These types of injuries can cause multilayer damage to vascular, nerve, tendon, and bone structures. Rapid intervention prevents permanent loss of function.

Crush injuries typically occur as a result of heavy objects falling or compressing the limb. Beyond superficial damage, muscle necrosis, nerve crushing, and vascular occlusion may develop. Deep tissue assessment should be performed, and surgical debridement should be applied if necessary.

In laceration injuries—especially in occupational accidents involving cutting machines and glass—nerve and tendon integrity is frequently disrupted. In such injuries, anatomical structures are restored with microsurgery. Early repair accelerates the healing process.

In occupational accidents, the type of injury, the worker’s position, and the equipment used should be analyzed to develop both treatment and preventive measures against recurrence. Postoperative rehabilitation is essential for the hand to return to its former function.

Things You Should Know

Information

Definition

Crush injuries, cuts, and occupational accidents are traumatic events that occur in the workplace and cause disruption of tissue integrity or damage to organs.

Crush Injuries

Injury to tissues such as skin, muscle, vessels, or bone due to high external pressure. class="ez-toc-section" id="What_Are_Crush_Injuries_Cuts_and_Occupational_Accidents">What Are Crush Injuries, Cuts, and Occupational Accidents?

Crush injuries, cuts, and occupational accidents are injuries that occur in the working environment as tissues are exposed to mechanical trauma. Crush injuries usually develop due to pressure from heavy objects, while cuts occur through contact with sharp tools. Occupational accidents, on the other hand, may cause sudden injuries depending on physical, chemical, or environmental risks. Such traumas can lead to clinical presentations ranging from mild tissue damage to severe organ injuries.

What is an occupational accident, and what is its importance from the perspective of plastic surgery?

The concept of an occupational accident goes far beyond legal definitions; it means that a person’s life changes suddenly and unexpectedly. From a medical perspective, there is a sudden and often severe transfer of energy from the outside that disrupts the integrity of the body. Sometimes this may be a simple paper cut in an office, and sometimes it may present as being trapped under a press machine weighing tons in a factory. The group we mainly focus on in our surgical practice is complex injuries where tissue loss occurs, bones are shattered, and nerves and vessels are severed.

Such accidents do not merely create a physical wound. For a worker who uses their hands, losing a finger can be equivalent to losing their profession. Therefore, as plastic surgeons, we do not aim only to suture and close the wound. Will that hand be able to grip again? Will that finger feel again? Will the patient be able to engage in social life without feeling ashamed of the scar on their face when they look in the mirror? The answers to all these questions are hidden in the first moment of intervention and the subsequent surgical planning. The medical classification of occupational accidents is shaped according to the severity of the injury, and we draw our treatment map accordingly.

The classification of occupational accidents by severity is as follows:

Near-miss events

Minor injuries

Serious injuries

Accidents causing permanent disability

Fatal accidents

What are the differences between laceration and crush injuries?

How an injury occurs—its mechanism—is the most fundamental factor that determines the success of treatment and the pathway to follow. Although cuts and crush injuries may sometimes look similar from the outside as bleeding wounds, they are two completely different worlds at the tissue level. Making this distinction correctly is vital in order to predict the healing process.

Sharp-object injuries usually occur with sharp objects such as knives, glass, metal sheets, or guillotine shears. The most distinctive feature of these injuries is that the wound edges are smooth. In this situation, which we medically call a clean cut, tissue loss is usually minimal. Because the energy is focused on a narrow area, the surrounding tissues are not extensively damaged. For example, in a finger injury caused by a glass cut, vessels and nerves have a clear line as if cut with a knife. This is a major advantage for surgical repair. Finding the severed ends, bringing them together under the microscope, and suturing them is technically more feasible. In this type of injury, the success rates of reattaching the limb (replantation) are quite high.

Crush injuries, on the other hand, are one of the most challenging tests of plastic surgery. They occur as a result of press machines, rollers, vehicle wheels, or heavy objects falling. Here, the tissue is not only cut but also compressed and burst under high pressure. At the cellular level, crush injuries damage the inner wall of blood vessels. Even if a vessel appears intact from the outside, it is extremely prone to clotting because its internal structure has been compromised. In crush injuries, tissue viability may not be immediately apparent; a piece of skin that looks healthy may darken days later due to circulatory impairment. Therefore, crush injuries require repeated debridement procedures and much more meticulous follow-up.

Characteristic features of crush injuries are:

Irregular wound edges

Widespread tissue loss

Tissue bridges

Extensive ecchymosis and bruising

Internal vascular damage

Edema and swelling

When a limb is amputated, what should the golden hours and first aid be like?

When a limb—especially parts such as a finger, hand, or arm—is amputated, a race against time begins for that part to be replanted. We call this period the ischemia time in medicine, meaning the duration during which the tissue remains without blood and oxygen. Muscle tissue is the least tolerant tissue in the body to lack of oxygen. Irreversible cell death in muscle cells begins within approximately 6 hours. In parts such as fingers, where there is less muscle tissue, this period can be extended up to 12–24 hours if appropriate preservation conditions are provided, but the goal is always to take the patient to surgery as soon as possible.

First aid performed at the scene directly affects the surgeon’s success in the operating room. Unfortunately, many limbs that could be saved are lost due to commonly misunderstood methods. The most common mistake is throwing the severed part directly into ice or into water. This causes the tissue to freeze or swell, killing the cells. What should be done is to wrap the severed part in a clean and slightly moist cloth, place it in a waterproof bag and seal it, and carry this bag inside another container filled with ice water. This method is the most powerful weapon of plastic surgery in the modern era. It involves suturing vessels and nerves with diameters below 1 millimeter—barely visible to the naked eye—under special high-magnification operating microscopes. This procedure requires not only magnification devices but also special manufacturing threads much thinner than a strand of hair and very delicate surgical instruments.

In occupational accidents, especially in hand and finger amputations, repairing the artery that brings clean blood and the vein that carries away deoxygenated blood is essential for the survival of the severed limb. These tiny tubes must be brought end-to-end and sutured in a leak-proof manner. The smallest mistake can cause the vessel to occlude and lead to the replanted finger turning purple and being lost within hours. Therefore, microsurgery requires great patience and attention in addition to the surgeon’s manual skill. Sometimes replanting a single finger may take hours. In multiple finger amputations, operation times can reach 10–15 hours.

The basic structures repaired in microsurgery are:

Arteries

Veins

Nerve fascicles

Lymphatic channels

Tendon sheaths

Why do hand injuries require such complex treatment?

The hand is one of the most complex engineering marvels of the human body. Many vital structures packed into a small volume work in perfect harmony like the mechanism of a watch. Injuries in this region mean not only a disruption of tissue integrity but also the disassembly of this delicate mechanism. Even a simple cut can permanently impair hand functions. This is because on the dorsum and the palm of the hand, bones, joints, tendons, vessels, and nerves are layered and intertwined.

Especially in hand surgery, the transition area from the palm to the fingers, known as “No Man’s Land,” is very critical. Here, the tendons that bend the fingers pass through narrow tunnels that keep them attached to the bone. The slightest edema, bleeding, or adhesion that forms after injury or surgery can prevent the tendon from gliding within this tunnel. As a result, even if the finger remains in place as a whole, it may become unable to bend or straighten. Therefore, the surgical strategy in hand injuries is not only to connect the parts but to preserve their ability to move.

Critical anatomical regions in hand injuries are:

Flexor tendon zones

Extensor tendon zones

Pulley system

Digital nerves

Metacarpal bones

Carpal tunnel

When there is tissue loss, what are reconstructive surgery and flap methods?

Sometimes an occupational accident is so severe that it becomes impossible to bring the wound edges together and suture them. Skin, muscle, or bone tissue has been fragmented and lost in the accident. A large gap remains that cannot be closed. In this case, the plastic surgeon implements an algorithm we call the “reconstructive ladder.” This ladder is a solution roadmap that progresses from the simplest method to the most complex. The aim is to achieve the best closure with the least damage.

If the loss is only at the skin level and the underlying bed has adequate blood supply, thin layers of skin taken from another part of the body, namely grafts, are used. However, if avascular structures such as bone, tendon, or cartilage are exposed, a skin graft will not take. In this case, it is necessary to bring in a thicker, living tissue with its own blood circulation. This is called “flap” surgery. Flaps are obtained either by advancing healthy tissue right next to the wound or by transferring tissue with its vessels from a very distant region of the body. In this advanced method called a free flap, tissue taken from the back or leg is connected by microsurgery to vessels at the hand or ankle and mounted there.

The basic methods used in reconstruction are:

Primary closure

Skin grafts

Local flaps

Regional flaps

Distant flaps

Free tissue id="Do_sensation_and_movement_return_after_a_nerve_laceration">Do sensation and movement return after a nerve laceration?

Nerve repair is the part of the process that requires the most patience from both the surgeon and the patient. Nerves are like electrical cables; when they are severed, conduction is cut immediately and sensation or movement is lost in the area supplied by that nerve. When we surgically suture the nerve sheaths under the microscope, nerve healing does not occur at that moment. This is only the creation of a bridge. Nerve fibers must regrow from the repair site toward the fingertip or the target muscle.

This regrowth process is quite slow; it progresses by an average of 1 millimeter per day. For example, if nerve repair was performed for a laceration at the wrist level, it may take months or even a year for the nerve to reach the fingertips and for sensation to return. During this process, the patient begins to feel tingling or electric-shock-like sensations as signs of recovery. This is an indication that the nerve is advancing (Tinel’s sign). However, age, smoking, and the level of injury directly affect the quality of healing. Children and young patients have a much higher potential for nerve recovery.

Factors affecting nerve healing are:

Patient age

Level of injury

Timing of repair

Wound cleanliness

Smoking

Rehabilitation process

What is the aesthetic approach in occupational accidents involving the face?

Unfortunately, occupational accidents can seriously affect not only the hands and arms but also the face, which reflects a person’s identity. Facial trauma is common in workers in mining, construction, or the metal industry due to flying fragments or falls. Facial injuries are not only physical damage but also bring a significant psychological burden. An individual who feels ashamed of a scar on their face may withdraw from society and develop depression. Therefore, the aesthetic vision of plastic surgery comes into play in facial trauma.

When repairing facial lacerations, stitches are not placed randomly. Planning is done so that the repair is parallel to the natural lines and contours of the face. The aim is for the scar to become inconspicuous by being hidden among these natural shadows and lines once healing is complete. In addition, fractures of facial bones can disrupt chewing function and facial symmetry. These fractures are usually repaired through hidden incisions made inside the mouth or within the scalp, so that no additional visible scar is left from the outside.

Common problems encountered in facial trauma are:

Facial bone fractures

Lip lacerations

Eyelid injuries

Nasal trauma

Ear amputations

Soft tissue loss

How does the postoperative rehabilitation process progress?

When the plastic surgeon’s work in the operating room is finished, in fact only half of the treatment process has been completed. Especially in hand injuries, if surgical success is fifty percent, rehabilitation is the other fifty percent of success. Even a perfectly repaired tendon or nerve can become nonfunctional if the patient does not perform the correct exercises after surgery. Immobility causes tissues to adhere to each other and joints to stiffen, while excessive and uncontrolled movement can cause the repaired sites to rupture. Physiotherapy provides this delicate balance.

The healing process generally follows a disciplined schedule. In the first weeks, the area is immobilized with supports called splints to allow the repaired tissues to heal, and edema is controlled. Then, under the guidance of a physiotherapist, passive movements are started—meaning the fingers are moved without the patient using their own strength. In the final stage, strengthening exercises and return-to-daily-life training are performed. In this process, the patient’s compliance and patience are as important as the surgeon’s skill.

The basic stages of rehabilitation are:

Protection and rest

Edema control

Passive movements

Active movements

Strengthening exercises

Sensory training

Occupational therapy

Frequently Asked Questions

In which sectors are crush injuries, cuts, and occupational accidents most common?

These types of accidents are most common in physically demanding sectors such as construction, manufacturing, agriculture, metalworking, and transportation.
