The nipple-areola complex is repositioned upward, excess skin is removed, and the breast tissue is recontoured and relocated to achieve a firmer and more youthful appearance.

Depending on the degree of sagging, different incision techniques may be used, including periareolar, vertical, or inverted-T patterns. While minimal incisions may be sufficient for mild sagging, more advanced cases may require a more comprehensive approach. Nipple position and breast symmetry are carefully planned in detail.

If necessary, the procedure can be combined with an implant to provide both volume and lift. This approach supports breasts that have deflated over time and helps achieve a fuller and more aesthetic shape. Combined procedures are determined by the surgeon based on individual needs.

After mastopexy, the average recovery period is about 1–2 weeks. In the first days, swelling, tenderness, and mild pain may occur; wearing a special supportive bra is recommended. Once healing is complete, the breasts take on a firmer, more symmetrical, and younger form, and results tend to remain long-lasting.

What You Need to Know

Information

Definition

A breast lift (mastopexy) is a surgical procedure performed by removing excess skin and tightening/reshaping breast tissue to restore sagging breasts and position the nipple at an ideal level.

Purpose

To correct breast sagging, elevate the nipple, and give the breasts a tighter, more aesthetic appearance; to address deformities related to childbirth, breastfeeding, weight changes, and aging.

Types of Procedures

Periareolar (circular), vertical, and inverted-T (anchor) mastopexy techniques; may be performed without adding volume or combined with silicone implants.

Type of Anesthesia

Usually performed under general anesthesia.

Operation Duration

On average, 2–3 hours; it may take longer if combined with implants.

Recovery Process

Rest is recommended for the first 1–2 weeks. class="ez-toc-section" id="What_Is_Breast_Lift_Surgery">What Is Breast Lift Surgery?

Breast lift surgery (mastopexy) is a surgical procedure designed to restore a more lifted and aesthetic appearance to breasts that have sagged and lost their shape. Aging, pregnancy, breastfeeding, weight fluctuations, and gravity can all contribute to breast ptosis (sagging). During surgery, excess skin is removed, breast tissue is reshaped, and the nipple is repositioned to an ideal level. If needed, an implant can be used to provide additional volume support. The goal is to achieve a younger, balanced breast contour.

Why does breast sagging occur, and how does mastopexy address it?

Breast sagging—medically called “breast ptosis”—is a natural response of tissues to biological aging and the mechanical stresses they endure over time. The breast does not contain a rigid internal structure such as bone or cartilage that can hold it up. Instead, breast tissue is supported and anchored to the chest wall by thin connective bands called “Cooper’s ligaments.” You can think of these as delicate straps or elastic bands that help suspend the breast.

Over the years, gravity’s constant pull stretches these ligaments. Just like an overused elastic band that loses its rebound, these ligaments may no longer return to their original tightness. In addition, with aging, the production of collagen and elastin slows down. The skin gradually loses its strength and ability to tighten, making it less capable of supporting the breast’s weight.

Mastopexy addresses these changes by tightening the loosened skin envelope, repositioning the breast tissue higher on the chest wall, and moving the nipple to a more youthful location.

Factors that can contribute to sagging include:

Gravity

Pregnancy

Breastfeeding

Rapid weight gain and loss

Menopause

Genetic factors

Smoking

How is the degree of sagging determined in mastopexy planning?

In clinical evaluation, the key criterion we use to determine sagging severity is the nipple position relative to the inframammary fold (the crease under the breast). While people sometimes mention “pencil tests,” surgical planning requires a more scientific approach, and the commonly used method is the “Regnault Classification.” This classification guides the surgeon in choosing the most appropriate technique.

In this system, the nipple’s position relative to the inframammary fold, as well as its direction, is critical. In mild sagging, the nipple may be at the level of the fold. In moderate and severe sagging, the nipple falls below the fold and may even point downward. There is also a special condition called “pseudoptosis,” where the nipple remains in a relatively normal position, but the lower breast tissue droops. This is often associated with volume loss.

According to the Regnault classification, stages include:

Grade 1 (Mild ptosis)

Grade 2 (Moderate ptosis)

Grade 3 (Severe ptosis)

Pseudoptosis

How is the mastopexy technique (incision pattern) selected?

There is no single “best” breast lift method—there is the best method for the patient’s anatomy. The surgeon creates a roadmap based on the amount of excess skin and the condition of the breast tissue. The core principle is to achieve the best shape with the least visible scarring. However, in some cases, a better breast shape may require accepting a more extensive scar pattern.

There are three main techniques. First, the periareolar technique, where the incision is made only around the areola—this is limited to very mild sagging. Second, the vertical (also called “lollipop”) technique, which includes an incision around the areola plus a vertical line down to the fold. This approach creates a beautiful conical breast shape and is widely used. Third, the inverted-T (or “anchor”) technique, which is used for significant skin excess and advanced sagging.

Common incision patterns include:

Periareolar technique

Vertical technique

Inverted-T technique

Why is the lollipop (vertical) mastopexy so commonly preferred?

Vertical mastopexy is one of the most important advancements in modern aesthetic surgery because it balances the delicate trade-off between “scar” and “shape” exceptionally well. Traditional approaches often leave a long horizontal scar under the breast, whereas the lollipop technique either avoids this horizontal scar entirely or keeps it very short.

But the real strength of the vertical technique is not only the scar—it is the shape it creates. By gathering breast tissue from the sides and reshaping it toward the center, it helps the breast look more lifted, firmer, and more projected forward. In other words, it turns a wide and flattened breast appearance into a younger, conical form. The lift obtained with this method also tends to be more resistant to gravity over time. In the early postoperative period, small skin puckering (“dog ears”) may appear at the lower end of the vertical scar, but these typically smooth out as tissues settle.

Advantages of the lollipop technique include:

Less scarring

More durable projection

Conical shape

Narrower breast base

Younger appearance

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Is a lift alone enough, or is a silicone implant also needed?

This is one of the questions that occupies patients’ minds the most. In many cases, your own tissue provides the answer. If you have adequate breast volume—meaning when you lift and gather the breast with your hand, the fullness feels satisfactory—then a lift alone is often enough. In this approach, your existing tissue is repositioned upward and reshaped.

However, especially after breastfeeding or major weight loss, breast tissue may have diminished significantly (involution). In that case, tightening skin can lift the breast, but it may not restore the desired upper fullness; the upper pole may still look empty. This is when “augmentation mastopexy,” the combination of a breast lift with an implant, becomes valuable. Even a small implant can restore the lost roundness in the cleavage area.

Situations that may indicate a need for an implant include:

Significant volume loss

Upper-pole emptiness

Small breast size

A desire for more fullness

Correction of asymmetry

Can my own fat be used during mastopexy?

Absolutely—and it has become increasingly popular in recent years. For patients who do not want a foreign material (silicone) but would like subtle fullness, especially in the upper breast, fat transfer (“lipofilling”) can be an excellent alternative. It is often considered a hybrid approach.

In this method, fat is harvested via liposuction from areas such as the abdomen, hips, or thighs, then purified through special processing. The fat is then injected to fill volume deficits—most commonly in the upper pole/cleavage region. Because fat tissue is rich in stem cells, it may also improve skin quality in the treated area. However, it is important to understand that fat transfer does not provide the same large, sharp volume increase that an implant can.

Benefits of fat transfer may include:

Natural look and feel

No foreign material

Body contouring benefit

Improved tissue quality

Assistance with symmetry

What should be considered during the preoperative preparation period?

A successful surgery and smooth recovery actually start weeks before entering the operating room. The first and most important issue is smoking. Breast lift surgery relies heavily on healthy blood flow. Nicotine constricts blood vessels and may compromise the circulation of reshaped breast tissue and the nipple, which can lead to wound-healing problems.

In addition, it is important to avoid certain medications and supplements that have blood-thinning effects. Anything that increases bleeding risk can affect both surgical comfort and the amount of bruising afterward. Radiologic breast imaging is also an essential part of preoperative planning.

Things to avoid before surgery include:

Smoking

Alcohol

Aspirin

Vitamin E

Green tea

Ginseng

Cherry stem tea

Garlic supplements

Omega-3

How does surgery day and the hospital process work for mastopexy patients?

On the morning of surgery, you arrive at the hospital fasting. After settling into your room and feeling more at ease, your surgeon will see you. This stage is crucial because the standing markings are the architectural plan of the operation. The new nipple position and the amount of skin to be removed are calculated with millimetric precision and drawn on the skin.

The operation is performed under general anesthesia and typically takes about 2–3 hours depending on the scope of the procedure. When you wake up, you will be wearing a special medical support bra designed to stabilize the new breast shape and control swelling. In most cases, patients stay in the hospital for one night. A feeling of pressure in the chest area during the first hours is completely normal.

Common steps during the hospital stay include:

IV line placement

Blood id="Is_recovery_after_mastopexy_difficult_and_what_should_you_pay_attention_to">Is recovery after mastopexy difficult, and what should you pay attention to?

The recovery period after a breast lift is usually not as painful as patients fear. Most patients describe a sensation of “tightness” and “pressure” rather than severe pain. This feeling is a natural consequence of tightening the skin. Resting at home for the first few days and avoiding excessive arm movements are important.

In the first weeks, the breasts will be swollen and therefore appear higher and firmer than expected. This should not be alarming; it is temporary. As swelling decreases and tissues relax and settle with gravity, the breasts gradually take on a more natural form. Wearing the recommended medical bra consistently is critical because it reduces tension on incision lines and supports shaping.

Key points during recovery include:

Sleeping on your back

Avoiding heavy lifting

Not straining the arms

Regular dressing changes

Wearing the support bra

Staying well-hydrated

Avoiding smoking

Do surgical scars disappear completely over time?

It is important to be honest and realistic here. Any surgical procedure that disrupts skin integrity leaves a scar. An “scarless” breast lift is not technically possible. However, the goal of plastic surgery is to place scars in the least noticeable locations and help them fade over time.

In the first months after surgery, scars may be red, slightly raised, and more visible. This indicates your body is actively repairing the area. Patience is essential. Scar maturation and fading to a skin-tone color can take 1–2 years depending on the individual. With proper care, scars typically become fine, subtle lines and can be easily hidden under bras or swimwear.

Scar-management options may include:

Silicone gels

Silicone sheets/tapes

Laser treatments

Steroid injections

Sun protection

Can mastopexy affect breastfeeding ability or nipple sensation?

For women planning future pregnancies, breastfeeding is often the biggest concern. Modern surgical techniques are designed to keep the nipple-areola complex attached to underlying breast tissue on a “pedicle” that preserves blood supply, sensation, and a large portion of milk ducts whenever possible.

Most women can breastfeed after mastopexy. However, there is always a theoretical risk of partial duct disruption or reduced milk supply, especially in more advanced sagging cases—making surgeon experience important. Nipple sensation can change temporarily after surgery; some patients experience numbness while others notice increased sensitivity. As nerve endings heal, this typically improves over the following months.

Possible sensory/functional changes include:

Temporary numbness

Increased sensitivity

Reduced milk supply

Breastfeeding success

Gradual return of sensation

What role does mastopexy play in a “Mommy Makeover”?

Pregnancy and breastfeeding commonly cause changes in both the abdomen and the breasts. A “Mommy Makeover” refers to a combined approach that addresses these areas in a single recovery period. A breast lift is often one of the most important components of this plan.

While sagging breasts are lifted, abdominal muscle repair and tummy tuck (abdominoplasty) can be performed in the same session to restore overall body proportions. This combined approach allows for a more dramatic transformation with a single anesthesia and recovery period. However, it is generally recommended to wait at least 6 months after breastfeeding ends—ideally about 1 year—before undergoing such a comprehensive surgery.

Common Mommy Makeover combinations include:

Breast lift

Breast augmentation

Tummy tuck

Liposuction

Genital aesthetic procedures

What are the possible risks and complications of mastopexy?

Like any surgical procedure, mastopexy carries potential risks. Being informed helps you prepare realistically. The most common issues tend to be minor wound-healing problems—especially small openings at the junction point of T-shaped incisions—which usually resolve with dressing care.

Breast asymmetry often exists even before surgery and is reduced as much as possible, but perfect millimetric symmetry cannot be guaranteed. In rare cases, hematoma (bleeding/collection) or infection may occur. Risks are generally higher in smokers due to impaired blood supply.

Possible complications include:

Bleeding

Infection

Wound opening

Asymmetry

Sensation changes

Unfavorable scarring

Fat necrosis

Are mastopexy results permanent, or will the breasts sag again?

A breast lift can “turn back the clock,” but it cannot stop time. The new shape typically lasts for many years. However, aging continues, skin elasticity gradually declines, and gravity keeps working.

For this reason, a degree of loosening after 10–15 years can be considered natural. That said, the breasts usually do not return to their preoperative sagging level.
