The prominent and flatter male-type forehead contour is adapted to feminine proportions and harmonized with the overall facial structure.

In this procedure, the bone over the frontal sinus that creates the forehead prominence is first contoured, or if necessary, reshaped to achieve a smoother contour. In the same session, the hairline can be advanced downward to reduce forehead height, creating a shorter and more feminine forehead appearance.

Eyebrow position can also be reshaped as part of forehead feminization. With a brow lift, the eyebrows are positioned higher and given a more arched shape. This gives the gaze a softer, more feminine expression.

Although the postoperative recovery process varies from person to person, patients can generally return to social life within 1–2 weeks. Since the incisions are hidden within the scalp, scars fade over time. As a result, the face gains a more balanced, aesthetic, and feminine structure.

Things You Should Know

Information

Definition

Forehead feminization is a facial feminization procedure that aims to soften a masculine (male-type) forehead structure using surgical or aesthetic methods and transform it into a more delicate, rounded, and feminine form.

Purpose

To flatten the prominent bony structure above the eyebrows to create a softer, more feminine face, ensure harmony between the forehead and brow structure, and create an aesthetic transition between the hairline and the forehead.

Targeted Structures

The bony prominence over the frontal sinus (glabella), frontal bones (frontal bone), hairline, sub-brow area.

Surgical Methods

Frontal bone contouring (forehead contouring): The protrusions above the eyebrows are filed down or reshaped by cutting.Frontal sinus wall osteotomy: A prominent sinus wall is cut, set back, and reshaped.Hairline advancement (lowering the hairline): Feminizes facial proportions by reducing forehead height.

Type of Anesthesia

Usually performed under general anesthesia.

Operation Duration

Varies depending on the scope of the procedure, but typically takes 2–4 hours.

Recovery Process

Swelling and bruising may occur in the first 1–2 weeks. class="ez-toc-section" id="What_Is_Forehead_Feminization_Surgery">What Is Forehead Feminization Surgery?

Forehead Feminization Surgery is a surgical procedure, performed especially as part of facial feminization surgery, that aims to give the forehead a softer, smoother, and more feminine appearance. In this surgery, the prominences of the forehead bone can be reduced, the brow bone can be contoured, and the hairline can be repositioned. The goal is to achieve a more balanced and feminine profile while preserving the overall aesthetic proportions of the face.

Why does forehead feminization change gender perception of the face so fundamentally?

When we examine the anatomical map of the face, the area where differences between sexes are most pronounced is not the lips or the cheeks; the truly defining difference is hidden in what we call the “T-zone,” around the eyes and the forehead line. The fundamental distinction between masculine and feminine faces is related to how light breaks across the surface. In men, the forehead structure usually creates a distinct bony protrusion just above the eyebrows and then extends backward with a flatter, sometimes harsh slope. This bony protrusion casts a shadow over the eyes like a cap brim. This gives the gaze a harsher, deeper, more serious, and inevitably more masculine expression.

In women, the forehead structure has a very different geometry. An ideal feminine forehead rises from above the eyebrows to the hairline in a continuous, vertical, and slightly convex curve. That hard bony protrusion is either absent or very minimal. This smooth surface allows light to spread across the forehead in a soft beam without breaking sharply. The core philosophy of forehead feminization surgery is to redesign this play of light by changing the bone structure. We aim to transform the shadowed and harsh expression created by a masculine forehead into a reflective, open, calm, and inviting feminine expression. When this change is achieved, even other masculine features in the rest of the face begin to appear much softer in the observer’s perception.

What are the distinct anatomical differences between a male and female forehead?

There are some fundamental bone and soft-tissue differences that we reference during surgical planning. Male and female skulls develop in different directions due to hormonal effects during puberty. For a surgeon to know what needs to be changed, they must recognize the anatomical markers coded as “masculine” very well. These markers are not merely aesthetic flaws; they are biological indicators of sex.

Characteristic features we commonly see in a male-type forehead include:

Supraorbital bony protrusion known as frontal bossing

A forehead plane that slopes backward

Larger and more voluminous frontal sinus cavities

An “M”-shaped hairline recession at the temples

A flatter, more horizontally positioned brow

Thicker and coarser bone tissue

Pronounced bony rims above the eye sockets

Each of these features is addressed one by one during surgery. Our aim is to reverse this list and create a forehead structure that is free of protrusions, rises vertically, has rounded contours, and is proportional to the hairline.

Why is computed tomography vital in preoperative planning?

A successful forehead feminization surgery begins long before the operating room, at the radiology workstation. Every patient’s anatomy is as unique as a fingerprint, and it is not possible to apply a one-size-fits-all technique. In this process, our greatest guide is high-resolution 3D Computed Tomography (CT) scans. Touching the forehead without these scans is like walking in the dark and is unacceptable by modern surgical standards.

The reason CT is so critical is the air cavities called the “frontal sinus,” located just behind the forehead, in the middle of the eyebrows. The anatomy of these sinuses varies tremendously from person to person. In some patients, the sinuses are very large and are the main reason for the prominent bony protrusion; in others, the sinus may not have developed at all, or the bony wall may be very thick. As surgeons, we must know with millimetric precision how much we can safely file down or reshape the bone. The CT scan clearly shows us where we must stop, the thickness of the anterior wall of the sinus, and the position of the posterior wall protecting the brain. In light of these data, we decide which surgical technique (Type 1, Type 2, or Type 3) will be the safest and most effective route for our patient.

Who is the Type 1 forehead feminization technique applied to?

In forehead feminization, the idea of “one technique fits everyone” is absolutely wrong. The first and simplest approach we determine based on bone structure and sinus anatomy is the Type 1 technique. This technique is essentially a shaving or filing procedure. However, not every patient is a suitable candidate for this method.

The ideal patient group for the Type 1 technique is as follows:

Those whose frontal sinus has not developed at all

Those with a very small sinus volume

Those with a fairly thick anterior sinus wall bone

Those with minimal forehead prominence

In this method, using special surgical instruments or ultrasonic piezo devices, we gently thin the excess bony protrusion above the eyebrows. However, we have a limit here: the thickness of the sinus wall. If the patient’s sinus wall is thin, shaving alone may lead to sinus perforation or may be insufficient to achieve the desired flattening. In most masculine foreheads, the bone is thin and there is an air cavity behind it, so aggressive shaving cannot be performed. Type 1 generally provides sufficient results for only a small portion of patients, around 5–10%.

Why is the Type 3 reconstruction technique considered the gold standard?

In the vast majority of our patients, there is a wide frontal sinus cavity under the hard and prominent brow bone. In this situation, shaving the bone from the outside is impossible because the bone is thin, and when it is thinned by a few millimeters, the sinus cavity is entered. At this point, the most sophisticated form of surgery—the Type 3 procedure—comes into play. This technique is not a “camouflage” procedure; it is truly a “reconstruction” process.

The stages of Type 3 surgery can be summarized as follows:

First, the bone segment on the anterior wall of the sinus is cut within the planned boundaries.

This bone segment is removed like a “window lid.”

The removed lid is contoured and thinned outside the body and given a flatter form.

Excess bony protrusions around the sinus cavity are removed.

The reshaped bone lid is repositioned not in its old place but more posteriorly and at a flatter angle.

The bone is fixed in its new position with titanium microplates and screws.

Although this may sound complex, it is actually the method that provides the most natural and permanent result. Because we change not only the thickness of the bone but also its position. Thus, the hard protrusion on the forehead completely disappears and is replaced by a smooth curve. Once healing is complete, full bony continuity is restored, and from the outside, no junction line or artificial appearance is noticeable.

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Why is the Type 2 technique losing validity in modern surgery?

Another method that emerged as an interim solution in the evolution of surgical techniques but has lost popularity today is Type 2. This technique was used in cases where shaving was insufficient but the surgeon hesitated to perform a full Type 3 reconstruction. Its logic is to shave the brow bone a bit and create an “illusion” by filling the depression in the upper forehead with bone cement or filler materials.

However, the disadvantages of this method have become clearer over time:

Enlarging the forehead by filling it forward instead of setting it back

Risk of surface irregularity and waviness

Possibility of foreign body reaction

Filler material may shift over time

Not providing the feel of natural bone tissue

Today, our aesthetic surgery perspective is built on solving problems anatomically rather than hiding or masking them. Since shaping the patient’s own bone tissue (Type 3) instead of using foreign material provides much healthier, lifelong permanent, and aesthetic results, the Type 2 approach is being preferred less and less.

How are brow position and soft-tissue adjustments performed?

Changing the bony framework is the foundation, but neglecting the soft tissue is like repairing a roof and not fixing the curtains—it leaves the result incomplete. In a feminine gaze, the position and shape of the eyebrows are at least as important as the bone underneath. In men, the eyebrows usually sit directly over the bony prominence and follow a flat, low line. In women, the eyebrows are positioned higher above the supraorbital rim and have a more arched shape with the lateral tails curving upward.

The soft-tissue procedures we perform during forehead feminization include:

Suspension of the lateral portions of the brows upward

Creating a feminine arch in the brows

Tightening and smoothing the forehead skin

Reducing fullness around the eyes

When the bony protrusion is removed, the tension above the brows decreases and the tissues are released. At this stage, we fix the brows in their new position higher and with a feminine arch. This not only beautifies the forehead but also reduces heaviness on the upper eyelid, helping the person look younger, more refreshed, and more awake. Sometimes eyelid surgery is added to complete the aesthetic harmony around the eyes.

How is forehead width balanced by hairline advancement?

A common complaint among many of our patients in the gender transition process or those who structurally have a broad forehead is that the hairline sits too far back or that there are “M”-shaped recessions at the temples due to male-pattern hair loss. A high and wide forehead can increase the vertical dimension of the face, causing an imbalanced appearance and strengthening a masculine perception.

Forehead feminization surgery is an excellent opportunity to lower the hairline (hairline lowering). If the patient’s forehead is vertically very long, we make the surgical incision not within the scalp but at the exact boundary where the hairline begins. In this way, by excising excess forehead skin and advancing the scalp forward, we can physically narrow the forehead. With this procedure, the “M”-shaped recessions are rounded into a more oval, feminine hairline, and forehead height is brought into ideal proportions. Especially in patients with significant forehead height, this maneuver can completely change the facial frame and provide a much more proportional and softer expression. In addition, a hair transplant can be planned simultaneously to further perfect the result.

Where are the scars and how are they concealed?

The biggest concern for patients in aesthetic surgery is naturally scarring. We often encounter the question, “Will there be a long scar across my face?” In forehead feminization, where the scar will be depends on the patient’s hairline plan, and surgical techniques are designed to conceal these scars.

The main incision approaches we use are:

Bicoronal Incision: An incision made within the scalp from one ear to the other, through the crown area. The scar remains completely within the hair.

Pretrichophytic Incision: An incision made exactly at the boundary where the hair starts, in patients who want to lower the hairline.

Especially in hairline incisions used to shorten the forehead, we apply a very special technique called “trichophytic closure.” In this technique, we suture the wound edges at such an angle and with such a method that during healing, hair follicles grow through and out of the suture line to the surface. In other words, hair grows directly through the scar line and naturally camouflages it. Once healing is complete (approximately 6–12 months), this scar usually becomes a barely visible thin line and is rarely noticeable in daily life—even at the hairdresser.

#section_832695872 often expect very severe pain because the bone is manipulated, but surprisingly, forehead feminization is one of the facial surgeries with the lowest pain scores. The reason is that the nerves providing sensation to the forehead are temporarily affected during surgery, and the area becomes numb for a while. Rather than unbearable pain, our patients describe a feeling of tightness or a pressure as if there is a “tight band around the head.”

Expected experiences during recovery include:

Temporary numbness in the forehead and scalp

Swelling and bruising descending around the eyes

A feeling of fullness and pressure in the head

Mild fatigue in the first days

The first 72 hours after surgery are the period when swelling and any bruising are most prominent. Due to gravity, this swelling can move from the forehead down to the eyelids. This is a completely natural and expected part of the process. By the end of the first week, with removal of the stitches, the patient can gradually return to social life. However, bone healing and the settling of soft tissue over the new skeletal framework require patience. Major swelling subsides within the first month, but achieving the final smooth forehead contour and complete resolution of scalp numbness may take 6 months, and sometimes up to 1 year depending on nerve healing speed.

Can non-surgical methods be effective for forehead feminization?

The search for change without surgery always exists. Patients sometimes come in with the question, “Can’t I get the same result with fillers or Botox?” The answer depends on the patient’s anatomy and expectations, but generally, it is that these methods cannot replace surgery.

Non-surgical methods may be used for:

Slightly lifting the brows (Botox)

Smoothing expression lines (Botox)

Filling temporal hollowing (Filler/Fat injection)

Improving skin quality (Mesotherapy)

However, if a patient has a prominent bony protrusion (frontal bossing), trying to hide it with fillers is a strategic mistake. Filling above and below the protrusion can push the forehead forward even more, leading to an unnatural, bulging appearance similar to a “dolphin forehead.” Our goal is not to “inflate” or enlarge the forehead but to “restructure” and reduce it backward. For this reason, medical aesthetic applications are generally preferred as complementary fine-tuning or to improve skin radiance after surgery.

What are the psychological and social effects of forehead feminization surgery?

As surgeons, explaining the technical, millimetric, and anatomical details is part of our job, but the point we truly focus on—and find most fulfilling—is the emotional transformation the patient feels when looking in the mirror.
