It is commonly used in hand and finger injuries and requires urgent intervention.

For a successful replantation, proper preservation of the amputated part (wrapped in sterile gauze, kept cold) and the shortest possible time to surgery are vital. During the procedure, vessels, nerves, tendons, and bone structures are repaired in sequence. Under the microscope, extremely fine vessel and nerve anastomoses are performed.

The decision on whether replantation should be performed depends on the condition of the amputated part, the patient’s age, general health, and functional expectations. Indications are particularly strong in thumb amputations, multiple finger amputations, and injuries of the dominant hand.

In the postoperative period, the continuity of vascular circulation must be monitored closely; precautions should be taken against complications such as embolism, thrombosis, and infection. Long-term rehabilitation and physical therapy play a decisive role in restoring hand mobility.

Things You Should Know

Information

Definition

Replantation (reattachment) of an amputated finger or limb is the process of restoring tissue viability by re-establishing vascular, nerve, tendon, and bone connections between a traumatically detached body part (finger, hand, arm, etc.) and the body using microsurgical techniques.

Goal

To restore the amputated limb functionally and aesthetically, and to regain as much natural movement, sensation, and strength as possible.

Where It Is Used

– Single or multiple finger amputations – Hand or arm amputations – Avulsion-type injuries (e.g., ring avulsion) – Amputations in children and the dominant hand

Evaluation Criteria

– Condition of the amputated part (clean cut vs. crush) – Time factor (if cold-preserved, replantation may be possible up to 12–24 hours) – Patient’s age, general health, and associated injuries

Surgical Steps

– Removal of contaminated/devitalized tissue (debridement) – Bone fixation – Tendon repair – Artery and vein anastomoses (microsurgery) – Nerve repairs – Skin closure

Need for Microsurgery

Vessel and nerve anastomoses are performed under a microscope. Amputation and Reattachment) Surgery?

Finger replantation is the procedure of reattaching an amputated finger using microsurgical techniques. In this operation, bone, tendon, vessel, and nerve structures are repaired under a specialized microscope and blood circulation is restored. The goal is to achieve the best possible aesthetic and functional healing. Success depends on the type of amputation, the time elapsed, and the patient’s general health. Early intervention improves outcomes.

Which injury mechanisms determine the surgical success of a severed limb?

When a patient arrives in the emergency department, the first factor we assess—and the one that most strongly influences the outcome—is how the injury happened. Not every cut or amputation is the same. The condition of the amputated finger on the table immediately defines the difficulty of the surgery and the likelihood of success. Understanding injury mechanisms explains why some cases do very well while others cannot be salvaged.

The most favorable group is the “guillotine-type” injury. Think of a clean cut from a sharp kitchen knife, a well-cutting industrial shear, or glass. The tissue edges are smooth and there is minimal crushing. This is ideal because vessel and nerve ends are easier to identify and tissue loss is minimal.

Another group is “crush” injuries. These are more complex and usually result from compression between heavy doors, press machines, or industrial equipment. The tissues are flattened between rigid surfaces. In such cases, vessels may be damaged not only at the cut edge but also further proximally. Because the inner lining of the vessel wall is injured, the risk of clotting is significantly higher.

The most challenging group—often with the lowest success rates—is “avulsion,” meaning a tearing or pulling-type amputation. Imagine a finger getting caught in a rotating machine or belt and being forcefully pulled, or a ring getting snagged and pulling the finger. In avulsion injuries, tissues are not cut; they are torn. Nerves and vessels stretch like rubber bands and rupture at their weakest points. Sometimes it looks like the injury is at the fingertip, but the nerve root may actually be torn far higher up, even from the elbow or armpit region. This is called “root avulsion.”

These injury types include:

Guillotine-type cuts

Localized crush injuries

Extensive crush injuries

Avulsion injuries

Combined injuries

What is ischemia time and how long can the amputated part survive?

The moment a body part is detached, the biological clock starts ticking. Without blood flow, the tissue becomes deprived of oxygen and cells gradually begin to die. We call this period “ischemia time.” Time is our greatest enemy. However, the allowable time depends on the temperature at which the part is kept and the type of tissue involved.

A critical distinction is between “warm ischemia” and “cold ischemia.” Warm ischemia means the amputated part is kept at room temperature. Cold ischemia means it is kept around refrigerator temperature, approximately +4°C. Cold slows cellular metabolism, reduces oxygen demand, and helps the tissue survive longer.

Another key factor is whether the amputated part contains significant muscle. Muscle tissue is among the least tolerant to oxygen deprivation. It deteriorates quickly and can release harmful substances. Therefore, a finger (with minimal muscle) can survive much longer under cold preservation than a larger limb segment rich in muscle.

In general, tissue tolerance to lack of oxygen differs by:

Finger (in cold conditions)

Hand and wrist level

Forearm and upper arm level

Warm ischemia tolerance of muscle-containing tissues

If a large, muscle-containing segment is replanted after excessive ischemia, a dangerous complication called “reperfusion injury” may occur. Toxins released from deteriorated muscle (such as potassium and lactic acid) can overwhelm the bloodstream, cause kidney failure, or even trigger life-threatening heart rhythm problems. For this reason, sometimes the safest decision is not to replant—because saving the patient’s life comes first.

How should the amputated part be transported, and what are common transport mistakes?

The success of replantation often begins at the accident scene—not in the operating room. No matter how skilled the surgeon is, if the part arrives “cooked” or “frozen,” there may be nothing that can be done. Unfortunately, common misconceptions can make salvage impossible.

One frequent mistake is placing the amputated part directly into ice. Direct ice contact can freeze the tissue and cause “cold injury.” Frozen cell membranes rupture, and the tissue may no longer be viable. Another mistake is soaking the part in alcohol, iodine, or hydrogen peroxide. These substances can damage and kill living cells. Storing the part in milk, yogurt, or dough increases infection risk and complicates cleaning.

The correct approach is the internationally accepted “two-bag method.” First, locate the amputated part and gently rinse it with clean water if it is visibly dirty—never scrub or rub. Then wrap it in a clean, slightly damp (not soaking wet) gauze or cloth. The answer is not always “yes.” Our goal is not to create a limb that survives but remains painful, nonfunctional, and burdensome. The aim is meaningful functional recovery. That is why we insist on replantation in some cases while advising against it in others.

Among the “absolute indications,” the thumb comes first. The thumb accounts for roughly half of hand function. Grasping, pinching, buttoning, and fine tasks rely heavily on it. Without a thumb, hand function can drop dramatically. Therefore, regardless of the level, thumb amputations are treated with maximum effort whenever feasible.

The second priority group is children. Children have an extraordinary healing potential compared to adults. Nerves regenerate faster, bones heal quickly, and the brain adapts more effectively. For this reason, even in distal or severely damaged amputations in children, we often attempt replantation.

Multiple finger amputations are also a priority. The hand can compensate for the loss of a single finger, but losing three or four fingers severely compromises grip function. Amputations at the wrist or palm level affect the entire hand, so replantation efforts are usually strong in these cases as well.

Priority situations include:

Thumb amputations

All amputations in children

Multiple finger amputations

Wrist-level amputations

Palm-level injuries

When is replantation contraindicated or risky?

Sometimes, even if replantation is technically possible, it may not be appropriate due to the patient’s overall condition or circumstances. These are called “contraindications.” The most important rule is: “Life first, then limb.” If the patient has major associated injuries such as head trauma, internal bleeding, or organ rupture, we cannot prioritize a lengthy hand operation over life-saving care.

Another limiting factor is severe crushing and contamination. If the tissue has been extensively crushed—like paper under a press—if the vascular bed is destroyed, or if there is not enough healthy tissue to repair, replantation is unlikely to succeed. In such cases, repairing the stump and enabling faster return to function may be the better option.

Smoking is also a major concern. Nicotine causes vasoconstriction (narrowing of blood vessels). The delicate vessels we repair with ultra-fine sutures may clot quickly in heavy smokers. The risk is explained clearly to the patient.

Psychological factors matter too. In cases of self-inflicted amputations related to severe psychiatric illness, replantation is often avoided because rehabilitation adherence may be poor and there may be a risk of re-injury.

Contraindications include:

Life-threatening associated injuries

Severely crushed tissues

Failure to retrieve the amputated part

Very advanced age and severe chronic illness

Severe psychiatric disorders

Excessively prolonged ischemia time

What are the stages and technical details of microsurgical replantation?

Replantation surgery is a marathon that tests patience and technical skill. Depending on the case, it may take 4 to 12 hours, and sometimes even longer. These procedures are performed under powerful microscopes that can magnify tissues up to 40 times. The sutures used are so fine that they may look like dust or spider webs to the naked eye.

We follow a specific sequence—like constructing a building. First, we establish the “framework” with bone fixation (osteosynthesis). Bone ends are cleaned, and sometimes the bone is shortened slightly to allow vessel length to match. The bones are then stabilized with wires or mini-plates. After the skeletal structure is secured, tendons are repaired—these are the “ropes” that move the finger.

Next comes the most critical step: vascular repair. Typically, arteries are repaired first to restore inflow. Under the microscope, vessel ends are prepared and connected end-to-end with multiple tiny stitches. When the clamps are released and the pale finger becomes pink again, that moment signals successful reperfusion.

But inflow alone is not enough—outflow is essential too. Therefore, veins are repaired as well. We often aim to repair two veins for each artery to improve drainage and reduce swelling. Finally, nerves are repaired and the skin is closed.

The typical surgical sequence is:

Wound cleaning (debridement)

Bone fixation

Tendon id="What_should_be_monitored_after_surgery_and_what_are_the_complications">What should be monitored after surgery, and what are the complications?

Leaving the operating room does not mean the process is finished—this is when the “delicate care” phase begins. The first 72 hours (the first 3 days) can be the most stressful period because the risk of thrombosis (clot formation) in repaired vessels is highest. The patient is monitored in a specialized setting similar to intensive care. Room temperature is important; the environment is kept warm because cold can cause vessel spasm and reduce blood flow.

Nurses check the finger frequently. The key criteria are simple but vital: the finger should remain pink, warm, and well-perfused. When the nail is pressed and released, the blanching should resolve within 1–2 seconds—this is called “capillary refill.”

If the finger suddenly turns pale and white, it suggests arterial occlusion (no inflow) and requires urgent action. If it becomes swollen, dark blue, and congested, it suggests venous occlusion (no outflow) and also requires rapid intervention.

Blood thinners are often used. The most important prohibition is smoking. Not only active smoking, but even exposure to smoke (“passive smoking”) can cause vasospasm and jeopardize the replanted finger.

Monitoring criteria include:

Skin color

Tissue temperature

Turgor and fullness

Capillary refill test

Bleeding control

Why and how is leech therapy (hirudotherapy) used in modern medicine?

Many people think leech therapy is purely traditional or “alternative.” In microsurgery, however, medicinal leeches can be an evidence-based tool—especially in cases of venous congestion after replantation. Sometimes, the artery can be repaired, but a suitable vein cannot be found or the repaired veins clot.

In that situation, blood enters the finger but cannot exit. The finger swells, becomes congested, and circulation may fail. Medicinal leeches (Hirudo medicinalis) can provide temporary venous drainage. When a leech bites, it not only removes blood but also releases hirudin, which prevents clotting. Even after the leech detaches, controlled bleeding may continue for hours.

This controlled outflow reduces pressure, helps remove deoxygenated blood, and buys time until new venous channels develop. Leech therapy is performed in a hospital setting with antibiotics to reduce infection risk, and blood counts are monitored. Only medical-grade leeches are used—not wild leeches.

Leech therapy helps by:

Reducing pressure through controlled blood removal

Preventing clotting

Improving circulation

Reducing edema

Supporting tissue nutrition

What are the reconstructive ladder, grafts, and flaps in tissue loss?

Sometimes, the amputation also removes skin. Even if bones, tendons, and vessels are repaired, there may be no healthy skin left to cover the structures. Exposed tendons or bone can dry out and become infected. That is where core plastic surgery tools come into play: grafts and flaps.

A “graft” is a skin patch taken from another area (often the thigh) and placed over the wound. A graft has no intrinsic blood supply; it survives by receiving nutrients from the wound bed. Therefore, a graft cannot reliably take on bare bone or tendon, because there is not enough vascularized tissue underneath to support it.

A “flap,” on the other hand, is a piece of tissue with its own blood supply. It can include skin, fat, fascia, muscle, or even bone. If it is rotated from a nearby area without disconnecting its vessels, it is called a “pedicled flap.” If it is transferred from a distant area (such as the back or abdomen) and its vessels are reconnected under a microscope, it is called a “free flap.” Because flaps bring their own blood supply, they are the most reliable option for covering complex wounds with exposed vital structures.

Reconstructive options include:

Skin grafts

Local flaps

Distant pedicled flaps

Free tissue transfers

Artificial skin applications

How does rehabilitation and physical therapy progress?

After surgery, we often tell patients: “We have done half of the job; the other half is in your hands.” In replantation, keeping the finger alive is not the only measure of success. If the finger cannot bend, cannot feel, and functions like an “accessory,” the patient cannot truly use it. That is why disciplined rehabilitation is essential.

The process typically begins with a 3–4 week “protection phase.” A splint is used to allow bones and tendons to heal. Sudden movements are avoided. However, complete immobility can cause stiffness. Therefore, after the initial protection, “passive motion” is introduced with a hand therapist—meaning the finger is moved with assistance rather than with the patient’s own force.

Once bones heal and fixation wires are removed, “active motion” begins. The patient starts using their own muscles to bend and straighten the finger. This phase can be challenging and requires patience.

Nerve recovery is the slowest part. Nerves regrow at an average rate of about 1 mm per day. After a wrist-level injury, it can take months for sensation to reach the fingertip. During this time, patients may feel electric shock-like sensations or tingling—often a positive sign of nerve regeneration.
