The goal is to improve both the aesthetic appearance and the functional capacity of the fingers.

Finger lengthening procedures are generally performed using a technique called “distraction osteogenesis.” In this method, the bone is cut and gradually lengthened with special devices in a controlled manner. Although the process is long, the desired length and functional improvement can be achieved.

Deformity corrections are applied in cases such as angular deviations, rotational deformities, or joint deformities in the fingers. Proper finger alignment is achieved through surgical techniques such as osteotomy, tendon transfer, or joint reconstruction.

Postoperative rehabilitation is essential to maintain range of motion and increase muscle strength. Physical therapy and splint use ensure the sustainability of the surgical success achieved. Follow-up conducted by a specialized team is critical for long-term function.

Things You Should Know

Information

Definition

Finger lengthening or deformity correction procedures aim to surgically correct length deficiency or deformities of the fingers caused by congenital anomalies, post-traumatic losses, or developmental disorders.

Goal

To functionally and aesthetically increase finger length, improve grasping and holding ability, eliminate deformities, and enhance the overall functionality of the hand.

Reasons for Application

– Post-traumatic amputations – Congenital short fingers (brachydactyly) – Congenital deformities (such as clinodactyly, camptodactyly, syndactyly) – Deformities developing after burns or infections – Deformities related to tendon or nerve injuries

Lengthening Methods

– Distraction osteogenesis: The bone is cut and slowly lengthened; length is increased in a controlled manner with an external fixator (Ilizarov, mini-fixator systems). – Grafting: Length can also be achieved using bone grafts.

Deformity Corrections

– Osteotomy: Cutting the bone and realigning it correctly. – Tendon transfers: Rebalancing muscle-tendon dynamics in functional deformities. – Joint stabilization/arthrodesis: Fixation of a damaged joint. – Soft tissue releases: Used to correct contractures.

Devices Used

External fixator systems, mini plate-screw systems, microsurgical instruments, bone graft materials, and tendon repair materials.

Surgical Process

Usually requires staged planning; depending on the type and severity of the deformity, more than one operation may be required. class="ez-toc-section-end">

Finger lengthening surgery is a procedure performed to surgically increase the length of the finger in cases of congenital short fingers (brachydactyly), shortening after trauma, or developmental disorders. The most commonly used technique is bone lengthening (distraction osteogenesis), where the bone is gradually separated in a controlled manner and new bone forms in the gap. The aim is to improve hand function and correct aesthetic appearance. The treatment process is planned according to the amount of lengthening and the patient’s healing capacity.

Why are hand aesthetics and function evaluated together?

In hand surgery, it is almost impossible to separate aesthetics from function because the attractive appearance of the hand largely depends on the correct alignment of its anatomical structures. The angle of a finger, the proportion of its length, or the smoothness of the nail bed are also indicators of how well that hand functions. When patients apply to us, they usually have two main concerns: “Will I be able to use my hand as before?” and “Will the appearance of my hand improve?” The answer to these questions is directly related to how carefully the surgery is planned.

Fingertips, in particular, contain one of the densest nerve networks in the human body. This remarkable sensory ability allows us to identify objects simply by touching them, even with our eyes closed. Protecting this sensation during surgery is of utmost importance. When correcting deformities or performing finger lengthening, focusing only on the bone may put this delicate sensory mechanism at risk. Therefore, each case is approached like a puzzle in which bone, skin, tendons, and nerves are essential pieces. An intervention made for aesthetic reasons may increase grip strength, while a procedure aimed at improving function may also restore a natural appearance that allows the patient to use their hand confidently in social life.

Conditions that impair hand aesthetics include:

Congenital anomalies

Traumatic amputations

Burn contractures

Nail deformities

Tumor excisions

Rheumatic deformities

What is distraction osteogenesis, the bone lengthening method?

Distraction osteogenesis is perhaps one of the most impressive examples of medicine imitating nature. The technique works by applying controlled and very gradual tension between two living bone segments, allowing the body to fill the gap with new bone tissue. Essentially, the body receives a signal that healing is required, and by prolonging this healing response, new tissue forms. In hand surgery, this technique is particularly valuable for individuals who have lost part of a finger due to accidents or children born with short fingers.

One fascinating aspect of this process is what we call “distraction histogenesis.” As the bone lengthens, the surrounding soft tissues adapt to this process. Blood vessels extend, nerves stretch, muscles lengthen, and the surface area of the skin increases. In traditional lengthening attempts, the surrounding skin or vessels often become the limiting factor. However, with distraction techniques, tissues grow at the cellular level in response to gradual stretching. As a result, natural lengthening can be achieved using the patient’s own tissues without the need for grafts from other parts of the body. In addition, because no permanent synthetic materials (such as plates or screws) are required, the risk of infection is lower and biological compatibility is excellent.

Advantages of the distraction method include:

Formation of the patient’s own bone tissue

Adaptation of surrounding soft tissues

No need for bone grafts

No permanent implants required

Lower risk of infection

Functional length gain

What stages are involved in the finger lengthening process?

Finger lengthening is not a one-day procedure but a treatment journey that takes time and patience. This journey generally consists of three main phases. The first stage is the “latency period,” which begins immediately after surgery. After the bone is surgically cut (osteotomy) and the device is placed, no lengthening is performed for about one week to ten days. This waiting period allows the tissues to recover from surgical trauma and initiate the healing process.

The second and most active phase is the “distraction phase.” After the latency period, screws on the device are gradually turned to separate the bone ends. Speed is critical at this stage. If the process is too fast, newly forming vessels may rupture or nerves may be damaged. If it is too slow, the bone may heal prematurely and prevent further lengthening. The ideal rate is generally about 1 millimeter per day, applied gradually in small increments throughout the day to provide gentle and continuous tension.

The final stage is the “consolidation period.” Once the desired length is reached, the distraction stops but the device remains in place. During this phase, the soft new bone tissue (callus) that formed in the gap gradually hardens and mineralizes. This process is similar to concrete setting; the structure exists but needs time to become strong enough to bear weight. Typically, about one month of consolidation is required for each centimeter of lengthening. When imaging confirms that the bone has hardened, the device is removed.

Critical steps in this process include:

Surgical osteotomy

Latency waiting period

Active distraction

Radiological monitoring

Consolidation phase

Removal of the device

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

How are congenital finger shortening and deformities treated?

Although congenital hand anomalies can be a major concern for parents, modern surgical techniques can achieve very successful results. Conditions such as brachydactyly, where fingers are unusually short, or symbrachydactyly, where fingers are both short and fused, are not only aesthetic concerns but also functional issues that can affect a child’s development. Children explore the world with their hands, hold pencils, and play. Therefore, when planning treatment, factors such as psychomotor development and school age must be considered.

The treatment approach varies depending on the severity of the anomaly. If finger shortening is mild and does not affect hand function, observation may sometimes be sufficient. However, if shortening interferes with grasping, surgical intervention becomes necessary. In cases such as symbrachydactyly, the fused fingers are first separated. Bone lengthening procedures may then be planned simultaneously or afterward. The goal is not merely to create a hand that appears normal but to provide a hand that functions effectively and does not restrict the child socially.

In cases of short fingers, the functional capacity of the thumb and little finger is particularly important. Lengthening these fingers can increase the width of the hand’s grasp and allow the child to hold larger objects. As plastic surgeons, while lengthening the bone structure, we also balance the ligaments and tendons of the joints so that the extended finger retains mobility.

Types of congenital anomalies include:

Brachydactyly

Symbrachydactyly

Syndactyly

Polydactyly

Macrodactyly

Clinodactyly

Is surgery for extra fingers and thumb duplication difficult?

Polydactyly, commonly known as having six fingers, can require technically demanding surgery—especially when it involves the thumb (such as Wassel Type IV). Because the thumb contributes to more than half of hand function, errors in this area can significantly reduce hand performance. When two thumbs are present, each is usually weaker than normal and shares anatomical structures. Therefore, it is not simply a matter of removing the extra one.

Our surgical strategy focuses on reconstruction. Usually, the less developed thumb is removed, while valuable ligaments, muscles, and tendons belonging to that thumb are preserved. These structures are transferred to the remaining thumb to strengthen it. For example, collateral ligaments may be transferred together with a small bone fragment to maintain joint stability. Otherwise, the remaining thumb may become weak or deviate laterally over time.

Additionally, the direction of the tendons responsible for thumb movement must be corrected. If tendons do not pull along the correct axis, deformities such as a “zigzag deformity” may develop. Any irregularities in the joint surface are reshaped, and bone curvature may be corrected with osteotomy. In short, this surgery is not merely removal but the art of reconstructing a single strong thumb using the existing anatomical structures.

Stages of Wassel Type IV surgery include:

Excision planning

Osteoperiosteal flap preparation

Ligament transfer

Tendon repair

Joint reshaping

Aesthetic incision closure

What are the treatment options in traumatic finger amputations?

Sudden finger amputation or severe damage due to an accident can be both physically and psychologically traumatic. In such cases, the first goal is to preserve tissue viability. However, during reconstruction, two main options are often considered: “ray resection” or “lengthening.” This decision depends on which finger was lost, the level of loss, and the patient’s expectations.

In amputations of fingers other than the thumb—especially the index or middle finger—“ray resection” may be performed to close the gap in the hand. In this procedure, the metacarpal bone of the missing finger is also removed, and the neighboring fingers are brought closer together. The result is a hand with four fingers but no gaps between them, creating a narrower yet functional hand. This approach allows faster recovery, but some patients may have difficulty accepting the aesthetic or psychological impact of having fewer fingers.

At this point, distraction lengthening becomes an alternative. If part of the finger (a stump) remains, it can be lengthened and restored to a functional level. In cases of multiple finger loss, every millimeter of remaining tissue is valuable. Lengthening the stump increases grasp capacity and allows the patient to feel that their finger is still present. Even in fingers with less functional contribution, such as the little finger, lengthening may be performed to maintain aesthetic balance or to fill the finger of a glove.

Treatment alternatives after trauma include:

Stump repair

Ray resection

Callus distraction

Finger transfer

Prosthetic applications

Do the devices used after surgery restrict daily life?

Devices used in finger lengthening treatments have become much smaller and lighter thanks to technological advances. Older bulky devices have been replaced by elegant “mini-fixators” designed to fit finger anatomy. However, even though they are small, having an external metal device on the finger requires certain adaptations in daily life.

These devices are extremely lightweight and do not create a sense of heaviness in the hand. However, since they remain outside the skin, they may catch on clothing while dressing. Therefore, loose-sleeved clothing is usually recommended. Turning the screws on the device is technically simple, and patients or their relatives can easily perform this adjustment at home. This allows patients to actively participate in their own treatment and feel a sense of control.

Hygiene is the most important aspect of this process. The wire entry points where the device attaches to the bone can become entry points for bacteria. Therefore, daily cleaning and dressing changes should not be neglected. Patients can shower by covering the device with waterproof protectors. In social situations, the visible device may attract curious attention, but most patients tolerate this temporary inconvenience by focusing on the final result.

Recommendations for device use id="How_is_soft_tissue_and_skin_repair_performed">How is soft tissue and skin repair performed?

In finger reconstruction, establishing the bone skeleton is as important as the quality of the soft tissue covering it. In traumatic injuries or severe burns, skin may also be lost along with bone. Without a durable, flexible skin cover that provides sensation, the underlying bone or tendon cannot function properly. Therefore, plastic surgeons perform tissue repair with the precision of a tailor.

Skin grafts are the most common method used to close areas with skin loss. These are patches of skin taken from another area of the body (often the thigh, inner arm, or groin). Thin grafts are used for superficial wounds. However, in deeper injuries where bone or tendon is exposed, full-thickness skin grafts or vascularized tissue transfers known as “flaps” are required. In flap surgery, the tissue is transferred together with its underlying fat and nourishing blood vessels. This provides padding at the fingertip and creates a surface resistant to pressure.

Microsurgical techniques play a vital role at this stage. In areas such as the hand, where circulation depends on very small vessels, the success of tissue transfer depends on connecting tiny vessels with microscopic precision. If old scars from previous injuries restrict finger movement, scar revision procedures can release these tissues. Techniques such as Z-plasty lengthen tension lines and allow the finger to bend more freely.

Soft tissue repair techniques include:

Full-thickness skin grafts

Split-thickness skin grafts

Local flaps

Distant flaps

Free tissue transfers

Z-plasty procedures

Why is nail aesthetics and nail bed repair necessary?

The nail is not only a protective shield for the fingertip but also an important aesthetic feature. A fingertip without a nail becomes both more sensitive and visually incomplete. Even the smallest irregularity in the nail bed can cause the nail to grow split, ingrown, or uneven. Therefore, the nail unit must never be overlooked when correcting finger deformities.

In nail surgery, the most important rule is preserving or reconstructing the nail root, known as the “matrix.” If the matrix is damaged, the nail may not grow properly or may grow deformed. In nail bed injuries after trauma, the bed is repaired with microscopic sutures to create a smooth surface. Because the nail grows by sliding over this bed, the smoother the surface, the better the nail grows.

Sometimes parts of the nail bed may be missing. In such cases, sterile matrix grafts taken from the toes or other fingers may be used to reconstruct the missing areas. In chronic ingrown nails, narrowing the edges of the nail bed can provide a permanent solution. All these procedures aim to ensure that the finger not only functions well but also appears natural.

Nail surgery procedures include:

Nail bed repair

Matrix grafting

Ingrown nail revision

Deformity correction

Tumor excision

Prosthetic nail preparation

How important is physical therapy during the recovery process?

In hand surgery, there is a saying: “Surgery is half the treatment; rehabilitation is the other half.” Even the most successful surgery can fail if the hand is not mobilized afterward. Joints tend to stiffen quickly, and tendons can adhere to surrounding tissues if they remain immobile. For this reason, physical therapy is not optional but an essential part of treatment.

Rehabilitation begins immediately after surgery. In the early phase, passive movements are initiated to reduce swelling and prevent tissue adhesions. In patients undergoing lengthening, this becomes even more critical. As the bone lengthens, the muscles and tendons attached to it must also stretch. Otherwise, the bone may lengthen while the tendon remains short, leaving the finger bent and unable to extend. Stretching exercises performed under the guidance of a physiotherapist help maintain this balance.

After bone healing is complete and the device is removed, strengthening exercises are introduced. Hand putty, springs, and fine motor skill training help restore hand strength. Patient compliance and adherence to home exercises are the key to success. Patient and disciplined physical therapy acts as the bridge that integrates surgical work into everyday life.

Goals of physical therapy include:

Edema control

Preservation of joint mobility

Prevention of tendon adhesions

Increase in muscle strength

Sensory training

Return to daily activities

What factors affect the success of surgery?

Although finger lengthening and reconstruction surgeries require advanced technology and experience, their success does not depend solely on the surgeon’s skill. Patient-related factors are among the most important determinants of the outcome. First and foremost, blood circulation is the fuel of healing. Smoking is therefore one of the greatest obstacles to success. It causes constriction of microvessels and significantly impairs bone healing and wound closure. Patients undergoing such complex procedures are strongly advised to stop smoking completely.

Another important factor is the patient’s age and overall health. Children have an extremely high bone healing potential and remarkable adaptability. In adults, systemic diseases such as diabetes must be well controlled. Psychological readiness is also important; patients must accept living with the device temporarily and remain patient throughout the process.
