By removing excess breast tissue and skin, the breasts are brought into proportion with the body, and the patient’s quality of life is improved.

Heavy breast volume can lead to back, shoulder, and neck pain, postural problems, and skin irritation. Breast reduction addresses these complaints while also improving aesthetics by repositioning the nipple to a higher, more youthful location.

The operation is usually performed under general anesthesia and takes about 2–4 hours. Incisions may be made around the areola and extend vertically down to the breast fold, and in some cases an additional incision along the fold is added to create an inverted-T (anchor) pattern. After surgery, a few days of rest are recommended, and full recovery may take 4–6 weeks.

Following breast reduction, the breasts become more lifted, symmetrical, and natural in appearance. Breastfeeding function and nipple sensation are typically preserved. The procedure offers long-lasting results, providing not only aesthetic improvement but also significant physical relief.

What You Need to Know

Information

Definition

Breast reduction (reduction mammaplasty) is a surgical procedure performed in large and sagging breasts to reduce volume and improve shape by removing excess breast tissue, fat, and skin.

Purpose

To relieve back, shoulder, and neck pain and posture problems caused by overly large breasts, facilitate physical activity, and achieve a more aesthetic appearance.

Types of Procedures

Vertical (lollipop), inverted-T (anchor), and short-scar techniques; the nipple is repositioned and a lift is performed during the same procedure.

Type of Anesthesia

Usually performed under general anesthesia.

Operation Duration

On average, 2–4 hours.

Recovery Process

Rest is recommended during the first week. class="ez-toc-section" id="What_Is_Breast_Reduction_Surgery">What Is Breast Reduction Surgery?

Breast reduction surgery (reduction mammaplasty) is a procedure performed to relieve the physical and psychological discomfort caused by large, heavy breasts. Excess breast tissue, fat, and skin are removed; the breast is reshaped, and the nipple is moved to an ideal position. Very large breast volume may cause back, neck, and shoulder pain, posture issues, and skin problems. The aim is to achieve a lighter, more proportional, and aesthetically pleasing breast appearance.

Who needs breast reduction, and who are the right candidates?

Breast hypertrophy—breasts that are disproportionately large relative to body size—may not be fully understood from the outside, but it can be extremely burdensome for those experiencing it. Many patients seek consultation with symptoms that begin in adolescence and progressively worsen over the years. These complaints are not limited to aesthetic concerns; they often include daily physical limitations:

Common physical complaints include:

Chronic neck pain

Back pain

Deep bra-strap grooves on the shoulders

Rashes under the breast fold

Fungal infections

Unpleasant odor

Poor posture

Restricted physical activity

Shortness of breath when lying on the back

The psychological dimension can be just as significant as the physical one. Women with very large breasts often choose loose clothing to hide their body contours. Especially in younger patients, this may lead to avoiding social environments, feeling uncomfortable at the beach or pool, and developing serious self-confidence issues. If physical development is complete, there is no uncontrolled hormonal imbalance, and expectations are realistic, you may be a good candidate. The goal is not “perfection,” but a healthier, more proportionate body you can feel at ease in.

How do breast anatomy and vascular structure influence surgical planning?

As a surgeon, I approach breast reduction like an architect approaching a restoration project. It is not enough to simply remove tissue and make the breast smaller; we must also preserve the “electrical wiring” (nerves) and “water pipes” (blood vessels) so the system continues to function safely. Mastering the deep anatomy of the breast is one of the most critical aspects of this operation.

The breast is not a random fat mass. It is a living organ containing milk ducts, blood vessels, and delicate nerve networks. During planning, we design a structure called a “pedicle.” The pedicle is a bridge of living tissue that preserves the blood supply and nerves while the nipple-areola complex is moved to its new position.

Pedicle choice depends on:

Breast size

Degree of nipple descent

Skin quality

Patient age

Smoking history

For example, in very long and sagging breasts where the nipple must be moved significantly upward, we must select the pedicle approach with the safest circulation. Otherwise, compromised blood flow can lead to tissue loss. This is why not every patient receives the same technique—your anatomy defines the surgical roadmap.

What should be considered during the preoperative preparation period?

Once we decide on surgery, we begin a detailed process to prepare you for a safe operation. This starts well before you enter the operating room. First, we must confirm that the breast tissue is healthy. For this reason, I request an up-to-date mammogram for every patient over 40. For patients under 40, breast ultrasound is usually sufficient. These studies both help detect any hidden masses and provide a baseline for postoperative follow-up imaging.

The strictest rule in preparation is smoking. Smoking constricts small blood vessels and significantly reduces blood flow. In breast reduction, proper healing and nipple viability depend on microcirculation. In smokers, wound healing becomes more difficult, openings may occur along incision lines, and the most serious concern—risk of nipple necrosis—increases. Our goal is to achieve the best shape with the least scarring, but avoiding scars to the point of sacrificing shape can result in a less satisfactory, shapeless breast over the long term.

Common techniques include:

Vertical mammaplasty (Lollipop technique):

This method is generally suitable for moderately large breasts without extreme sagging. The scar resembles a lollipop: a circle around the areola plus a vertical line down. There is no horizontal scar in the inframammary fold. It can provide excellent lift and reshaping, but it may be insufficient when a large amount of skin must be removed.

Inverted-T (Anchor) technique:

This is the gold standard for very large (gigantomastia) and severely sagging breasts. If the nipple sits 10–15 cm (or more) below its ideal position, skin must be tightened in both vertical and horizontal dimensions. The scar pattern includes an areolar circle, a vertical line, and a horizontal line hidden in the breast fold—commonly called the “anchor” scar. This approach gives the surgeon more freedom to shape the breast and can better resist recurrent sagging in the long term.

Key factors guiding technique selection:

Breast volume

Degree of sagging

Skin elasticity

Nipple position

Is it possible to reduce breasts with liposuction alone?

This is a very common question. Liposuction can be an important adjunct in breast reduction, but it is rarely a complete solution on its own. The breast is not made of fat only; it also contains firm glandular tissue and connective tissue. Liposuction can remove fatty components, but it cannot effectively remove glandular tissue.

In addition, most patients seeking reduction have excess skin and sagging. Think of a balloon: when you remove the “contents,” the balloon shrinks but the skin becomes loose and droops. If we do not excise and tighten the skin, liposuction alone can leave a smaller but more deflated, more sagging breast.

We commonly use liposuction to:

Reduce axillary (underarm) fullness

Slim the lateral chest area

Smooth transitions

Improve bra-line bulges

This combined approach helps the breast blend more naturally with the chest wall and produces smoother-looking results.

How are nipple sensation and blood supply protected during surgery?

One of the biggest fears in breast reduction is loss of sensation. Nipple sensation and sexual responsiveness are important for many women. The pedicle technique is specifically designed to preserve these functions by protecting the blood vessels and nerves supplying the nipple.

Nipple sensation is primarily carried by branches of intercostal nerves (especially the lateral branch of the 4th intercostal nerve). Surgeons understand these pathways and preserve the tissue block where these nerves travel. With medial or superomedial pedicle techniques in particular, nerve networks are often safely maintained within the preserved tissue bridge.

That said, temporary sensation changes are common after surgery. This may present as numbness or, in some cases, hypersensitivity (a “shock-like” feeling). This is usually related to swelling and temporary nerve irritation.

What to know about sensory recovery:

Numbness is common in the first weeks

Tingling sensations often appear over time

Full recovery may take 6–12 months

Recovery is typically faster in non-smokers

Younger patients often adapt more easily

In extremely large breasts, the risk of sensory loss is somewhat higher because nerves have been stretched for years and the nipple must be moved a longer distance. Still, permanent complete loss of sensation is rare.

How does surgery day and the hospital process work?

On surgery day, you arrive fasting in the morning. After you settle into your room, we perform standing examinations and markings. These markings are the surgical map and must be done while standing because breast shape changes when lying down. After markings, the anesthesia team evaluates you, and you proceed to the operating room.

The operation is performed under general anesthesia, meaning you are fully asleep and feel nothing. Depending on breast size, the procedure usually lasts about 2.5–4 hours. At the end of surgery, we may place thin tubes called drains to remove fluid and blood that could otherwise collect. The word “drain” can sound intimidating, but drains are often a major ally in reducing swelling and bruising.

Typical hospital steps include:

A medical support bra is applied after surgery

You are encouraged to walk after 4–6 hours

Light food intake begins

Pain is controlled with medication

Usually a 1-night hospital stay

Drain output is checked the next day

If drain output is minimal, drains are removed before discharge. Removal takes only seconds and is usually not painful.

#col-80799821 > .col-inner {
margin: 0px 0px -45px id="What_is_the_recovery_like_and_when_can_you_return_to_normal_life">What is the recovery like, and when can you return to normal life?

Pain after breast reduction is often what surprises patients the most—in a positive way. Many say, “Doctor, it was much easier than I expected.” The sensation is usually not sharp pain but rather muscle soreness and pressure, like you did a very intense workout. This is typically well controlled with simple pain relievers.

After returning home, we recommend resting for the first few days, but this does not mean staying in bed all day. Short walks around the house improve circulation and support healing.

Key recovery recommendations include:

Sleeping on your back

Avoiding lifting arms overhead forcefully

Not lifting heavy objects

Keeping the supportive bra on as advised

Staying well-hydrated

Eating a protein-rich diet

Showering is usually allowed around day 3 or 4. Returning to social life and desk-based work is typically possible within about 7–10 days. However, returning to the gym, swimming, or heavy exercise usually requires about 6 weeks of patience.

In the first weeks, the breasts may look higher, firmer, and somewhat misshapen. The lower part may appear flat while the upper part looks overly full—this is normal. Surgeons often call this the “boxy” appearance. As swelling decreases and tissues soften, the breast settles into a natural shape.

Are scars permanent, and how do they change over time?

Honesty matters in plastic surgery: wherever the skin is cut, a scar forms. There is no truly “scarless” breast reduction. The art is placing scars where they can be concealed and ensuring they heal as finely as possible.

Scar maturation is a living process. Immediately after surgery, scars may appear as thin lines. From about the first month, scars can turn red, become more noticeable, and sometimes feel firm as part of normal healing. This stage can be worrying but is temporary. After around 6 months, scars usually begin to fade, soften, and approach a more skin-toned color.

Factors affecting scar quality include:

Genetic tendency

Smoking

Skin tone

Postoperative care

Use of silicone sheets

Sun protection

Many patients report that by the one-year follow-up, scars are no longer a major concern—especially because they are hidden under a bra and have faded significantly. The comfort and aesthetic improvement often far outweigh the presence of fine scars.

Does this surgery affect breastfeeding or cancer risk?

For younger women considering breast reduction, the biggest question is often: “Will I be able to breastfeed?” With modern techniques, we aim to preserve the nipple and the main milk duct system as a functional unit. Because some gland tissue is removed, milk production may decrease somewhat. However, since the core duct structures are usually preserved, the ability to breastfeed often remains.

That said, medicine can rarely offer a 100% guarantee. Except for rare techniques where the nipple is completely detached and grafted (free nipple graft), the chance of breastfeeding is generally high.

Regarding breast cancer: this surgery does not increase cancer risk—and may actually reduce it. When breast tissue volume decreases, the amount of tissue where cancer could potentially develop also decreases. In addition, every removed tissue specimen is sent to pathology for microscopic examination. This can sometimes detect tiny, hidden abnormalities that were not visible on imaging, making the procedure somewhat “check-up” like in that sense.

What are the possible risks and complications?

Every surgical procedure carries risks, and the best approach is to proceed with awareness. Breast reduction is generally safe, but there is no such thing as risk-free surgery—there are manageable risks.

Potential issues include:

Hematoma

Infection

Wound opening

Fat necrosis

Asymmetry

Sensation changes

Allergic reactions

The most common issue is small wound openings at the “T-junction” in the inverted-T technique, where blood supply is naturally weaker. These typically heal with dressing care. The most feared complication—nipple necrosis—is very rare and most often associated with heavy smoking or uncontrolled diabetes.
