It can negatively affect hand function and limit daily activities. With early intervention, permanent restriction can often be prevented.

Post-traumatic joint stiffness most commonly occurs due to fibrosis in the tendons or surrounding joint structures. Finger fractures, dislocations, or surgical procedures may increase this risk—especially when immobilization is prolonged. An appropriate rehabilitation process is critical.

Inflammatory conditions such as rheumatoid arthritis and osteoarthritis can also cause chronic stiffness in the finger joints. This may present with morning stiffness and progressive loss of motion. Early diagnosis, appropriate medical management, and tailored exercise programs can help keep symptoms under control.

Treatment varies depending on the underlying cause. Physical therapy, joint mobilization techniques, splinting, and, when necessary, surgical release procedures may be used. The goal is to improve joint range of motion and restore functional use of the hand.

Things You Should Know

Information

Definition

Finger joint stiffness is a condition characterized by partial or complete loss of range of motion in one or more finger joints. id="What_Is_Finger_Joint_Stiffness">What Is Finger Joint Stiffness?

Finger joint stiffness refers to a decrease in joint range of motion that can develop after trauma, surgery, infection, or prolonged immobilization. Thickening of the joint capsule, shortening of ligaments, and adhesions may occur. Patients may experience limited movement, pain, and functional loss. Treatment can include physical therapy, splinting, and—when needed—surgical release procedures. Early rehabilitation plays a key role in preventing permanent stiffness.

Why does finger joint stiffness develop, and what is the anatomy involved?

The agility of our fingers is truly a biomechanical marvel. Each finger contains hinge-like joints that allow smooth motion. But unlike a simple door hinge, finger joints are supported by complex soft-tissue structures that stabilize the joint, prevent hyperextension, and provide side-to-side support.

Key structures include:

Volar plate

Collateral ligaments

Joint capsule

Flexor tendon sheaths

Extensor mechanism

Under normal conditions, when you bend and straighten a finger, the capsule surrounding the joint expands and collapses like an accordion. The collateral ligaments tighten and loosen during movement, preventing the joint from drifting sideways. When stiffness begins, these normally elastic tissues may become thick, shortened, and rigid—almost like dried leather.

The proximal interphalangeal (PIP) joint (the middle joint of the finger) is especially prone to stiffness. Because it has a long lever arm and relatively delicate soft-tissue support, it is vulnerable to trauma. Even a minor crush or sprain can trigger excessive scar formation. Unfortunately, this scar tissue can behave like a biological glue, binding together structures that must glide freely. As a result, the finger may become unable to fully bend or fully straighten.

How can finger joint stiffness after a fracture be prevented?

Post-traumatic stiffness is one of the most common scenarios in clinical practice. After a finger fracture, the body switches into an “emergency repair mode.” Healing cells rush to the injured area and collagen production begins to bridge the fracture. This is normal—until the repair response spreads excessively into surrounding tissues.

If the finger is immobilized too long in a cast or splint, the collagen produced may not only stabilize the bone but also bind the tendons, ligaments, and capsule together. Swelling (edema) is the biggest fuel for this process. An edematous tissue is highly prone to stiffening. That’s why a key principle is: “Keeping a finger completely still until the fracture heals can be disastrous for joint motion.”

Once fracture stability is achieved—sometimes with pins or screws—controlled motion becomes crucial. In some cases, pain after trauma is disproportionately severe compared to the initial injury. This may indicate Complex Regional Pain Syndrome (CRPS), also known as Sudeck’s atrophy.

Signs may include:

Severe burning pain

Excessive swelling

Skin discoloration (purple/bluish)

Glossy skin appearance

Excessive sweating

Nail changes

Joint stiffness

When this develops, patients may avoid touching or moving the finger due to pain. The more movement decreases, the more swelling increases—and stiffness escalates, creating a vicious cycle. Therefore, fracture rehabilitation should be managed not only with “bone healing” in mind, but also with a strong focus on preventing joint freezing.

What is Dupuytren’s contracture, and can it cause finger joint stiffness?

Beyond traumatic causes, one of the most interesting and genetically influenced causes of finger stiffness is Dupuytren’s disease. This condition involves abnormal thickening and shortening of the palmar fascia—the strong connective tissue layer beneath the skin of the palm. It typically progresses slowly and silently. Because it is common in Northern European populations, it is sometimes nicknamed the “Viking disease.”

It usually becomes noticeable after the age of 40–50. Early on, patients may feel small hard nodules in the palm but ignore them because they are not painful. Over time, these nodules can form thick cords extending toward the fingers. Like puppet strings, these cords pull the fingers—especially the ring and little finger—toward the palm. Eventually, the patient cannot straighten the fingers even with effort.

Risk factors include:

Genetic predisposition

Older age

Male sex

Diabetes

Smoking

Alcohol consumption

Epilepsy medications

Advanced imaging is often unnecessary for diagnosis. A simple “tabletop test” can be enough: the patient tries to place the palm flat on a table. If the palm cannot fully contact the surface and the fingers form a tent-like shape, intervention may be needed. The stiffness initially comes from the tightened fascia rather than the joint itself, but if left untreated, the joint capsule can eventually adapt and shorten permanently.

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

What are the surgical treatment options for Dupuytren’s disease?

Treatment is individualized in Dupuytren’s contracture—there is no one-size-fits-all operation. Planning depends on stage, age, occupation, and overall health status.

Common approaches include:

Needle aponeurotomy

Collagenase injection

Limited (partial) fasciectomy

Dermofasciectomy

In early stages or in older patients who are not ideal candidates for general anesthesia, needle aponeurotomy can be an option. No scalpel or sutures are used. Under local anesthesia, a needle is inserted through the skin and the tight bands are disrupted, allowing the finger to straighten. Recovery is fast, but recurrence is more common because diseased tissue is not removed.

Another minimally invasive option is collagenase injection, an enzyme injected into the cord. After about 24 hours, the finger is gently manipulated so the weakened cord ruptures, releasing the contracture. This approach can be effective but may be costly and not suitable for every patient.

The most durable “gold standard” option is limited fasciectomy, where the diseased fascia is surgically removed through carefully planned incisions. Straight-line cuts are avoided because they can contract during healing and recreate bending. Instead, zig-zag (Z-plasty-style) incisions help lengthen skin and reduce contracture risk. In stubborn or recurrent cases, dermofasciectomy (removal of diseased fascia along with overlying skin, followed by a skin graft) may be considered.

What are tendon adhesions, and how do they relate to finger joint stiffness?

Finger tendons function like the brake cables of a bicycle. They glide through tendon sheaths and under pulley systems as they pass over bones. This gliding is normally extremely smooth. However, after a tendon repair or tendon sheath inflammation, the surface may lose its slipperiness.

Adhesions after tendon repair are among the most common causes of finger stiffness. The muscle contracts and pulls, but because the tendon is stuck to surrounding tissues, motion cannot be transmitted to the fingertip. If a well-structured therapy program still fails to improve motion by around the third month, tenolysis may be considered.

Tenolysis aims to release adhesions and scar tissue around the tendon using magnification (loupes or microscope). It is delicate—almost like an archaeological excavation—because the tiny vessels nourishing the tendon must be preserved.

Another common tendon-related issue is “trigger finger,” where the tendon or the entrance of its tunnel (the A1 pulley) thickens.

Trigger finger symptoms include:

Pain at the base of the finger

A catching sensation during movement

A clicking sound

Locking of the finger

Morning stiffness

Because trigger finger limits motion, it can indirectly lead to joint stiffness. Treatment is usually straightforward: under local anesthesia, the tight pulley is released to allow the tendon to glide freely.

Is there a surgical solution when the joint capsule becomes stiff?

Sometimes the problem is not the tendon or the palmar fascia, but the “hinge” itself—the joint capsule and ligaments. When the capsule tightens or ligaments become rigid, the finger can lock. Surgical options include arthrolysis (joint release) and capsulotomy.

For example, if the finger is stuck in a bent position and cannot be straightened, the volar plate on the palm side may be adhered. In that case, a volar approach allows release of the adhesions. Conversely, if the finger remains straight and cannot bend into a fist, the dorsal capsule or collateral ligaments on the back side may be shortened; a dorsal capsulotomy may restore flexibility.

Fracture-dislocations that disrupt the joint surface are among the most challenging cases. When cartilage is damaged, the surface becomes irregular and movement may be painful.

In advanced cases, potential options include:

Internal fixation

Arthroplasty

Arthrodesis (fusion)

Interposition arthroplasty

When a joint is beyond salvage, a stable, pain-free finger (fusion) may be preferable to a painful, nonfunctional joint. However, preserving motion is typically the priority whenever possible.

Why is finger joint stiffness more noticeable in the morning?

Morning stiffness is related to reduced movement overnight, changes in joint fluid dynamics, and temporary tissue tightness.
