They are of vital importance, especially in limb replantations.

Microsurgical vascular repair is performed under a surgical microscope that provides high magnification, using very fine suture materials. Thanks to this technique, even vessels with a millimeter diameter can be successfully sutured. During the procedure, the vessel ends are debrided and an end-to-end anastomosis is performed.

Arterial repairs are prioritized to restore the delivery of oxygenated blood to the relevant tissue. Venous repairs, on the other hand, ensure venous return, preventing edema and circulatory disorders. Repairing both structures is critical for tissue survival.

In postoperative follow-up, vessel patency, signs of circulation, and tissue viability are closely monitored. Anticoagulant therapy may be administered. In patients who undergo vascular repair, success rates depend on the time to intervention, surgical experience, and accompanying tissue injuries.

Things You Need to Know

Information

Definition

Vascular repairs performed with microsurgery are the surgical repair or reconnection of arteries or veins, typically 1–3 mm in diameter. class="ez-toc-section" id="What_Are_Vascular_Repairs_Microsurgery">What Are Vascular Repairs (Microsurgery)?

Vascular repairs (microsurgery) are procedures in which small arteries and veins, usually 1–3 millimeters in diameter, are repaired under an operating microscope using specialized fine surgical instruments and suture materials. They are most commonly performed in the reattachment of amputated limbs after trauma, tissue transfers, and free flap procedures. The aim is to restore blood circulation to preserve tissue viability and prevent loss of function. It requires high technical skill and experience.

What are the standard suture techniques used in vascular repairs?

In vascular repairs, the most frequently used method by surgeons—one that has proven its reliability for years and is considered the “gold standard”—is to bring the vessel ends together and connect them with individual sutures. We call this an “end-to-end anastomosis.” The threads (sutures) used during this procedure are 10/0 or 11/0 nylon sutures. These numbers may not mean much to you, but let me put it this way: when viewed with the naked eye, these threads look like a speck of dust floating in the air or a thin spider web.

The main goal in this standard technique is to keep the tunnel inside the vessel (the lumen) open and to leave no slightest irregularity that could cause blood clotting. Each stitch must gently approximate the layers of the vessel. If it is tied too tightly, the vessel constricts and blood cannot pass; if it is tied too loosely, blood leaks and a clot forms. Achieving this delicate balance requires great experience.

The main objectives of this method are:

Lumen patency

Leak-proof sealing

Tissue integrity

Prevention of clotting

Continuity of flow

What role does the Z-plasty technique play in preventing vascular narrowing?

In surgical practice, we are always pursuing the question, “How can we do it better?” Although standard repairs performed with a straight incision often yield excellent results, sometimes healing tissue can constrict and narrow the vessel opening. This is a risk factor especially in pediatric patients or tissues with growth potential. At this point, we adapt one of the fundamental principles of plastic surgery—the “Z-plasty” technique—to vessels.

As the name suggests, Z-plasty refers to cutting and suturing the vessel ends not as a straight line but in an interlocking “Z” shape or in a way that creates triangular flaps. Why do we go to this trouble? Because a wound that heals as a straight ring tends to constrict over time like the mouth of a drawstring bag. With Z-plasty, when we make the repair line zigzag, we break this constriction effect. More importantly, this technique physically enlarges the diameter of the repaired line. You can think of it like widening a narrow door at its hinges.

This technique is not applied to every vessel. In particular, it may not be preferred in extra-vascular tubular structures—for example, in urologic microsurgery in the vas deferens (ductus deferens)—because the tissue is firm and difficult to shape. However, in flexible vascular structures, especially when long-term patency is critical, Z-plasty can be a lifesaving maneuver.

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Are sutureless methods and fibrin adhesives safe?

With advances in technology, the philosophy of “less foreign material, more natural healing” has come to the forefront in surgery. After all, no matter how fine the threads we use are, they are foreign materials for the body and can create a reaction, even at a microscopic level. To minimize this reaction and shorten operation times, sutureless or minimal-suture methods have been developed.

The leading approaches include the “Telescopic Method” and “Fibrin Adhesives.” In the telescopic method, one end of the vessel is placed inside the other end, like nested cups or the parts of a telescope. This ensures that the inner surface of the vessel (the intima) provides smooth continuity in the direction of blood flow. Biological adhesives are then used to stabilize this junction.

Fibrin adhesives contain clotting proteins derived from human blood. In other words, they mimic the body’s own wound-healing mechanism. Using only adhesive can create a risk of separation, especially in high-pressure arteries, so we generally adopt a hybrid approach. To ensure safety, we place 2 or 3 guiding sutures and then apply adhesive to provide sealing and complete union.

The advantages provided by this approach are:

Short operation time

Less tissue damage

Low inflammation

Leak-proof sealing

Ease of application

Why is Folded-Stent technology critically important in venous repairs?

In microsurgery, repairing arteries is relatively more “pleasant” because their walls are thicker, their lumens remain open, and they are easier to suture. However, veins can sometimes turn into a nightmare for surgeons. The walls of veins are so thin and lax that they constantly collapse and stick together like an empty pouch.

This sticking creates a major risk while placing sutures: “back-walling,” meaning suturing the posterior wall. It is the risk of accidentally catching the back wall while passing the needle through the front wall, thereby completely closing the vessel lumen. If you do not notice this mistake, the vessel occludes as soon as blood flow starts and the entire surgery becomes futile.

To solve this problem, the “Folded-Stent” technique has been developed. In this method, a temporarily folded, thin stent (support material) is placed inside the vessel. This stent supports the vessel from within like a tent pole, keeping it open. This allows the surgeon to clearly distinguish the front and back of the vessel and pass the needle safely. After the procedure, the stent is removed or left in place if it is a resorbable type. Studies have shown that this technique reduces the margin of error in venous repairs to almost zero and dramatically increases procedural speed.

What are the success rates of free tissue transfer (flap)?

Türkiye is one of the leading countries in the world in plastic and reconstructive surgery. Data from our university hospitals and major clinics show that success rates in free tissue transfers (microvascular free flaps) are 95% and above. This is fully compatible with world standards, and it is an area where we are even at a better level than many Western countries.

Success means not only suturing the vessel, but also the transplanted tissue surviving in its new location and gaining function. For example, we are talking about complex procedures such as a thin skin and vessel segment taken from your arm (Radial Forearm Flap) covering a defect in your neck, or a segment taken from the shin bone in your leg (Fibula Flap) being used as a jawbone.

Although failure is rare, it usually becomes apparent within the first 72 hours. In the radial forearm flap, problems may be more related to the artery, whereas in the fibula flap, venous insufficiency may be seen more frequently. However, in experienced hands, these risks are minimal.

Commonly used flap types include:

Radial Forearm Flap

Fibula Flap

Rectus Abdominis class="ez-toc-section-end">

After the surgery ends, the most stressful but most important process begins for us: follow-up. The color, temperature, and fullness of the transferred tissue tell us everything. If a vascular repair fails or becomes occluded, the tissue immediately signals us. Reading these signals correctly is vital to save the tissue.

We may encounter two main problems: arterial insufficiency (no inflow) or venous insufficiency (no outflow).

In arterial insufficiency, fresh blood does not reach the tissue. In this case, the tissue becomes pale, takes on a marble-white color, and feels ice-cold to the touch. Even if you prick it with a needle, there is no bleeding, or there is only a small amount of dark-colored oozing. This means “the faucet is closed.”

Venous insufficiency is more insidious and dangerous. Blood reaches the tissue but cannot return. Like a sink with a clogged drain, the tissue fills with blood, swells, and turns purple (cyanosis). The tissue color may shift from purple to blue and even black. It feels tense to the touch and shows a very rapid response in the capillary refill test (pressing with a finger and releasing), because it has turned into a pool of blood inside.

Signs of venous insufficiency include:

Purple discoloration

Edema

Increased warmth

Rapid capillary refill

Dark-colored bleeding

When is the decision made for re-exploration, meaning repeat surgery?

In microsurgery, the “wait and see” tactic generally does not work. If there is a problem, the solution is usually surgical. If we detect a circulatory disorder in the tissue in the early postoperative period (within the first 48 hours), we urgently take the patient back to the operating room. We call this “re-exploration.” This is not a failure; on the contrary, it is a heroic move to save the tissue.

Statistics present an interesting piece of data: In about half of the cases taken back for repeat surgery, the main problem is not vascular occlusion but a “hematoma,” meaning a collection of blood clot at the surgical site. Because we give our patients anticoagulant medications after surgery, there can be oozing-type bleeding. This blood accumulates within the tissue, hardens, and compresses the vessels from the outside. In this case, what we do is simply remove that blood collection. The moment the pressure is relieved, the vessel starts working again and the tissue pinks up within seconds. However, if there is a true vascular occlusion (thrombosis), it is necessary to excise that segment and suture it again. Timing is everything here; if it is delayed, the tissue may be lost irreversibly.

Frequently Asked Questions

How are vascular repairs performed with microsurgery?

Microsurgical vascular repair is performed by suturing end-to-end vessels smaller than a millimeter using specialized microscopes and fine suture materials.
