Especially in hand surgery, they play a critical role in restoring function after trauma. The operation is performed with special instruments under high magnification.

Arterial and venous anastomoses are performed to restore blood flow to the tissue. This procedure is used in replantation, free tissue transfers, and injuries with impaired circulation. The strength and patency of the anastomosis directly determine tissue viability.

Nerve anastomoses are important for restoring sensory and motor functions. After the nerve ends are debrided, they are sutured end-to-end using epineurial or fascicular techniques. Nerve healing is slow, and full results emerge over months. When necessary, a nerve graft is used.

Strict follow-up is required in the postoperative period. Doppler ultrasound and capillary refill are monitored to evaluate vessel patency. In terms of nerve healing, it is supported with regular physical therapy, sensory re-education, and electrotherapy methods. Success depends on the type of injury and the timing of repair.

Things You Need to Know

Information

Definition

Vascular and nerve anastomoses are procedures in which severed or cut vessel and nerve ends are connected (sutured end-to-end) using microsurgical techniques. class="ez-toc-section" id="What_Are_Vascular_and_Nerve_Anastomoses">What Are Vascular and Nerve Anastomoses?

Vascular and nerve anastomoses are procedures in which vessel and nerve ends that have been cut or damaged during surgery are reconnected using microsurgical techniques. While vascular anastomosis ensures the continuity of blood circulation, nerve anastomosis aims to restore sensory and motor functions. These procedures are performed in situations such as trauma, tumor surgery, or replantation, and meticulous technique and proper timing are of great importance for successful outcomes.

What is microsurgery and why is it indispensable in plastic surgery?

To understand microsurgery, we first need to change our perception of scale. In daily life or in general surgical procedures, tissues, organs, and structures are usually of a size that can be easily seen, held, and managed with the naked eye. However, when it comes to reattaching a finger amputated due to an occupational accident, closing a large tissue defect in the leg after cancer surgery with muscle tissue taken from the back, or enabling a patient who has experienced facial paralysis to smile again, things drop down to millimetric dimensions.

At this point, microsurgery becomes the cornerstone of free tissue transfers and replantations (reattachment of amputated limbs). Success depends on the surgeon connecting vessels thinner than 1 millimeter to establish flawless blood flow and aligning nerve fibers thinner than a strand of hair to restore sensation and movement. These procedures ensure not only that the tissue is physically present, but that it is biologically “alive.” A tissue that does not receive blood cannot be nourished and dies; a muscle without nerve conduction atrophies over time and loses its function. Therefore, these anastomoses are the most critical stage that directly determines the patient’s quality of life.

What is the technological setup in the operating room and how is the microscope used?

Microsurgical operations take place in a very different atmosphere and with technical equipment than the standard surgeries we are used to. The central and most important component of these operations is high-tech operating microscopes that give the surgeon a kind of “super vision.” The human eye loses the ability to distinguish details after a certain distance and cannot clearly see millimetric structures. Modern microscopes, however, magnify the surgical field up to 40 times, making even the slightest irregularity in the inner wall of a vessel or micron-sized gaps between nerve fibers visible.

These optical systems not only magnify; they also preserve depth perception, enabling the surgeon to work in three dimensions. Powerful and focused light sources illuminate even the deepest and most hidden points of the surgical field, preventing shadow formation. Especially in what we call supermicrosurgery, where vessels under 0.8 mm are handled, achieving success without this technology is almost impossible. Thanks to this high-resolution imaging, tissue damage is minimized, work is more precise, and the healing process is accelerated.

How are microsurgical instruments and the surgeon’s hand tremor controlled?

Many patients naturally worry about the surgeon’s hand trembling or making a mistake while working with such small structures. However, microsurgery is an art performed not with brute force, but with delicate balance, patience, and support mechanisms. The surgeon stabilizes the arms and wrists with special supports, completely eliminating physiological tremor. Another major secret here is the special design of the instruments used.

Microsurgical instruments are designed with space-technology precision to control the force applied to tissue at the microgram level. Usually produced from titanium or high-quality stainless steel alloys, these instruments are extremely lightweight, have matte surfaces that prevent glare under microscope light, and tips as thin as one-tenth of a millimeter. These instruments are so delicate that their tips can be damaged by the slightest impact. Therefore, their maintenance and sterilization are performed with great care.

The main instruments used in this specialized surgery are:

Micro scissors

Micro needle holders

Fine tissue forceps

Vascular clamps

Micro dilators

How fine are the suture materials (sutures) used?

The needles and threads used in microsurgery, medically called sutures, are unlike any material we think of when we hear the word “stitching” in daily life. These threads are so thin that you may have difficulty noticing them with the naked eye when removed from the package; most of the time, they can only be clearly seen under the microscope. Suture thickness is numbered according to international standards; in microsurgery, 9-0, 10-0, and 11-0 threads are frequently used. The higher the number, the thinner the thread.

The most important criterion in selecting these materials is that the thread must be “inert.” In other words, the body should not show a foreign-body reaction to it and it should not cause inflammation. Monofilament (single-fiber) structures such as nylon or polypropylene are generally preferred. Multifilament, braided threads are considered risky in these delicate procedures because they can harbor bacteria between fibers and create friction while passing through tissue. Monofilament threads glide through tissue smoothly, but in terms of knot security they require the surgeon to use very specific and careful knotting techniques. Under the microscope, the surgeon must adjust the knots tied with these threads to an ideal tension—tight enough to prevent blood leakage, but loose enough not to choke the tissue and impair its blood supply.

What should be considered when performing a vascular anastomosis?

Vascular anastomosis, meaning the joining of vessels, is the most stressful and at the same time the most critical stage of microsurgery. The goal here is not merely to connect two tubes, but to ensure that the blood flowing through them moves smoothly without creating turbulence. The innermost surface of the vessel that contacts blood is the “intima” layer. The slightest irregularity in this layer, an incorrect suture pass, or a piece of tissue protruding into the lumen draws clotting elements to that area. This leads to clot (thrombus) formation at the anastomosis line and vessel occlusion. An occluded vessel means the entire surgery has failed.

In plastic surgery, the “end-to-end” technique is most commonly used. Two vessel ends are brought face-to-face under the microscope and sutured circumferentially with microscopic stitches. However, sometimes there may be a diameter mismatch between the recipient vessel in the transfer site and the flap vessel. In this case, “end-to-side” techniques or special incision methods that tolerate diameter differences come into play. Very fine-tipped instruments and vasodilator medications are used to prevent vessel spasm and to widen the vessel opening. When the anastomosis is completed and the clamps are released, seeing that pale tissue suddenly turn pink and watching blood circulation begin is the greatest reward for the surgeon.

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

How is graft use performed when the vessel length is insufficient?

Sometimes, due to the severity of the trauma, the extent of the injury, or the gap created after tumor removal, the vessel ends cannot reach each other. Suturing a vessel under tension is one of the biggest mistakes that can be made in microsurgery. Tension disrupts the vessel wall, impedes blood flow, and causes sutures to tear. In such cases, it is necessary to place a “bridge” between the two ends, meaning to fill the gap. In medical terms, this is called a “vein graft.”

A healthy vessel segment taken from another part of the body—usually from superficial veins in the leg or arm—is used to patch the missing section. This can be compared to adding an extra pipe to a water plumbing system, but biological rules are much stricter. To prevent the valves inside the harvested vessel from obstructing the direction of blood flow, the graft may need to be reversed before suturing. Although special techniques such as the Linton method or synthetic grafts may rarely be used, the patient’s own tissue (autologous graft) always provides the best compatibility and the highest chance of success.

What is the repair and healing process like for severed nerves?

Nerves are like the electrical cables of our body. They enable movement by carrying commands from the brain to muscles (motor nerves) and allow us to feel by transmitting sensations from the skin and joints to the brain (sensory nerves). When a nerve is cut, the portion beyond the cut—meaning the end whose connection to the brain is severed—undergoes degeneration. This is called “Wallerian degeneration.” The nerve fibers dissolve, but the outer sheath of the nerve and the tunnel-like structure inside are generally preserved.

The healing process begins with the surgical reconnection of the severed nerve ends. However, function does not return immediately at the moment of reconnection. The nerve fibers (axons) must regrow from the repaired site, passing through the prepared tunnels until they reach the target organ (fingertip or muscle). This growth rate is on average 1 millimeter per day. Therefore, after a cut at the wrist level, it may take months—sometimes more than a year—for sensation to return to the fingertip. During this long waiting period, the surgeon’s role is to reconnect the nerve ends in the most perfect and properly aligned way so that this journey can begin.

Which nerve repair technique is preferred?

Success in nerve repair depends on matching the correct cables to each other. Inside nerves, there are sub-bundles called “fascicles” that contain thousands of nerve fibers. Some bundles provide movement, while others carry only sensation. A wrong match can cause a motor command from the brain to go to the skin, or a sense of touch to go to a muscle, resulting in dysfunction. The surgeon chooses the most appropriate technique according to the type and level of the injury.

The main nerve repair techniques used are:

Epineurial repair

Fascicular repair

Epiperineurial technique

Group fascicular repair

In epineurial repair, the outermost sheath of the nerve is sutured; this is a less traumatic and faster method. In fascicular repair, the bundles within the nerve are separated and sutured individually. This method provides more precise alignment but requires more surgical dissection. The goal is always to achieve healing without forming scar tissue that would prevent nerve fibers from crossing.

When is a nerve graft or nerve transfer necessary?

If there is a very large gap between the nerve ends and it is impossible to bring them together end-to-end, the “grafting” method is used. Nerves such as the sural nerve behind the leg—which can be sacrificed without causing significant functional loss in the body (leaving only a small numb area on the heel)—are harvested and transferred to the damaged area. This nerve segment serves as a guiding tunnel for the regenerating axons.

In more advanced and complex cases, a method called “Nerve Transfer” (Neurotization) is applied. This technique is lifesaving especially in root avulsion injuries or in situations where the target is too far to be reached within the nerve growth time. A branch of a functioning, intact nerve is sacrificed and directly connected to the nerve leading to a non-functioning, paralyzed muscle. In other words, we redirect energy from one part of the body to another to save a more critical function. For example, intercostal nerves or cervical nerves can be used to restore shoulder movement.

Why are body temperature and fluid balance important during surgery?

Microsurgical success is not limited to the surgeon’s manual skill; the patient’s overall physiological balance also determines the outcome. Vessels, especially micro-scale vessels, respond to cold, pain, and stress by constricting (vasoconstriction). This constriction can be disastrous for a newly sutured delicate vessel and can completely stop blood flow.

Therefore, operating rooms where microsurgery is performed are kept warmer than normal, and the patient’s body temperature is continuously maintained with special warming devices. Hypothermia (a drop in body temperature) is one of the biggest enemies. In addition, measures are taken by the anesthesia team to ensure the patient does not feel pain and does not shiver. To keep vessels open, the patient’s intravascular fluid volume must be adequate. For this reason, plenty of fluids are administered during and after surgery, and urine output is closely monitored. Adequate fluids increase blood flowability and keep the amount of oxygen delivered to tissues (perfusion) high.

During which surgical procedures are vascular and nerve anastomoses performed?

Vascular and nerve anastomoses are generally performed during microsurgery-requiring finger replantations, free tissue transfers, nerve repairs, and reconstructive surgeries carried out after traumatic amputations.

What is vascular anastomosis and how is it performed?

Vascular anastomosis is the procedure of restoring blood flow by suturing two vessel ends to each other using a microsurgical technique.
