation, and distributes tension across the abdominal surface so that the scar remains thinner.

How is pain management after surgery and is Lipoabdominoplasty painful?

One of the biggest concerns patients have before surgery is the pain they will experience. However, thanks to developments in anesthesia and pain management protocols, this process has become much more comfortable. Today, modern “opioid-sparing” approaches that reduce the use of heavy narcotic painkillers are adopted.

Among these approaches are TAP block (Transversus Abdominis Plane block) and Rectus Sheath Block. These procedures involve injecting long-acting local anesthetics into the nerves that provide sensation to the abdominal wall under ultrasound guidance during surgery. It works on a similar principle to a dentist numbing the area before tooth extraction, but its effect lasts much longer.

Thanks to these blocks, the patient does not feel severe incision pain when waking up. Within the first 24 hours, the pain is usually described as a muscle soreness, as if you had done very intense sit-ups at the gym. Sharp and unbearable pain is not expected. With the effect of the blocks and supportive painkillers administered intravenously, this process is passed quite comfortably. Good pain control also facilitates mobilizing the patient (early mobilization) a few hours after surgery. Early walking both regulates blood circulation and initiates the recovery process.

What awaits the patient during the recovery process and what should be considered after Lipoabdominoplasty?

The recovery process progresses in a linear course and the patient’s comfort increases with each passing day. Usually, a hospital stay of 1 or 2 nights is sufficient. During the return home and afterward, there are some important points the patient should pay attention to:

In the first days, the patient is asked to remain slightly bent forward (V position) while lying in bed and walking. This position reduces tension on the abdominal skin, allowing the patient to feel more comfortable and protecting the suture line. Generally, within 5–6 days the body adapts to this tension and the patient can stand fully upright.

Regarding drains; thanks to modern techniques and progressive tension sutures, drains now remain for a much shorter time, and in some cases they are not used at all. If they are placed, they are usually removed painlessly when the fluid amount decreases, either during discharge or about 3–4 days after surgery.

Points to consider during the recovery period are as follows:

    • Sleeping in the V position during the first week
    • Drinking plenty of water
    • Taking frequent short walks inside the house
    • Using prescribed medications regularly
    • Avoiding sudden movements
    • Limiting salt consumption

Although returning to social life varies from person to person, desk work and light social activities can usually be resumed by the end of the second week. However, heavy lifting and movements that strain the abdominal muscles should be avoided during this period. For driving, reflexes should return and abdominal pain should completely disappear to ensure safety in case of sudden braking.

Does corset use affect the result of Lipoabdominoplasty?

We can say that the corset is the “hidden hero” of this surgery. The medical corset worn after surgery is used not only to shape the body but also as a medical necessity and is an integral part of the recovery process.

External pressure is vital for the spaces formed under the skin and in areas where liposuction was performed to adhere together and for edema (swelling) to remain under control. The corset supports the abdominal wall and helps the patient feel secure while moving. It also contributes to wound healing by reducing tension on the suture lines.

Recommendations regarding corset use are as follows:

    • Wearing it day and night for the first 4 weeks
    • Not removing it except during showers
    • Being able to wear it under clothes
    • Wearing it over a cotton undershirt to prevent skin irritation

Usually after the 4th week, depending on the condition of the edema, it may be recommended to wear it only during the day or only at night for another 2 weeks. In total, a period of about 6 weeks may be mentioned. It is important that the corset has the correct tightness; it should not be so tight that it disrupts blood circulation nor so loose that it provides no support.

Are surgical scars permanent and how is the Lipoabdominoplasty scar hidden?

One of the topics patients are most curious about is scars. Since lipoabdominoplasty is performed with a surgical incision, leaving a scar is inevitable. However, the art of plastic surgery lies in hiding this scar in the best way and making it less noticeable over time.

The incision line is planned as low as possible according to the patient’s anatomy, below the cesarean scar and completely within the limits of bikini or underwear lines. Thus, when the patient wears swimwear or low-waist trousers, the scar is not visible from the outside.

Scar healing is a long process. While it appears as a thin line in the first weeks, it may become slightly red and more visible in the 2nd and 3rd months depending on the body’s healing mechanism. This is a completely normal biological process. From the 6th month onward, the color begins to fade, the scar flattens, and it approaches the skin tone. It may take 12 to 18 months for the scar to take its final form.

Things that can be done to improve scar quality include:

    • Using silicone-based gels
    • Applying scar tapes
    • Protecting the area from the sun
    • Not smoking
    • Keeping the skin moisturized

When are the results seen and what happens if I gain weight after Lipoabdominoplasty?

Immediately after surgery, the abdomen will already appear flatter and the sagging will have disappeared. However, this is not the final result. Due to surgical edema and swelling, it takes time for the details to appear. The first 3 weeks are the period when edema is most intense. From the 6th week onward, most of the swelling resolves and body contours begin to become more defined.

Especially the slimming of the waist area and the definition of abdominal muscles may take up to 6 months. Skin tightening (retraction) in areas where liposuction was performed and the adaptation of tissues to their new position require patience.

Fat cells removed from the body with lipoabdominoplasty do not return. In other words, the number of fat cells in the treated areas has been permanently reduced. However, other remaining fat cells in the body still have the capacity to enlarge in volume if excessive calories are consumed. If the patient maintains healthy nutrition and regular exercise habits after surgery, the results can remain permanent for life.

Possible risks in case of excessive weight gain include:

    • Increase in visceral (internal organ) fat
    • Re-stretching of abdominal skin
    • Loss of waist curvature
    • Deformation of the result

    Frequently Asked Questions

Lipoabdominoplasty includes applying liposuction to the abdomen and waist area in addition to the classic tummy tuck surgery. In this way, both sagging skin and excess fat tissue are removed in the same session.

It is suitable for individuals who have regional fat excess along with skin sagging, are in good general health, and are close to their ideal weight. It is especially frequently preferred in individuals experiencing body deformation after childbirth.

Thanks to liposuction, the waistline is also shaped along with the abdomen. This allows body contours to become more defined and provides a more proportionate and aesthetic appearance.

Pain, swelling, and bruising may occur in the first days. Most patients can return to work within 10–14 days. Full recovery usually takes 6–8 weeks as the areas where liposuction was applied also heal.

Yes, if necessary, the rectus muscles are sutured together to strengthen the abdominal wall. In this way, problems such as abdominal laxity or umbilical hernia can also be corrected.

Incisions are planned to remain within the bikini area. Liposuction entry points are very small and become less noticeable over time. Scars are kept to a minimum with aesthetic suturing techniques.

Generally, it is recommended to use a special medical corset for about 6 weeks. The corset helps reduce edema and stabilize the new body form.

Depending on the individual, an average slimming of 5–10 cm around the abdomen can be achieved. Results vary according to the amount of fat removed and skin elasticity.

When a healthy lifestyle is maintained, the results remain permanent for many years. However, situations such as weight gain or pregnancy may negatively affect the aesthetic results.

With the reshaping of body contours, self-confidence increases, clothing choices become easier, and body perception changes positively. This effect is especially more noticeable after childbirth.
