It both improves aesthetic appearance and, in advanced cases, provides functional benefits by expanding the field of vision.

Upper eyelid blepharoplasty is performed by removing excess skin and sometimes fatty tissue to correct the drooping lid structure. This procedure provides a younger and more dynamic facial expression and may also reduce complaints of eye fatigue.

In lower eyelid aesthetics, the goal is to smooth under-eye bags and tighten the loosened skin structure. The tired expression around the eyes is eliminated with this procedure. If necessary, laser-assisted methods can also provide improvement on the skin surface.

The postoperative recovery process is generally fast. Mild swelling and bruising may occur during the first few days; patients can mostly return to their daily lives within 7–10 days. Since scars are hidden within the natural eyelid creases, they become less noticeable over time and aesthetic integrity is preserved.

Things You Should Know

Information

Description

Blepharoplasty is a surgical intervention performed to correct aesthetic or functional problems such as drooping, puffiness, and excess skin on the upper and/or lower eyelids.

Purpose

To eliminate excess skin that obstructs the field of vision, correct a tired and aged facial expression, and rejuvenate the eye area.

Types of Procedure

Upper blepharoplasty (upper eyelid), lower blepharoplasty (bags and sagging in the lower lid), combined blepharoplasty (both lids together).

Type of Anesthesia

Usually performed with local anesthesia and sedation; class="ez-toc-section-end">

Eyelid surgery is a surgical procedure performed to correct the appearance of drooping, puffiness, and excess skin on the upper and/or lower eyelids. This operation, medically known as blepharoplasty, is performed by removing or repositioning excess skin and fatty tissue. It can help address both aesthetic concerns and functional problems that narrow the field of vision, and it provides a more refreshed facial expression.

Why Is the Eye Area the First Place Where Signs of Aging Appear on the Face, and How Does This Process Work?

The unquestionable epicenter of facial aesthetics is the eyes. However, when examined anatomically, it is not difficult to understand why this area ages so quickly. The thinnest skin in our body is on our eyelids. This delicate structure, thinner than even half a millimeter on average, is quite unprotected against external factors, much like a thin silk fabric. Sun rays, wind, makeup products, and even the blinking movement we perform thousands of times a day create constant wear on this thin skin.

Over time, the collagen fibers that provide the skin’s elasticity begin to loosen. But the issue is not limited to the skin. The muscle layer just beneath the skin and the fat pads deeper down that protect the eyeball against impacts are also affected by this process. In youth, there is a tight membrane called the “septum” that keeps these fat pads in place. You can think of this membrane like a dam wall. As age advances, this dam wall weakens and, unable to withstand the pressure of the water behind it (i.e., the fatty tissue), allows it to bulge forward. In fact, the formations we call “eye bags” are not newly formed masses but our normal anatomical structures that have shifted and herniated.

Factors that accelerate aging in this area include:

Genetic predisposition

Sun damage

Smoking

Insufficient sleep

Allergic disposition

Constant screen use

Incorrect cosmetic products

Excessive salt consumption

What Natural Results Are Targeted in the Modern Understanding of Blepharoplasty?

In the past, the approach to eyelid surgeries was quite simple; surgeons would remove as much sagging skin and herniated fat as possible. However, over time it was seen that this “remove and discard” philosophy could cause patients’ eyes to look hollow, take on a skeletal appearance, and paradoxically appear older in the long term. Today’s modern understanding of plastic surgery is built entirely on “volume preservation” and “repositioning.”

The goal is no longer to eliminate tissues but to move them back to where they belong. One of the most important signs of youth in the face is fullness. Removing fullness around the eyes and leaving only tight skin gives the person a frozen and artificial expression. In modern techniques, especially in the lower eyelid, the fats that cause puffiness are not removed; instead, they are spread into the hollows just beneath the eye, called the “tear trough.” This process is somewhat like using the person’s own tissue as a filler. Thus, the sharp transition between the cheek and the eyelid is softened and the play of light and shadow is balanced. Ultimately, the target is not for the patient to look “operated on,” but to look refreshed as if they have just returned from a long and pleasant vacation.

Essential goals in modern surgery include:

Brighter-looking eyes

Elimination of a tired expression

Preservation of eye shape

Natural eyelid crease

Volume restoration

Concealment of scars

Personalized planning

Is Upper Eyelid Aesthetics Performed Only for Beauty, or Does It Also Solve Health Problems?

When upper eyelid aesthetics is mentioned, the first thing that comes to mind is a younger appearance, but this surgery also has serious functional benefits that affect patients’ quality of life. Excessive skin sagging, which we call dermatochalasis, in advanced stages piles over the eyelashes and creates a curtain-like effect. This limits the upper and outer parts of a person’s field of vision. In order to see the world from a wider angle and lift that heaviness off their eyes, the patient unconsciously feels the need to constantly raise their eyebrows.

This reflex movement causes the “frontalis” muscle in the forehead to remain contracted throughout the day, even for years. Many patients complain of persistent headaches starting in the later hours of the day, felt in the forehead and temples, but they do not know that the cause is their eyelids. After surgery, one of the most common sentences patients say is, “How bright the world is,” while another is, “That chronic heaviness and pain in my forehead is gone.” Therefore, this surgery is not only an aesthetic touch that prevents makeup from smudging, but also a functional treatment that increases visual comfort and provides neurological relief.

Functional problems caused by upper eyelid drooping include:

Narrowing of the visual field

Eye fatigue

Tension-type headaches

Increase in forehead wrinkles

Difficulty reading

Difficulty driving

A stinging sensation in the eyelashes

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Why Do Under-Eye Bags and Hollows Occur Together in the Lower Eyelid, and How Are They Treated?

Lower eyelid aesthetics is one of the most sophisticated areas of facial surgery and requires detailed planning. Patients often see both swelling and a deep dark circle or hollow under their eyes when they look in the mirror. Although this may seem contradictory, it is an anatomically interrelated process. With aging and the effect of gravity, cheek tissue sags downward. This sagging makes the boundary between the lower eyelid and the cheek more pronounced and creates a deep valley called the “nasojugal groove.”

At the same time, the fat pads that protect the eye herniate forward, forming a mound just above this valley. This “mound” (fat bag) and “valley” (tear trough) side by side cause light to break unevenly on the surface. While light hits the mound, the inside of the valley remains in shadow. In fact, the biggest cause of those dark circles and bruising under the eyes is this shadowing. The main strategy in treatment is to level this uneven terrain. Rather than removing and discarding the fats that form the bag, the surgeon transfers these fats like a bridge into the valley below (the hollow). Thus, both the mound lowers and the valley fills. Light is now distributed evenly across this area, and the tired expression gives way to a smooth transition.

Signs seen in suitable candidates for this procedure include:

Prominent under-eye bags

Tear trough hollowing

Sagging cheek tissue

Under-eye dark circles

Deepened lines

Loss of elasticity

Why Are Preoperative Measurements and Analyses the Key to Success?

It should not be forgotten that success in aesthetic surgery is not a coincidence, but the result of good planning. Each individual’s facial anatomy, bone structure, position of the eyeball, and skin quality are different from one another like a fingerprint. Therefore, there is no “one technique that fits everyone.” The preoperative examination is the most critical stage in which the roadmap of the surgery is drawn.

Especially in lower eyelid surgery, the tone and elasticity of the lid must be tested. When the surgeon gently pulls the lower eyelid down with a finger and releases it, they check how quickly the lid returns to its original position (Snap test). If the lid returns slowly or is loose, a standard skin removal procedure may be risky, because a loose lid can be pulled downward during the healing process. In this case, the surgeon must add supportive additional procedures to the plan. In addition, whether the patient has complaints of dry eye is a vital detail. In patients with low tear quality, the surgical plan should be more conservative (protective). These detailed analyses are indispensable for avoiding postoperative surprises and achieving the most natural result.

Critical parameters evaluated during the examination include:

Eyebrow position

Eyelid crease height

Skin quality

Location of fat bags

Tear production

Eyelid closure strength

Structure of the bony framework

Presence of asymmetry

For Which Patients Is Scarless Eyelid Aesthetics (Transconjunctival Approach) Suitable?

With the development of technology and refinement of surgical techniques, it has become possible in certain patient groups to perform completely “scarless” surgeries without making any external incision. In this method, referred to in medical literature as “transconjunctival blepharoplasty,” the incision is made not through the skin but through the pink mucosal tissue (conjunctiva) on the inner side of the lower eyelid.

This technique is the gold standard especially for young and middle-aged patients who have no excess skin, whose skin is still tight and elastic, but who have genetically prominent eye bags. Since there are no external stitches, the recovery process is incredibly fast. The biggest advantage of this method is that the muscle layer surrounding the eye (orbicularis) is not cut at all. Because muscle integrity is preserved, the natural expression of the eye is not disturbed, facial expressions are not affected, and postoperative complaints such as bruising and swelling remain at a minimum level. If the patient has very mild skin laxity, a combined rejuvenation can be achieved by adding laser skin tightening to this internally performed surgery.

The main advantages provided by this method include:

No visible scarring

Preservation of muscle tissue

No distortion of eye shape

Fast recovery process

Natural facial id="How_Do_Almond_Eye_Aesthetics_and_Canthopexy_Procedures_Change_the_Look">How Do Almond Eye Aesthetics and Canthopexy Procedures Change the Look?

“Almond Eye” or “Slanted Eye” aesthetics, which we have often heard in recent years with the influence of social media, are actually procedures related to the position of the outer corner of the eye. In an ideal and youthful eye structure, the outer corner of the eye is a few millimeters higher than the inner corner. However, when the outer corner is lower due to structural features or aging, the person may develop a sad, melancholic, or tired expression.

Canthopexy and canthoplasty are two different techniques used to correct this condition. Canthopexy is a more conservative procedure generally performed to strengthen and support the lower eyelid. Here, the shape of the eye is not changed dramatically; the existing structure is simply reinforced and made more resistant to aging. Canthoplasty, on the other hand, is a more radical procedure; the tendon at the outer corner of the eye is released and re-suspended from a higher point on the periosteum. This procedure truly pulls the eye and gives it an almond shape. However, the important point here is that this slant should not be exaggerated. Not every face type can carry an overly slanted eye. The goal is not to transform the person into someone else, but to achieve a firm and energetic look that is harmonious with facial features.

The effects of outer eye corner aesthetics include:

A more slanted eye shape

Elimination of a sad expression

Repair of lower lid laxity

A younger appearance

Harmony with facial dynamics

Addition of depth to the gaze

How Does Laser Technology Affect the Recovery Process in Eyelid Surgeries?

The instruments used in surgery directly affect the comfort of the outcome. While scalpels or electrocautery devices are used in traditional methods, today carbon dioxide () laser technology has revolutionized blepharoplasty surgeries. The working principle of the laser is to seal blood vessels instantly with heat energy while cutting the tissue.

When working in a highly vascular area such as the eyelids, having almost no bleeding provides tremendous visual clarity for the surgeon. This means protecting these delicate anatomical structures. For the patient, the biggest advantage of this is experiencing much less bruising and swelling in the postoperative period. The “black eye” appearance seen in patients operated on with the classical method is much less common with laser use. In addition, the controlled heat emitted by the laser triggers collagen production along the incision line and helps tighten the skin. A fractional laser pass applied to fine wrinkles around the eyes at the end of the surgery improves the surface quality of the skin and creates an “ironing effect.”

The distinguishing features of laser-assisted surgery include:

Minimal bleeding

Reduced edema and bruising

Short operation time

High precision

Skin-tightening effect

Fast return to social life

Less pain sensation

What Is the Difference Between Eyelid Drooping (Ptosis) and Skin Sagging?

The two concepts that our patients most frequently confuse are skin sagging and true eyelid drooping (ptosis). In fact, they are completely different mechanisms and their treatments are different as well. Blepharoplasty is only the removal of excess skin and fat. However, if the patient has ptosis, the problem is not in the skin but in weakness of the muscle that lifts the eyelid (levator muscle) or its detachment.

The easiest way to understand this is: If, when you lift your eyebrows up with your hand, your eyelid still covers your pupil, you probably have ptosis. In these patients, the eyelid covers the colored part of the eye (iris) and the pupil more than normal, which gives the person a constantly sleepy, faint, or as if intoxicated expression. If only skin removal surgery is performed on such a patient, the eyelid will still remain low, and the result will not be aesthetically satisfying because the distance between the eyebrow and the eyelashes will narrow. In the presence of ptosis, a procedure aimed at the muscle—such as a “shortening” or “strengthening”—must be added to the surgical plan. During this procedure, while the patient is under light sedation, they are asked to open and close their eye so that millimetric adjustments can be made.

Signs of ptosis include:

Covering of the pupil

Sleepy gaze

Constant need to raise the eyebrows

Increasing eye fatigue in the evenings

Asymmetric eye opening

Tilting the head backward (chin lifting)

What Should Patients Pay Attention to During the Recovery Process, and How Does the Process Progress?

Contrary to what is feared, eyelid surgeries are not painful procedures. Patients usually say right after surgery, “I feel a stinging in my eye” or “My eyelids feel tight,” but severe pain is rare. The golden rule of the recovery process is the first 48 hours. During this period, applying cold therapy (ice compresses) in a disciplined manner prevents 70% of swelling and bruising.

Keeping the head above body level in the first days, i.e., sleeping with two pillows, helps the edema drain downward with the help of gravity. Stitches are usually very fine and are placed in aesthetically hidden areas (the eyelid crease or lash line). On average, these stitches are removed within 5 to 7 days. After the stitches are removed, the patient can return to social life by applying concealer makeup. In the first weeks, tearing, sensitivity to light, or blurred vision may occur; these are a natural part of the healing process and are temporary. Artificial tear drops make this process quite comfortable.

Things to avoid during recovery include:

Strenuous sports

Tasks performed while bending forward

Hot baths and saunas

Contact lens use

Rubbing the eyes

Direct sunlight

Blood-thinning medications

Excessively salty foods

What Are the Possible Complications and Risks?

As in every field of medicine, there is no such thing as zero risk in blepharoplasty. However, in experienced hands and with the right techniques, these risks are minimized. The most common and usually temporary issues are asymmetries and prolonged edema. Sometimes one eye may heal faster than the other, creating a temporary appearance of asymmetry; patience is required.

More serious complications include outward turning of the lower eyelid (ectropion). This usually occurs when too much skin is removed or when a loose lid is not supported sufficiently. If detected early, it can be resolved with massage and medical treatments; in the late period, it can be corrected with small revision surgeries. Another condition is “lagophthalmos,” meaning the inability to fully close the eye. This can also be due to excessive skin removal and may lead to drying of the cornea. Therefore, surgeons always prefer to stay within the “safe margin”; leaving a little extra skin is always better than the inability to close the eye. The most feared complication, bleeding behind the eye (retrobulbar hematoma), is extremely rare but requires urgent intervention. For this reason, it is important for patients to contact their doctors without delay if they experience severe pain and vision loss after surgery.

Rare but possible risks include:

Temporary dry eye

Infection

Wound healing problems

Permanent asymmetry

Eyelid malposition

Visual disturbances

Hematoma (blood accumulation)

Eyelid aesthetics is one of the surgeries with the highest patient satisfaction when performed at the right time and in the right hands. Remember, the goal is not to change you, but to remove the mask of fatigue on your face and bring back the energetic, vibrant, and healthy expression underneath. Regaining the sparkle in your eyes will positively affect not only your appearance in the mirror but also your outlook on life.

Frequently Asked Questions

How can dry eyes be prevented after blepharoplasty?

Artificial tear drops are used for dry eyes after surgery.
