One of the most commonly preferred methods is transferring the big toe to replace the thumb.

This operation is performed to regain grasping, holding, and fine motor skills after the loss of the thumb or other critical fingers. The toe is harvested together with its blood vessels, nerves, and tendons, and is transplanted to the hand using microsurgical techniques.

The transferred finger can move functionally and offers a natural appearance aesthetically. Thumb reconstruction can restore up to 40% of hand function. When necessary, second toes can also be transferred for other fingers.

Long-term rehabilitation and physical therapy are required after surgery. Sensation, movement, and grip strength improve over time. This procedure is of great importance for patients with finger loss to maintain independent daily living and return to work.

What You Need to Know

Information

Definition

Toe-to-hand finger transfer is the microsurgical transplantation of a toe to the hand under a microscope to restore finger function in people who have lost fingers due to trauma, congenital anomalies, or tumors.

Purpose

To restore grasping, holding, and fine motor skills; improve hand aesthetics and function; enhance the patient’s capacity for independent living.

Areas of Application

Finger amputations (especially the thumb), hand deformities, multiple finger deficiencies, congenital absence of fingers.

Surgical Methods

Big toe transfer (hallux transfer): The most commonly performed method.Second or third toe transfer: Chosen for aesthetic and functional harmony.Double toe transfer: In cases where more than one finger is missing.

Type of Anesthesia

Performed under general anesthesia.

Operation Duration

Depending on the number of transfers and complexity, it may take 6–12 hours.

Recovery Process

Hospital stay is typically 7–10 days; full recovery and rehabilitation may take 3–6 months. class="ez-toc-section" id="What_Is_Toe-to-Hand_Finger_Transfer_Surgery">What Is Toe-to-Hand Finger Transfer Surgery?

Toe-to-hand finger transfer surgery is the microsurgical transplantation of a toe to the hand in patients who have lost a finger due to trauma or congenital deficiency. The goal is to restore grasping and holding function of the hand. Blood vessels, nerves, tendons, and bone structures are precisely connected. Proper patient selection and an intensive rehabilitation process play a critical role in the functional success of the surgery.

Why is our hand so important and why is finger transfer necessary?

The complexity and functional capacity of the human hand is one of the most fundamental features that distinguishes us from other living beings. The hand is the brain’s extension into the outside world. It does not only hold objects; we touch our loved ones, feel warmth, find our way in the dark, and express ourselves. The most critical player of this extraordinary structure is undoubtedly the thumb. Thanks to its ability to oppose the other fingers, the thumb gives us the power to grasp, hold, and manipulate objects.

Nearly half of the hand’s functions are provided by the thumb. Loss of this finger causes the hand to lose much of its identity as a “hand.” Simple actions such as buttoning a shirt, holding a pen, or grasping a glass become impossible. Finger transfer comes into play at exactly this point. Instead of prostheses that are non-living, insensate, and limited in movement, we aim to rebuild these functions using living tissue taken from the patient’s own body.

The main goals of this surgery are:

Grasping ability

Precision pinch

Sensory restoration

Aesthetic integrity

Psychological recovery

How has finger transfer technology developed historically?

Medical history is full of efforts to compensate for limb loss. For centuries, hooks, wooden prostheses, or purely cosmetic gloves—familiar from pirate movies—were used. However, none of these could restore lost sensation and fine motor movements. The real turning point came with the development of optical magnification systems and microsurgical instruments.

About 50 years ago, the discovery that blood vessels and nerves could be sutured under a microscope opened a new era. The first successful finger transfers were seen as science fiction. Since then, the fineness of sutures, the image quality of microscopes, and surgical techniques have advanced at an incredible pace. Today we can safely connect vessels far thinner than a human hair and repair not only the finger itself but also the neural networks that allow it to feel. Cases once considered “impossible” have become routine procedures in experienced hands.

Which patients are suitable candidates for finger transfer?

We do not recommend this surgery to every patient who has experienced finger loss. This is a serious decision-making process and a path that the patient and surgeon walk together. The patient’s overall health, expectations, and lifestyle are determining factors. In general, we plan this surgery in cases where essential hand functions are severely impaired.

Common indications include:

Thumb amputations

Multiple finger loss

Metacarpal-level loss

Congenital hand anomalies

Constriction band syndrome

Severe burn sequelae

Young and active patients who have lost the thumb due to occupational accidents or traffic accidents benefit the most, because hand function is vital for their professional and social lives. However, in patients with advanced diabetes, heavy smoking, or compromised blood vessels (atherosclerosis), we approach more cautiously because risks may outweigh benefits.

Which toe is chosen as a donor and will walking be affected?

This is the question I hear most often in the clinic—the moment I see the most concern in my patients’ eyes: “If you take a toe from my foot, will I be unable to walk?” or “Will I limp?”

This concern is completely natural, and the answer is reassuring: No, your ability to walk will not be permanently impaired. The foot has an extraordinary ability to adapt in terms of weight-bearing and balance. In donor selection, we typically have two main options, each with its own advantages:

Second Toe:

This is the most commonly preferred donor site. Removing the second toe does not disrupt the arch of the foot or the mechanics of stepping. Aesthetically, the gap in the foot tends to narrow over time as the other toes move closer, making it less noticeable. It is an excellent option for reconstructing long fingers or creating a thumb in children.

Big Toe:

This toe is the closest match to the thumb in size, strength, and nail structure. If our goal is a very strong grip and an aesthetically excellent thumb appearance, we consider this option. However, removing the entire big toe may cause a slight loss of power during the “push-off” phase of walking. For this reason, we often use a special technique called the “wrap-around” method. In this technique, the bone and joint of the big toe are left in place; only the nail, skin, and soft tissue envelope are harvested and “wrapped” over the hand’s bony framework. This provides an excellent thumb while preserving foot function.

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

What is considered during the preoperative preparation process?

A successful microsurgical procedure begins long before the scalpel touches the skin. The preparation phase is a period during which the surgeon mentally simulates the operation repeatedly. First, we perform detailed vascular mapping.

Methods used in this process include:

Physical examination

Hand radiographs

Color Doppler ultrasonography

CT angiography

MR angiography

The condition of the vessels in the injured hand varies depending on the severity of the trauma. Sometimes we may need to find healthy vessels higher up, such as in the forearm. Likewise, we must know vascular variations in the foot in advance. In about 10–15% of people, the dorsalis pedis artery follows a different course than expected. Creating this “road map” is essential to avoid surprises during surgery.

In addition, quitting smoking is the most critical prerequisite. Smoking causes vasoconstriction, disrupts blood flow, and directly threatens the success of such a delicate operation. We clearly explain to our patients that they must stop smoking completely at least 4–6 weeks before surgery.

What happens in the operating room during microsurgery?

These operations are considered a “marathon” of surgery. They usually take 6 to 12 hours and require intense concentration. To shorten the duration and reduce anesthesia time, two teams often work simultaneously: one prepares the toe, while the other prepares the recipient site in the hand.

The technical stages follow a specific sequence. First, the bony framework is established. The transferred toe is fixed to the hand bone using plates, screws, or wires. Then the tendons that will move the finger are repaired. This establishes the mechanical linkage that allows bending and straightening.

The final and most critical stage is vascular and nerve repair performed under the microscope. Using sutures finer than a hair, arteries and veins that are about 1–2 mm in diameter are connected. When the vascular clamps are released, seeing the pale, white toe begin to turn pink—watching blood reach the fingertip—is an indescribable moment of happiness for everyone in the operating room. Finally, the nerves are repaired and the skin is closed.

Why are the first 72 hours after surgery so critical?

Leaving the operating room does not mean the job is finished. In microsurgery, there is a “first 72 hours” rule. This period carries the highest risk of clot formation at the vascular connections. For this reason, we monitor our patients very closely. Even room temperature matters; the environment must be kept warm to prevent the patient from getting cold and the vessels from constricting.

Parameters we monitor include:

Finger color

Finger temperature

Capillary refill time

Finger turgor

Bleeding response

If the finger starts turning purple, it suggests a venous problem; if it becomes pale and cold, it indicates arterial inflow compromise. In such a situation, we may need to decide within minutes and take the patient back to the operating room. We also use specific fluids and medications to thin the blood and reduce clot risk. In some cases, we may even utilize medical leech therapy to relieve venous congestion. This period is sleepless for both the patient and the doctor—but absolutely worth it.

How does the rehabilitation process work and when can the patient use the hand?

Finger transfer is not only a surgical procedure but also a long-term training process. In the first weeks after surgery, the finger is protected in a splint. When bone union and tendon healing begin—usually taking about 3–4 weeks—physical therapy starts.

Stages of rehabilitation include:

Passive movements

Active movements

Strengthening exercises

Sensory re-education

Fine motor skills training

At first, the therapist moves the finger; later, the patient begins active movement. One of the most fascinating aspects is the brain’s adaptation. In the early period, when the patient touches the transferred finger, it may feel as if they are touching a toe. The brain has “coded” that tissue as part of the foot for years. Over time, with sensory re-education, the brain accepts the new situation and processes signals as a “hand finger.”

Sensory return depends on nerve healing speed. Nerves regenerate at roughly about 1 mm per day. Therefore, sensation reaching the fingertip can take 6 months to 1 year depending on the length of the transfer. Temperature and pain are usually the first sensations to return; fine touch typically develops later.

#col-643131921 > .col-inner {
margin: 0px 0px -45px id="Is_finger_transfer_in_children_different_from_adults">Is finger transfer in children different from adults?

Pediatric patients are a unique group in reconstructive surgery. Transfers performed in children have important advantages and disadvantages compared to adults. The greatest advantage is children’s healing potential and their brain’s (neuroplasticity) ability to adapt. Children learn to use the transferred finger as a natural part of their hand in a short time.

Key points we consider in children include:

Growth plates

Vessel diameter

Age at surgery

Family compliance

When transferring a finger in children, we pay special attention to protecting the “growth plate” (epiphysis), which enables bone elongation. This allows the transferred finger to grow and develop as the child grows—a biological miracle that no prosthesis can provide. We generally aim to perform these surgeries before the child reaches school age—preferably between 2 and 4 years old—before they enter the period of holding a pencil and social interaction. However, because children’s vessels are much thinner, the technique requires even greater precision.

What are the possible complications and risks?

As with any surgical procedure, this complex operation carries risks. We discuss these openly and transparently with our patients. Success rates in experienced centers are above 95%, but in medicine there is no such thing as zero risk.

Potential issues include:

Arterial thrombosis

Venous insufficiency

Infection

Tendon adhesions

Joint stiffness

Cold intolerance

The most feared early complication is vascular thrombosis, in which urgent intervention is performed to try to salvage the transferred finger. In the late period, finger motion may remain limited due to tendon adhesions. In such cases, tendon release surgeries called “tenolysis” may be required. In addition, the transferred finger may be more sensitive to cold for a period. However, all of these risks are manageable compared to the major benefit of restoring hand function.

How are the aesthetic results and adaptation to the new hand?

Aesthetics is an important part of this procedure, just like function. Of course, a toe is not an exact replica of a finger; it may appear slightly more knobby or fuller. But for us, “aesthetics” is not only about looking beautiful—it is about looking “normal.” Having a complete five-finger hand means the person can take their hand out of their pocket in social settings and shake hands without hesitation.

The psychological change we see in our patients is remarkable. Watching individuals who used to hide their hands use them confidently after surgery is the greatest reward for us as surgeons. With its nail structure, skin texture, and fingerprint, the transferred finger becomes a living part of you that also ages with you. Over time, scars fade, the finger refines, and it integrates with the overall structure of the hand.

Why is this surgery more than an “organ transplant”?

When people think of organ transplantation, kidneys, liver, or heart usually come to mind—organs that sustain life. Hand or finger transfer, however, is not about “keeping someone alive,” but about “helping them live.” It restores independence, productivity, and social presence.

The success of this operation depends not only on the surgeon’s skill but also on the patient’s determination. Avoiding smoking, staying consistent with rehabilitation exercises, and being patient are essential.
