It is most commonly a congenital anomaly and is reported more frequently in boys. Surgical treatment can provide functional improvement and better aesthetic appearance.

Potential causes include genetic factors, chromosomal abnormalities, and certain syndromes. It is seen more often in genetic conditions such as Apert, Poland, and Carpenter syndromes. Exposure to radiation or certain medications may also increase the risk.

Diagnosis can usually be made at birth with a physical examination. When needed, imaging such as X-ray or MRI may be used to evaluate the bone structure. The diagnostic work-up is important for surgical planning and may require a multidisciplinary approach.

Treatment is typically surgical, and the ideal timing is generally between 6 months and 2 years of age. The procedure may involve skin grafting and delicate separation of nerves and tendons. Postoperative hand therapy is critical for regaining optimal function.

Things You Should Know

Information

Definition

Syndactyly is the fusion of two or more fingers at the level of skin, soft tissue, and sometimes bone. It is one of the most common congenital hand anomalies.

Frequency

Occurs in about 1 in 2,000–3,000 live births. It is more common in males and most frequently affects the web space between the 3rd and 4th fingers.

Types

Simple syndactyly: fusion involves soft tissues only. Complex syndactyly: fusion includes bones and/or nail structures. Complicated syndactyly: associated with additional structural anomalies.

Fingers Commonly Affected

Most often between the middle and ring fingers (3rd and 4th). Fusion involving the thumb and index finger typically causes more significant functional limitation.

Causes

Failure of normal finger separation during embryonic development. Most cases are isolated and sporadic, but some are inherited. It may be associated with syndromes such as Apert, Poland, and Carpenter.

Diagnostic Methods

Diagnosed with physical examination after birth. X-rays may be used to assess bony fusion. Genetic testing may be required in syndromic cases.

Functional Impact

Restricted finger motion, difficulty with grasping, reduced fine motor skills, and aesthetic concerns.

Surgical Indications

Fusions with finger-length discrepancy (especially those causing functional limitation), cases involving the thumb, or complex cases typically require surgery.

Timing of Surgery

Often performed around 1 year of age. Cases involving the thumb or index finger may require earlier intervention (6–9 months).

Surgical Techniques

Skin separation, skin reconstruction using Z-plasty, skin grafting (often from the groin), and local flaps to prevent re-adhesion.

Complications

Graft loss, scar formation, limited finger motion, recurrence (re-adhesion), and color mismatch of the tissues.

Postoperative Follow-Up

Regular dressing changes, hand therapy, scar monitoring, and orthoses when needed. Finger function development should be monitored.

Long-Term Outcomes

With successful surgery, functional and aesthetic improvement is achievable. Some cases may require a second corrective procedure.

Prof. Dr. Serdar Nasır
Specialist in Plastic, Reconstructive and Aesthetic Surgery, Hand Surgery

He completed his medical education at Ankara University Faculty of Medicine in 1996 and his specialty training between 1996–2002 at Hacettepe University Faculty of Medicine, Department of Plastic, Reconstructive and Aesthetic Surgery.

Between 2002–2009, he worked at Süleyman Demirel University Faculty of Medicine, Department of Plastic, Reconstructive and Aesthetic Surgery.

Since 2009, he has been working at Hacettepe University Faculty of Medicine, Department of Plastic, Reconstructive and Aesthetic Surgery. class="ez-toc-section" id="What_Is_Syndactyly_Webbed_Fingers_Surgery">What Is Syndactyly (Webbed Fingers) Surgery?

Syndactyly (webbed fingers) surgery is a procedure that separates fingers that are congenitally fused. The aim is to improve functional use of the hand, enhance gripping ability, and achieve a more natural appearance. During surgery, the skin and soft tissues—and, when necessary, bony connections—are carefully separated. In some cases, a skin graft may be required. The operation is usually planned in early childhood, and earlier intervention often leads to better functional outcomes.

What Biological Process Underlies the Development of Syndactyly?

Human development in the womb is truly remarkable. In very early pregnancy—around the 4th week—the hands and feet appear as paddle-like structures without distinct fingers. In a normal developmental timeline, between the 6th and 8th weeks, the tissue between future fingers should dissolve in a programmed manner so that the fingers can separate. This planned cell death is called apoptosis. Syndactyly occurs when this separation process pauses or fails to complete at any stage.

Naturally, families wonder why this happens. In most cases, syndactyly is not caused by anything a parent did or did not do—it is simply a variation in embryologic development. However, genetics can play a role. If there is a family history, inherited patterns may be considered. Familial transmission is reported more often in syndactyly involving the 3rd and 4th fingers. Still, many cases occur sporadically even when no one else in the family is affected.

The clinical presentation can vary widely. In some children, fingers are connected only by a thin skin web. In others, nails—and even bones—may be fused. This variability requires an individualized, “tailor-made” surgical plan for each patient.

How Are Syndactyly Types Classified, and What Is Simple Syndactyly?

For surgical planning, we classify syndactyly based on what structures are fused. This helps determine the complexity, operative time, and the techniques required. Broadly, syndactyly is categorized as simple or complex.

Simple syndactyly is generally more manageable surgically. In this type, the connection between fingers involves only skin and soft tissue; the bones are not fused. However, simple syndactyly is further divided based on whether the fusion reaches the fingertip.

Subtypes of simple syndactyly include:

Simple incomplete syndactyly

Simple complete syndactyly

In incomplete cases, the fusion does not extend all the way to the fingertip, leaving part of the fingers separated. In complete cases, the fusion runs continuously from the base to the nail tip. Although labeled “simple,” complete syndactyly often involves a greater skin deficit after separation, so skin grafting may be more frequently required.

How Are Bones Affected in Complex and Complicated Syndactyly?

Complex syndactyly requires more detailed surgical planning because the connection is not limited to skin. The adjacent finger bones may be fused—often toward the distal segments—creating a bony union (synostosis). This prevents independent finger movement.

Complicated syndactyly is the most challenging form. In addition to bony fusion, structures that are normally separate—such as vessels, nerves, or tendons—may be shared or anatomically abnormal. Extra bony elements can also be present. These cases are often associated with genetic syndromes.

Syndromes that may be associated with syndactyly include:

Apert syndrome

Poland syndrome

Carpenter syndrome

Pfeiffer syndrome

In syndromic cases, the goal is not only finger separation but also restoration of overall hand structure and function. Treatment can therefore be longer-term and may require multiple staged operations.

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Which Imaging Methods Are Used for Preoperative Diagnosis and Planning?

A physical examination provides valuable information, but operating without understanding what lies beneath the skin is like walking in the dark. We need to evaluate the bone anatomy and—critically—map the vascular supply to the fingers.

As a first step, we routinely request hand X-rays. Plain radiographs show where the bones begin and end, the condition of the joints, and whether there is bony fusion.

In modern practice, vascular imaging is especially important, because the biggest concern during separation is injury to the arteries supplying the fingers.

Common imaging methods include:

Plain X-ray radiographs

Color Doppler ultrasonography

Magnetic resonance imaging (MRI)

Color Doppler ultrasound is often our most useful tool. It allows us to visualize the course of digital arteries, their branching points (bifurcations), and flow characteristics in millimeter-level detail. If a branching pattern is higher or lower than expected, we adjust the surgical plan accordingly. Doppler ultrasound is safe and painless for infants because it involves no radiation. In complex cases, MRI can help assess tendons and soft tissues in more detail.

Why Is Surgical Timing So Critical in Syndactyly Treatment?

When to operate is one of the most common and confusing questions for parents. Timing requires balancing two factors: the risks of anesthesia and technical difficulty in very small hands versus the functional development and growth potential of the hand.

In general, surgery is planned between 6 months and 2 years of age. Before 6 months, anesthesia tolerance is lower and the fingers are extremely small, which can make the procedure technically harder. However, certain cases require earlier surgery (often between 6–12 months).

Situations that may require earlier surgery include:

Thumb–index finger syndactyly

Ring–small finger syndactyly

Multiple-finger syndactyly

Why the urgency? The thumb contributes roughly half of overall hand function. For grasping, pinching, and manipulating objects, a free first web space is essential. That is why thumb-involving syndactyly is often corrected before age 1.

Similarly, ring–small finger syndactyly can create a “tethering” effect because the small finger is shorter. As a child grows, the longer ring finger can be pulled down and bend due to the attached shorter finger, leading to permanent deformity. Early separation helps prevent this.

When finger lengths are similar—such as middle–ring (3rd web space)—this tethering risk is lower. In those cases, we may wait until the hand is larger and surgery can be safer, commonly around 18–24 months.

What Are Modern Techniques in Syndactyly Surgery and What Are Zig-Zag Incisions?

The goal in surgery is not merely to separate fingers, but to achieve a natural appearance and full mobility. Straight-line incisions were used historically, but as they heal, they can contract and restrict motion—so they are largely avoided today. Modern surgery uses geometric principles.

One common approach is using zig-zag incisions along the sides of the fingers, based on Z-plasty principles. The idea is simple: straight scars tend to shorten as they heal, while a zig-zag pattern distributes tension and allows the tissue to stretch. As the child grows, the scar is less likely to tighten and pull the fingers into deformity.

Another key step is creating a well-shaped web commissure. Anatomically, the web space should slope gently from the dorsum toward the palm. To achieve this, we often design a rectangular or triangular dorsal flap and inset it into the web space to form a new web base. These flaps help provide a natural width and depth, reducing the risk of re-adhesion and improving function.

Which fingers are most commonly affected by syndactyly (webbed fingers)?

Syndactyly most commonly occurs between the middle and ring fingers.
