The male jaw is typically wide, angular, and prominent; this structure is softened through surgery.

With mandibular contour surgery, the sides of the jawline are narrowed and angular protrusions are filed down. The chin tip (mentum) is reduced and reshaped into a shorter and more oval form. In this way, the lower face is reshaped to match feminine proportions.

If necessary, the chin tip can be set back or moved upward. In some cases, if there is excess soft tissue beneath the chin, a more elegant transition line can be created with liposuction or submental tightening procedures. All these procedures provide balance to the lower facial profile.

The postoperative recovery period is generally 2–3 weeks. As swelling subsides, the final appearance settles within a few months. This intervention contributes to the gender affirmation process by making the jaw structure slimmer, more delicate, and harmonious with the overall feminine structure of the face.

Things You Should Know

Information

Definition

The transformation of masculine jawlines into feminine jawlines is the surgical or aesthetic conversion of a masculine (male-type) wide, angular, and prominent jaw structure into a narrower, rounded, and more delicate feminine jaw form.

Purpose

To feminize the overall aesthetic proportions of the lower face, soften facial features, and reshape the jawline and chin tip in accordance with facial feminization.

Targeted Structures

Mandibular (jawbone) angles, chin tip (menton), jawline, masseter muscle (indirectly).

Surgical Methods

Mandibular angle resection: Filing or removing the jaw corners.Genioplasty: Narrowing, contouring, and repositioning the chin tip when necessary.Lower jaw contouring: Overall thinning and smoothing of the bone surfaces.

Additional Methods

Masseter Botox: Creating a narrower facial contour by thinning the jaw muscles (non-surgical, temporary).Filler applications: Temporary contour corrections (not an alternative to surgery).

Type of Anesthesia

Surgical procedures are performed under general anesthesia; Botox and filler applications can be performed with local anesthesia.

Operation Duration

Surgical procedures last between 2–4 hours. Questions

What Is the Transformation of Masculine Jawlines into Feminine Jawlines?

The transformation of masculine jawlines into feminine jawlines refers to surgical or medical aesthetic procedures applied within the scope of facial feminization. In this process, the more angular and prominent male jaw structure is transformed into a narrower, oval, and softer feminine form. The procedure may be performed through jaw reduction, chin contouring, or filler applications and is planned while considering the overall aesthetic balance of the face.

What are the fundamental differences between male and female jaw anatomy?

When planning surgery, we must first understand the anatomical rules set by nature. Beneath the differences that are perceived as “hard” or “soft” when looking at male and female facial skeletons from the outside lie very clear structural realities. The male jaw is generally wider, more square-shaped, and volumetrically larger due to developmental processes and hormonal effects.

The most prominent difference lies in the turning angle of the jawbone just below the ear, known as the mandibular angle. In men, this region usually forms a very steep and sharp angle. This gives the face a wide, box-like appearance when viewed from the front. In women, this angle is much wider and descends from below the ear toward the chin tip with a much softer, curved line.

The main elements creating these anatomical differences include:

The degree of the mandibular angle

The width of the chin tip

The thickness of the bone tissue

The prominence of muscle attachment points

The vertical height of the chin tip

The depth of the mentolabial fold

In women, the bone surface is smoother, the cortical layer is thinner, and the overall contours are more subtle. In addition, while the chin tip in men is often wide, blunt, and sometimes divided in the middle, in women it has a more pointed and delicate projection that gathers at a single point. Our primary goal here is to take these dominant features of male anatomy and bring them toward feminine standards within the limits allowed by anatomy.

How is the ideal feminine jawline determined during surgical planning?

Every face has its own story and proportions. For this reason, creating a single standard female jaw is not a correct approach. However, there are universal parameters that are perceived as feminine in aesthetic perception. An ideal feminine jawline, when viewed from the front profile, forms a shape that narrows downward from the upper part of the face, without sharp corners, resembling an oval or an inverted triangle.

During the planning phase, we focus not only on the patient’s jaw but on the entire face. This is because the jaw is the frame of the face. If the cheekbones are very wide, excessively narrowing the jaw may make the face appear unbalanced. Our aim here is to make the width between the two jaw corners narrower than the width between the cheekbones.

Profile analysis is also very critical in this process. When viewed from the side profile, the relationship of the chin tip with the nose and lips is very important. In men, the fold under the lower lip is deeper and sharper, whereas in women it is shallower with softer transitions. While planning surgery, we aim not only to reduce the bone but also to bring the soft tissue transitions into feminine norms.

Which techniques are used in mandibuloplasty surgery?

Mandibuloplasty, which means reshaping the jawbone, is the cornerstone of this process. We perform the entire procedure through incisions made inside the mouth. In this way, no scar remains visible from the outside, that is, on your facial skin, which is a great aesthetic advantage.

At this stage, several different techniques are used together to change the shape of the bone. In male-type jaws, the jaw angle is usually turned outward. Using surgical saws or more modern technologies such as sound wave–based devices, we remove this outward-pointing sharp corner in a curved manner. This process softens the sharp angle and transforms it into a feminine curve.

However, simply cutting the corner is often not enough. The jaw must also appear narrow when viewed from the front. The mandibular bone has a layered structure. Without compromising the strength of the bone, we thin only the outermost thick layer along its length. This procedure reduces the width of the face millimetrically and replaces the coarse appearance with a more delicate contour.

Technologies used during this procedure include:

Piezoelectric surgical devices

Micro saws

Bone rasps

High-speed drills

Why is chin aesthetics the key point of feminization?

After narrowing the jaw corners, attention turns to the very tip of the chin. The chin tip is the signature area that determines the character of the face. A wide and square chin tip can maintain the masculine perception of the face no matter how much the jaw is narrowed. Therefore, we always perform chin aesthetics simultaneously with jawline contouring.

Here, we generally prefer techniques in which the bone is cut and reshaped. We divide the chin bone with a special incision. After removing the excess bone segment in the middle, we bring the side segments together at the midline. This procedure allows us to narrow and sharpen the wide male chin tip and transform it into a feminine form.

If our patient’s face is vertically very long, we can shorten the face by removing an additional horizontal piece of bone during this procedure. Conversely, if the chin is too far back, we can strengthen the profile by moving the cut bone segment slightly forward. All these bone segments are fixed in their new positions with titanium plates and screws. As a result, we achieve a continuous, smooth, and tapering line starting from the jaw corner and ending at the chin tip.

How are soft tissues and nerves protected during the surgical procedure?

The most delicate point of these surgeries is the nerve line that provides sensation to the lower lip and chin tip. This nerve runs along a canal inside the jawbone and emerges from a specific point to spread toward the lip. The most important part of surgical planning is protecting this nerve.

Thanks to the technologies and imaging methods we use in modern surgery, we know very clearly where this nerve passes. Some special devices we use during surgery are designed to cut only hard tissue, meaning bone. When they come into contact with soft tissue, such as a nerve or vessel, the device stops cutting or does not cause damage. This technology allows us to minimize the risk of nerve injury.

In addition, treating tissues with respect during surgery and progressing beneath the periosteum reduces bleeding and swelling. We work by leaving a safe area around the opening where the nerve exits. Our goal is not only to achieve an aesthetic appearance but also to preserve function and sensation.

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Why is it necessary to address the masseter muscle in addition to the bone structure?

Sometimes even after thinning the bone structure, a patient may still feel that their jaw appears angular when looking in the mirror. One of the main reasons for this is the chewing muscles. These muscles are located just above the jaw corner and are very powerful. In men, due to genetic predisposition or habits such as teeth clenching, these muscles may enlarge excessively.

After thinning the bone, if the muscle remains very thick, achieving the delicate appearance we desire becomes difficult. The volume of the muscle can cover the bone and mask the thinning procedure we performed. Therefore, the muscle must also be reduced.

The methods we apply at this stage include:

Surgical muscle shaving

Botulinum toxin applications

Muscle reduction with radiofrequency

We generally prefer non-surgical methods, especially toxin applications. By applying treatments to the muscle at regular intervals before or after surgery, we reduce its activity. A muscle that is not active weakens over time and loses volume. This is an excellent complement to bone surgery. As the muscle becomes thinner, the newly shaped bone structure beneath it becomes more visible and the face appears even narrower.

How do procedures performed on cheek fat and the submental area support the process?

The face is a whole and cannot be considered in separate parts. Even if we make the jawline perfect at the bone level, excessive fullness in the cheeks or fat under the chin can overshadow that sharp line. In the male face, the cheeks may appear flatter, but sometimes deep fat pads inside the cheeks can make the lower face appear heavy and wide.

In such cases, we can remove cheek fat through the same incision inside the mouth during the bone surgery session. By removing part of the stubborn fat mass inside the cheek, we emphasize the hollow beneath the cheekbones. This procedure makes the line descending from the cheekbone to the jaw more dramatic and feminine.

Similarly, the submental and neck region is very important. A feminine neck angle should be sharp and well-defined. If there is fat accumulation here, we remove it through liposuction. If necessary, we use energy-based technologies to tighten sagging skin. In this way, the jawline appears perfectly tight not only at the bone level but also with the skin envelope over it.

Soft tissue procedures that can be combined include:

Buccal fat pad removal

Submental liposuction

Neck lift

Skin tightening with radiofrequency

What is the importance of 3D imaging and planning before surgery?

The era of approximate surgery is over. Technology allows us to deeply visualize the patient’s anatomy and predict the outcome. We always request a three-dimensional tomography from each patient. With this scan, we can examine the thickness of the bone, the exact route of the nerve canal, the condition of the tooth roots, and asymmetries on a computer screen.

With these data, we perform virtual planning. On the computer, we calculate exactly where to cut the bone and how much to thin it, with millimetric precision. In some complex cases, we even produce patient-specific guides according to this plan. These guides are placed on the bone during surgery and show us where the cuts should be made. This method reduces the margin of error almost to zero.

In addition, simulations are very valuable for communication with our patients. When the patient sees approximately how their face will look after surgery on the screen, their concerns decrease and we can place their expectations on a realistic basis. The alignment of expectations with results is the most important criterion for success in aesthetic surgery.

What is the recovery process like and when can patients return to social life?

Since this is a major transformation surgery, the recovery process requires some patience, but it is not as painful as many imagine. We usually host our patients in the hospital for a short period. After the surgery, a special bandage is placed on the face. This bandage is very important to limit swelling and help the skin adapt to the new bone structure.

The first few days are the period when swelling is most intense. Your face may look quite swollen, which is expected and completely temporary. Rather than pain, a feeling of tightness dominates, and it can be easily controlled with simple painkillers.

Points to consider during the recovery process include:

Sleeping with a high pillow

Applying ice

Avoiding heavy activities

Staying away from hot environments

Maintaining regular oral care

Nutrition is critically important during this period. Because there are intraoral incisions, we recommend consuming liquid and puree-style soft foods during the first week. Avoiding strain on the chewing muscles is important for bone healing. In addition, maintaining oral hygiene with special mouthwashes we provide after each meal eliminates the risk of infection.

Recommended foods include:

Warm soups

Fruit purees

Yogurt

Pudding

Protein drinks

Returning to social life is usually possible after the second week. Most of the swelling will have subsided, but the face settling and the emergence of fine details may take several months. The bone reaching its final shape and the thinning of soft tissue is a biological process that takes time. During this period, we stay in constant communication with our patients and manage the process together.

#col-829022012 > .col-inner {
margin: 0px 0px -45px id="What_are_the_possible_risks_and_complications_of_this_operation">What are the possible risks and complications of this operation?

Like every surgical procedure, facial feminization surgery has risks, and discussing them openly with our patients is the foundation of trust. The most common condition is temporary sensory changes. Because swelling in the jaw area puts pressure on the nerves, mild numbness may occur in the lower lip and chin. In the vast majority of cases, this condition improves spontaneously over time.

Permanent sensory loss is very rare in experienced hands and with modern technologies. The risk of infection exists because the procedure is performed inside the mouth, but with preventive antibiotic use and good oral hygiene, this risk is very low. Sometimes a condition called soft tissue sagging may occur, where the skin appears loose after the bone is reduced. To prevent this, we use tissue suspension techniques during surgery.

Possible complications include:

Temporary sensory loss

Hematoma

Infection

Asymmetry

Soft tissue sagging

Can similar results be achieved with non-surgical methods?

The answer to this question depends entirely on the patient’s existing anatomy and expectations. If the bone structure is very wide, angular, and prominent, it is unfortunately not possible to achieve full feminization using only non-surgical methods. Without reducing bone volume, it is difficult to narrow the face with only superficial procedures.

However, we have strong camouflage strategies for patients whose bone structure is borderline or who do not wish to undergo surgery. It is possible to narrow the face with applications to the chewing muscles and create a more pointed and feminine appearance by adding filler to the chin tip. In addition, melting cheek fat and tightening the skin with energy-based devices can make the face appear more oval.

Non-surgical options include:

Hyaluronic acid fillers

Masseter Botox

Focused ultrasound

Thread face lifting

Enzymatic lipolysis

The effect of these methods is usually temporary and does not provide skeletal reduction; they only create a visual illusion and soft tissue thinning. We generally prefer to use these methods either in mild cases or as complementary treatments to perfect the result after surgery.

Why is patient compliance critical in the postoperative period?

As surgeons, we complete the technical part of the work in the operating room, but the quality of the healing process is largely shaped by the care the patient shows afterward. Wearing the chin bandage we provide for the specified duration is vital for proper swelling distribution and for the skin to adhere to the new bone structure. Removing the bandage early or using it irregularly may prolong the recovery process.

Smoking is the greatest enemy of wound healing. By reducing blood circulation at the suture lines, it increases the risk of infection and tissue loss. For this reason, smoking is strictly prohibited for a certain period before and after surgery. In addition, avoiding heavy sports during the first weeks, sleeping with the head elevated, and staying away from hot environments such as baths or saunas helps swelling subside quickly. The higher the patient compliance, the more satisfying the aesthetic result we achieve.

Why is a multidisciplinary and holistic approach necessary?

The transformation of masculine jawlines into feminine forms cannot be considered a simple bone surgery. This process is a multidimensional field where aesthetic surgery, facial surgery principles, medical aesthetics, and even psychology intersect. Each patient is unique, and every face has different needs.

The patient’s perception of gender identity and the self they wish to see in the mirror is our most important guide. Sometimes addressing only the jaw is not enough; the harmony with the nose, forehead, or lips must also be considered. While changing one part of the face, it is essential not to disrupt the balance with other areas.
