The hand is one of the most burn-prone areas, and without timely intervention, serious limitations in motion can develop.

Treatment is planned according to the depth and extent of the burn. While superficial burns may heal with conservative care, deep partial-thickness and full-thickness burns may require skin grafting or flap surgery. Proper positioning that keeps the fingers extended plays a key role in preventing contractures.

After early wound management, scar tissue and contractures may form and severely restrict hand function. In such cases, reconstructive procedures are performed to restore finger and joint motion. Tendon and nerve repairs may also be required.

Rehabilitation is an essential part of post-burn hand reconstruction. Physical therapy, occupational therapy, and custom splints help maintain mobility. Long-term follow-up and individualized exercise programs are effective in preventing permanent functional loss.

Things You Should Know

Information

Definition

Post-burn hand reconstruction includes surgical and conservative treatments for tissue loss, contractures, and functional impairment caused by thermal, chemical, or electrical burns of the hand.

Goals

Restoring hand functions (grip, grasp, finger motion), releasing contractures, preserving sensation, improving aesthetic appearance, and enabling return to daily activities.

Affected Structures

Skin, subcutaneous tissues, tendons, nerves, vessels, muscles, joints, and bones—especially the fingers, palm, and dorsum of the hand.

Common Problems

– Skin loss – Scars – Joint contractures – Sensory disturbances – Aesthetic deformities – Functional limitations

Surgical Methods

– Skin grafting (split-thickness, full-thickness) – Flap surgery (local, regional, or free flaps) – Z-plasty and other contracture release techniques – Tendon, nerve, and vessel repairs – Arthrodesis or osteotomy – Microsurgical reconstructions

Conservative Treatments

Pressure therapy (compression garments), silicone sheets, splinting, physical therapy, and hand rehabilitation.

Rehabilitation

Early passive and active range-of-motion exercises, splinting, edema control, scar management, strengthening, and functional use training.

Treatment Course

Varies with burn severity; after acute-phase care, reconstruction and rehabilitation continue through subacute and chronic phases. class="ez-toc-section" id="What_Is_Post-Burn_Hand_Reconstruction">What Is Post-Burn Hand Reconstruction?

Post-burn hand reconstruction includes surgical and rehabilitative treatments that address damage to the skin, muscles, tendons, and joints caused by burns. The goals are to restore hand function, reduce movement limitation, and improve aesthetic appearance. Reconstructive techniques such as contracture release, skin grafting, and flap procedures may be used. Early intervention and consistent physical therapy play an important role in treatment success.

How Does the Hand’s Anatomy Make Burn Treatment More Challenging?

From a burn surgery perspective, the hand’s anatomy is a true “knife-edge” balance. By nature, the hand is both remarkably resilient and surprisingly fragile. On the back of the hand, the skin is extremely thin—almost paper-like. This thinness allows the skin to glide smoothly when we make a fist. But in a burn injury, the same feature becomes a major disadvantage. A thin barrier cannot hold heat, so a burn can deepen quickly and reach the tendons and joints just underneath.

On the other hand, the palm is thick and tough to support gripping. While this skin is more heat-resistant, healing creates a different problem: palmar burns have a strong tendency to contract during healing. Without proper intervention, the fingers can pull into the palm and the hand may become difficult—or even impossible—to open. Because of the amount of function packed into a very small surface area, hand burns require highly specialized care compared to burns in other body regions.

What Should Be Considered When Burn Depth Determines the Treatment Plan?

The most important compass guiding our treatment roadmap is burn depth. Burns can range from superficial redness to injuries that extend down to bone. Correctly identifying depth is essential to avoid unnecessary surgery—or, conversely, to prevent delayed interventions.

First-degree burns are the kind many people experience after excessive sun exposure and usually do not require surgery. However, second- and third-degree burns change the picture. In “deep second-degree” burns, the skin tries to regenerate but tends to produce excessive scar tissue, which can form stiff bands that limit motion. In third-degree burns, all layers of the skin are lost; the body no longer has the capacity to repair the area, so skin transfer becomes necessary. Fourth-degree burns are the most severe and involve muscle and bone, often requiring tissue transfer with microsurgical techniques.

What Are Common Mistakes at the Moment of a Burn, and What Should First Aid Be?

What is done immediately after a burn can influence the success of surgeries performed months later. Many widely believed home remedies can unfortunately increase tissue damage. The first goal is to reduce the heat within the tissue and prevent the burn from progressing deeper.

The most important correct actions at the moment of injury include:

Rinsing with tap water

Removing jewelry

Covering with a clean cloth

Eliminating chemical agents (in chemical burns)

One of the most common mistakes is applying ice directly to the burn. Ice constricts blood vessels in already injured tissue, can severely reduce blood flow, and may deepen the injury. Instead, 15–20 minutes of cool or lukewarm running water is usually sufficient. Rings, watches, and bracelets should be removed immediately because swelling can turn them into a tourniquet that compromises circulation.

Why Can Escharotomy Be Vital for Saving the Hand?

In deep burns, the skin loses elasticity and becomes a hard, dead tissue layer known as eschar—almost like armor. This tissue does not stretch. Meanwhile, swelling develops rapidly underneath as fluid accumulates. Because the rigid eschar cannot expand, pressure rises uncontrollably, similar to a sealed pressure cooker. The pressure can become so high that it compresses the vessels supplying the hand and fingers, stopping blood flow.

If this is not addressed urgently, fingers can be lost within hours. This is where an escharotomy becomes life-saving. In the operating room—or in extreme emergencies at the bedside—we make long incisions along the sides of the hand through the eschar. As soon as the eschar is released, tissues decompress, vessels reopen, and blood flow to the fingertips can return. This is not an aesthetic procedure; it is an emergency measure to keep the limb viable.

When Is the Right Timing for Reconstructive Surgery?

Timing in post-burn hand reconstruction requires patience and experience. Performing aesthetic corrections immediately after wound closure is usually not ideal because burn scars are biologically active. In the first months, scars are red, firm, and raised. Surgery during this phase can increase bleeding risk and may lead to poorer scar outcomes.

Our general approach is to wait for scar maturation—when the scar softens and fades—which typically takes about 6 months to 1 year, although it varies from person to person. However, waiting is not always possible.

Situations that may require earlier surgery include:

Inability to close the eyelid

Complete blockage of finger motion

Inability to open the mouth

Risk of permanent joint stiffness

Growth restriction in children

In these functional urgency scenarios, we intervene before scars fully mature because prolonged immobility can cause irreversible joint changes and stiffness.

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Which Factors Influence Success in Repair With Skin Grafts?

The most fundamental way to cover soft-tissue loss is with skin grafts—commonly called “skin patches.” We harvest healthy skin from less visible areas such as the thigh or abdomen and transfer it to the burned region. In hand reconstruction, choosing the graft type is a critical decision.

We mainly use two types of grafts:

Split-thickness skin grafts

Full-thickness skin grafts

Split-thickness grafts are lifesaving for covering large areas and the donor site can heal on its own. However, they tend to contract as they heal. In areas where motion is vital—such as the dorsum of the hand or finger joints—this contraction can limit movement.

For this reason, we often prefer full-thickness grafts on the palm and the inner surfaces of the fingers. These grafts are thicker, contract less, are more elastic, and better tolerate pressure. Their drawback is that they can be more demanding to “take.” The hand must be kept still after surgery and blood must not collect under the graft. We explain carefully why immobilization is essential for the graft to adhere properly.

How Do Z-Plasty and Local Tissue Rearrangement Relieve Tightness?

Sometimes a burn does not leave a wide open defect, but instead creates a tight, cord-like band that blocks finger extension. The patient wants to straighten the finger, but the band pulls the skin and prevents motion. In such cases, instead of using a graft, we may use geometric rearrangement techniques.

Z-plasty is one of the cornerstones of plastic surgery. By making Z-shaped incisions across the tight band, we change the direction of the scar. This both lengthens the tight line and redirects tension, allowing movement to return.

In web-space contractures and finger adhesions (secondary syndactyly), we often design local flaps using healthy side skin of the fingers. This well-vascularized tissue is moved to the burned area. Because it carries its own blood supply, it tends to heal with better quality and does not contract like a graft.

When Is the “Groin Flap” Procedure Used?

In severe burns, damage can be so deep that bones, tendons, or nerves remain exposed. A skin graft cannot be placed directly on these structures because there is no viable bed to nourish the graft. In such cases, we use tissue transfer techniques. One classic and still reliable option is the groin flap.

This procedure can be challenging for patients. We elevate healthy tissue from the lower abdomen/groin with a vascular “pedicle,” and place the hand under that tissue through a tunnel—meaning the hand stays attached to the groin for about three weeks.

The main difficulties include:

Challenges with personal care

Shoulder and elbow discomfort

Restricted sleeping positions

Psychological stress

During this period, the tissue is nourished by groin vessels. After about three weeks, the hand develops its own blood supply to the transferred tissue, and in a second operation we divide the flap. This “biologic dressing” can be a safe solution that prevents amputation in cases where modern microsurgical options are too risky.

How Does Scarless Repair With Tissue Expanders Work?

One of the most fascinating tools in plastic surgery is the tissue expander. Skin can stretch and generate additional surface area when placed under gradual tension—similar to how abdominal skin expands during pregnancy. We use this principle therapeutically to remove burn scars and replace them with nearby, high-quality skin.

The process works like this: we place a deflated silicone balloon under healthy skin adjacent to the burn scar. Once the incision heals, the patient visits weekly and we inject saline into the expander, slowly inflating it. As it expands, the overlying skin stretches and increases in surface area.

Advantages of tissue expansion include:

Excellent color and texture match

No “patch” appearance

No additional major scarring

Preservation of hair follicles and sweat glands

When enough extra skin is created—usually after 2–3 months—we remove the expander in a second procedure, excise the burn scar, and advance the expanded skin to close the area. This approach can provide the most aesthetically satisfying results, particularly for larger scars on the dorsum of the hand and forearm.

#col-2014370265 > burns can destroy not only skin but also the motor units that move the fingers—muscles and tendons. Sometimes the skin is repaired but the patient still cannot lift the finger because the tendon has been burned or has become severely adherent. This is when tendon transfer surgery becomes relevant.

Some muscles and tendons in the body have redundancy or can be reassigned without causing major loss. In tendon transfer, we detach the tendon of a working muscle and connect it to the tendon of a non-functioning muscle, effectively re-routing power from a functioning “engine” to a paralyzed movement.

After surgery, the most critical step is that the brain must learn the new wiring. The patient must relearn how to use a muscle that previously served one action to produce a new movement. This neurologic adaptation requires intensive therapy and patience.

How Does Physical Therapy Influence Surgical Success?

In burn reconstruction, surgery is only half the work; the other half is rehabilitation. Even the most technically perfect procedure can fail functionally if the patient does not undergo effective therapy. A burned hand naturally tends to close and stiffen due to pain avoidance and scar contraction. If left alone, a claw-like deformity can develop.

Key tools in rehabilitation include:

Custom thermoplastic splints

Compression garments

Silicone gel sheets

Stretching exercises

Compression garments are particularly important to reduce hypertrophic scar formation. They often must be worn for up to 23 hours a day for months. For many patients, this phase can be more exhausting than surgery itself due to constant pressure, itching, and heat. Still, meticulous rehab is essential to achieve a functional hand.

What Is a Marjolin Ulcer and Why Is Long-Term Follow-Up Necessary?

A burn is not an injury we can simply say is “over.” It leaves lifelong scar tissue and requires long-term vigilance.
