It grows slowly and is usually painless. It is commonly seen in areas such as the hand, arm, and back. Lipomas in the hand and wrist can cause symptoms if they compress nearby nerves or blood vessels.

Among benign soft-tissue tumors, there are also lesions such as fibroma, neurofibroma, glomus tumor, and giant cell tumor. These masses are typically well-circumscribed, encapsulated, and do not invade surrounding tissues. Their clinical course is usually slow.

Diagnosis begins with a physical examination. Ultrasound and MRI are used to evaluate the location and characteristics of the mass. More detailed assessment is especially needed for lesions close to nerves or vessels. In selected cases, a biopsy may be performed.

Treatment depends on the tumor’s size, location, and the patient’s symptoms. Masses that cause symptoms or create aesthetic concerns are removed surgically. Lipomas rarely recur and usually do not undergo malignant transformation.

Things You Should Know

Information

Definition

A lipoma is a benign soft-tissue tumor that arises from fatty tissue and is generally slow-growing and painless. Other benign soft-tissue tumors are similarly non-aggressive and do not invade surrounding tissues.

Frequency

Lipoma is the most common type of soft-tissue tumor. It is frequently seen in adults; it is uncommon in the hand and wrist but can occur there.

Common Locations

In subcutaneous tissue; it can develop in many areas such as the hand, forearm, shoulder, back, and neck. class="ez-toc-section" id="What_Is_Lipoma_Fatty_Tumor_Surgery">What Is Lipoma (Fatty Tumor) Surgery?

Lipoma (fatty tumor) surgery is the surgical removal of fatty tissue masses that develop under the skin and are usually benign. Lipomas are mostly harmless; however, surgery may be preferred in cases of growth, pain, aesthetic concern, or the need to clarify the diagnosis. The procedure is usually performed under local anesthesia and is completed in a short time. The removed tissue may be sent for pathological examination.

Why Do Lipomas Form and Do Genetic Factors Matter?

So why do these masses form in some people while others never develop them? This is one of the most common questions we hear in the clinic. When we look at the scientific data, the strongest factor appears to be genetic predisposition. If there is a history of multiple “fatty lumps” in your family—parents or siblings—your likelihood of developing them increases. Genetic analyses have shown that certain rearrangements in specific chromosomal regions can trigger excessive proliferation of fat cells. This is especially evident in conditions such as familial multiple lipomatosis.

In addition, metabolic factors and trauma may influence the process. Although statistical associations have been reported between obesity, diabetes, or high cholesterol and lipoma formation, lipomas behave independently of the body’s general fat metabolism. In other words, even if you lose weight through dieting, lipomas usually do not shrink. Another interesting mechanism is trauma: a strong blow to a specific area may act as a trigger during the tissue-healing process and create a background for mass development. Some conditions associated with lipoma formation include:

Genetic predisposition

Diabetes

Obesity

Hyperlipidemia

Blunt trauma

Chromosomal abnormalities

Metabolic syndrome

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Is Every Lump a Lipoma, and What Are the Types?

One of the most common misconceptions is assuming that every movable lump under the skin is a simple lipoma. Yes, lipomas are very common, but soft-tissue tumors represent a wide spectrum, and not every mass originates from fatty tissue. As plastic surgeons, during the examination we look at firmness, location, and mobility to get important clues about the origin of the mass. Not every fat-based growth is a “standard” lipoma—some variants can present quite differently.

In angiolipoma, for example, there are dense vascular networks within the fatty tissue. These masses, unlike classic lipomas, are painful to touch and can be multiple, especially in young adults and often on the arms. The pain may be due to tiny clots in the vessels within the tumor. Other examples include leiomyomas arising from muscle tissue, or schwannomas arising from the nerve sheath. Nerve sheath tumors may produce an “electric shock” sensation when pressed. Vascular lesions such as hemangiomas, or firm fibromas arising from connective tissue, are also common benign masses. Frequently encountered lipoma variants and benign tumor types include:

Angiolipoma

Myolipoma

Fibrolipoma

Intramuscular lipoma

Hibernoma

Lipoblastoma

Neurofibroma

Schwannoma

Leiomyoma

Hemangioma

How Can a Malignant Mass (Sarcoma) Be Distinguished From a Lipoma?

In plastic surgery practice, one of the most important issues we must never overlook is excluding sarcomas—malignant soft-tissue cancers. Although most soft-tissue tumors are benign, rare malignant entities such as liposarcoma can put both the patient’s life and limb at risk if they are mistaken for a “simple lump” and removed inappropriately. When making this distinction, we look for certain warning signs—often called “red flags.”

If a mass is growing rapidly, has a clearly increasing diameter, and is located deep, alarm bells should ring. Simple lipomas are usually soft, mobile, and the skin over them looks normal. Malignant tumors, however, may feel fixed to surrounding tissues, and there may be increased visible vessels, warmth, or skin color change. Pain is also an important clue. Standard lipomas are generally painless or cause discomfort only when they become large and compress adjacent structures. Deep, persistent aching pain or night pain can indicate nerve compression or a more aggressive biological behavior. Features that should raise suspicion for malignancy include:

Rapid growth

Size over 5 cm

Deep location

Firm consistency

Irregular borders

Limited mobility (fixed mass)

Skin ulceration

Persistent pain

Prominent superficial veins

When Are Ultrasound and MRI Necessary in the Diagnostic Process?

For a small, superficial, mobile mass with classic features, an experienced surgeon’s physical exam is often sufficient. However, if the mass is large, deeply located, or the clinical findings are suspicious, radiologic imaging becomes important. Imaging is the surgeon’s “eyes” and allows preoperative mapping.

Ultrasound is a practical, radiation-free first-line tool that helps determine whether a mass is fluid-filled or solid. However, the “gold standard” imaging for soft-tissue tumors is MRI. MRI visualizes soft tissues in fine detail: it shows which muscle compartment the mass lies in, how close it is to major neurovascular bundles, and whether the content is pure fat tissue or a more complex structure. It serves as our roadmap for surgical planning. Methods used in the diagnostic process include:

Physical examination

Ultrasonography

MRI (Magnetic Resonance Imaging)

CT (Computed class="ez-toc-section-end">

If clinical or radiologic findings cannot exclude malignancy, histopathological confirmation before surgery is essential. Attempting to remove a potentially malignant mass without knowing what it is can be a medical mistake. In such cases, biopsy becomes necessary. Fine-needle aspiration is often insufficient because it does not show tissue architecture.

Therefore, we typically prefer core needle biopsies (tru-cut), which obtain a thicker tissue sample. This is done under local anesthesia and is generally not painful. The most critical point from a plastic surgery perspective is planning the biopsy pathway. The biopsy tract—the channel created by the needle—must be planned so that it will be fully included and removed within the definitive surgical excision. A biopsy performed from an incorrect route may seed tumor cells into clean planes and compromise surgical margin control. Key principles in biopsy planning include:

Core needle (tru-cut) technique

Marking the tract

Planning the incision line

Multidisciplinary approach

Adequate tissue sampling

Do Lipomas Go Away With Medication, or Is Surgery Necessary?

Another common question is: “Can I leave it alone—won’t it go away with a pill or a cream?” The medical reality is that a true lipoma or soft-tissue tumor does not disappear or shrink on its own. Over-the-counter herbal mixtures or creams cannot “melt” an encapsulated fatty mass under the skin. Some medical approaches such as steroid injections may be tried rarely, but they do not reliably eliminate the mass, and recurrence can be common.

The cornerstone of treatment is surgical excision. That said, not every lipoma must be removed immediately. If the mass is small, located in an inconspicuous area, painless, and not growing, it can be monitored after discussion with the patient. However, masses that create visible deformity, catch on clothing, enlarge and compress nerves, or show rapid growth are generally recommended for removal. Situations where surgery is usually recommended include:

Rapid growth

Pain

Aesthetic concerns

Limitation of movement

Nerve compression

Suspicion of malignancy

Risk of infection

Increase in size

What Is the Recovery Process Like and What Is the Risk of Recurrence?

Most lipoma and benign soft-tissue tumor surgeries are performed under local anesthesia, and patients can usually go home the same day. Recovery is generally quick. Mild postoperative pain can occur, but it is usually well controlled with simple pain relievers. Many patients return to daily activities the next day, though heavy exercise may need to be avoided for a short period.

After removing large lipomas or intramuscular masses, a “dead space” can remain. Sometimes the body fills this space with fluid—this is called a seroma. Compression dressings are used after surgery to reduce this risk. As for recurrence: if the lipoma is completely removed together with its capsule, the chance of recurrence in the same location is very low. However, if part of the capsule remains or if liposuction is used with incomplete removal, recurrence can occur. Factors that may affect recurrence include:

Incomplete excision

Residual capsule

History of lipomatosis

Intramuscular location

Incorrect diagnosis

Surgical technique

Frequently Asked Questions

In which areas do lipomas most commonly occur?

Lipomas most commonly appear in areas rich in soft tissue such as the back, shoulders, neck, arms, and thighs.
