It is especially preferred in trans women as an important part of facial feminization surgery.

The procedure is performed under general anesthesia and is carried out through a small incision parallel to the natural skin lines on the front of the neck. The protrusion of the thyroid cartilage at the front of the larynx is carefully shaved to achieve a flatter, more feminine neck profile.

During surgery, a careful technique is used to avoid damage to the vocal cords. When performed by experienced surgeons, the risk of deterioration in voice quality is extremely low. The procedure usually takes 30–60 minutes, and same-day discharge is possible.

The recovery process is fast; most patients can return to normal activities within a few days. Since the scar is hidden in the neck creases, it is not noticeable aesthetically. Adam’s apple shaving softens the lower facial profile and strengthens gender congruence.

Things You Should Know

Information

Definition

Adam’s apple shaving (tracheal shave / chondrolaryngoplasty) is the surgical reduction of the thyroid cartilage (Adam’s apple), which is located in the neck and is prominent in men. It is generally performed as part of facial feminization surgery.

Purpose

To make the neck area more delicate and flatter, create a feminine profile appearance, and achieve gender congruence in trans women.

Area of Application

The thyroid cartilage (Adam’s apple) on the front of the neck.

Surgical Method

Through a small skin incision, the excess bony/cartilaginous structure on the anterior surface of the thyroid cartilage is filed down or removed. Minimally invasive techniques are used without damaging the vocal cords.

Type of Anesthesia

Usually performed under general anesthesia; in some centers, sedation + local anesthesia may also be preferred.

Operation Duration

Takes an average of 30–60 minutes.

Recovery Process

Swelling, mild pain, and a feeling of tightness decrease within the first 1–2 weeks. class="ez-toc-section" id="What_Is_Adams_Apple_Shaving_Surgery">What Is Adam’s Apple Shaving Surgery?

Adam’s apple shaving surgery, medically known as chondrolaryngoplasty, is the procedure of surgically reducing the prominent thyroid cartilage on the front of the neck. It is generally preferred for aesthetic concerns or as part of the gender affirmation process. Care is taken not to damage the vocal cords during the procedure. The surgery is most often performed under general anesthesia, and the recovery process is usually short and uneventful.

Why Does the Adam’s Apple Form and What Is Its Anatomical Structure?

Just as every structure in our body has a biological rationale, the Adam’s apple that you request to be reduced for aesthetic concerns also exists there for very important reasons. This hard structure you can feel on the front of your neck, just below the jawline, is actually the “thyroid cartilage,” the largest part of the cartilaginous framework that forms the larynx. You can think of it as a natural shield or armor produced by the body to protect your vocal cords from impacts, traumas, or blows from the outside world.

This cartilaginous structure is not a single piece. It consists of two broad wings, medically called laminae. These two wings come together at the front of the neck starting from fetal development and form a “V” shape. This junction point creates the protrusion we see from the outside. Looking at its anatomical neighbors, the hyoid bone connected to the base of the tongue is located just above it, and the cricoid cartilage, which connects to the trachea, is located below it. The thyroid gland is situated slightly lower than this structure. Therefore, this area is the exact crossroads of vital functions such as breathing, speaking, and swallowing.

The anatomical components of this region are:

Thyroid cartilage

Cricoid cartilage

Hyoid bone

Thyrohyoid membrane

Vocal cords

Why Does the Appearance of the Adam’s Apple Differ in Men and Women?

The answer to this question lies in the hormonal storms of puberty and the sex-specific course of skeletal development. In fact, during childhood, the laryngeal structures, neck profiles, and voice tones of girls and boys are quite similar. However, when puberty begins, the hormonal codes brought by biological sex come into play. Especially in males, increased testosterone levels trigger the growth and reshaping of the laryngeal cartilages.

In men, as this growth occurs, the two wings of the thyroid cartilage meet at the front at a fairly narrow and sharp angle. This angle is around 90 degrees in men. This acute angle causes the cartilage to project forward sharply and leads to the characteristic, prominent Adam’s apple appearance. At the same time, this growth allows the vocal cords inside to lengthen and thicken, which is the fundamental reason why the male voice deepens and “breaks.”

In women, the process works differently. Due to the effect of estrogen and the lower levels of testosterone, cartilage development occurs in a more limited and flatter manner. In women, the wings of the thyroid cartilage meet at an approximately 120-degree, wider and softer angle. This wide angle prevents the cartilage from protruding forward and allows it to sit more embedded and flatter within the neck tissues. In other words, women also have this cartilage, but because of its angle and size, it does not create a noticeable shadow from the outside.

Factors affecting the appearance include:

Cartilage junction angle

Thickness of neck fat tissue

Vertical height of the thyroid cartilage

Structure of the neck muscles

Skin elasticity

Who Is a Suitable Candidate for Adam’s Apple Shaving Surgery?

Chondrolaryngoplasty is most commonly requested today by trans women (individuals in the male-to-female transition process) as an indispensable part of the Facial Feminization Surgery (FFS) package. Because no matter how successful a facial procedure, rhinoplasty, or jaw contouring may be, a prominent protrusion in the middle of the neck can continue to create a “masculine” perception and push the individual toward social dysphoria. For this reason, softening the neck profile in the gender affirmation process is a psychological necessity for the individual to match their identity with their reflection in the mirror.

However, our patient profile is not limited to trans individuals. Some biological men (cisgender) may be seriously bothered by an excessively large, sharp, or asymmetric Adam’s apple. This can create various aesthetic and functional issues, ranging from difficulty buttoning shirts to frequent cuts while shaving the area, or the sense of a disproportionate mass on a thin neck. Likewise, although rare, in some biological women the thyroid cartilage may be more prominent than expected due to hormonal or genetic reasons.

Eligibility criteria for surgery include:

Having realistic expectations

Being in good general health

No active infection in the neck area

Normal vocal cord function

Being able to comply with the recovery process

Contact us to get information about treatments and create an appointment!

 Contact via WhatsApp

Which Tests Are Performed Before Surgery and How Is the Process Planned?

Successful surgery begins long before the operating room, during a detailed examination and planning process. In our first consultation, our priority is to manually examine (palpate) the neck area to understand the hardness, size, mobility of the cartilage, and how smoothly the skin glides over the cartilage. However, touching from the outside does not tell us everything about the critical internal boundaries. We need to clearly see what is happening behind the cartilage.

At this stage, the most critical point is determining where the vocal cords attach to the inner surface of the cartilage. This attachment point, which we call the “anterior commissure,” is our surgical limit—our red line. To measure this distance, we generally use radiological imaging methods. We request a neck CT scan or a lateral neck X-ray. In these images, we measure the thickness of the cartilage and the attachment point of the vocal cords with millimetric precision.

If the attachment point of the vocal cords is anatomically very high, the amount of cartilage we can safely shave may decrease. Knowing this before surgery allows us to provide an honest and scientific answer to the question, “How flat can your neck become?” In addition, we обязательно analyze your voice quality before surgery. If you have existing hoarseness, vocal fatigue, or a chronic throat infection, these must first be evaluated by ENT (Ear, Nose and Throat) specialists.

Possible tests include:

Lateral neck X-ray

Neck computed tomography

Videolaryngostroboscopy

Voice analysis

Thyroid function tests

How Are the Surgical Technique and Incision Site Determined in Adam’s Apple Shaving?

The surgery is usually performed under general anesthesia, meaning you will be fully asleep and will not hear or feel anything. The procedure takes an average of 45 minutes to 1 hour. As surgeons, our main goal is to achieve the best viewing angle with the smallest possible scar and to shape the cartilage safely. There are different approach strategies for this, but the most common and reliable is the “external horizontal incision.”

In this technique, we make a small incision of about 2–3 centimeters either directly over the most prominent point of the Adam’s apple or within a natural skin crease in the neck (for example, the horizontal neck lines that form with age). When it heals, this incision is camouflaged among the natural shadows of the neck. After making the incision, we gently separate the thin muscle layers under the skin and expose the thyroid cartilage. We then lift the membrane over the cartilage (perichondrium) to reach the main shaping field.

During the shaping phase, we use precise shaving devices called “burrs,” which are high-speed tools with diamond or steel tips, or very sharp surgical scalpels. The upper and middle parts of the cartilage that protrude forward are thinned layer by layer with millimetric precision. You can compare this to a sculptor carving marble; while removing the excess, you must not compromise the main stability of the structure.

Surgical instruments used include:

Surgical scalpel

Diamond-tipped burr

Tissue retractors

Fine dissection scissors

Bipolar cautery

What Is Done During Adam’s Apple Surgery to Protect the Vocal Cords?

The biggest fear among our patients—and the most justified question—is voice health. Concerns such as “Will I lose my voice?” or “Will my voice become hoarse?” are very common. Adam’s apple shaving is not technically a procedure performed on the vocal cords; it is a modification carried out on the “outer facade” of the chamber where the vocal cords are located. However, these two structures are neighbors separated by mere millimeters.

The vocal cords are suspended on the inner surface of the thyroid cartilage, exactly at the midline, at a point called the “anterior commissure.” While shaving the cartilage by thinning it from the outside inward, we approach this attachment point. The surgeon’s mastery comes into play here: we want to achieve the flattest possible neck, but we must never weaken this attachment site. If the cartilage is thinned excessively and this attachment point is damaged, the vocal cords may loosen. This can cause a permanent lowering of pitch, a muffled voice, or voice breaks.

To minimize this risk, during surgery we sometimes use an internal light source or mark the vocal cord attachment point with a small needle. We stay a few millimeters above the safe limit—that is, above the vocal cord attachment site. Our principle is: “Rather than risking laryngeal function and voice quality, it is always better to leave a very slight cartilage convexity.”

Safety measures include:

Identification of the anterior commissure

Gradual shaving technique

Use of nerve monitoring

Keeping the surgical field clear

Gentle tissue maneuvers

What Should You Expect During Recovery and How Should the Adam’s Apple Area Be Cared For?

The recovery process after chondrolaryngoplasty is generally quite comfortable for our patients and, contrary to fears, it is not a very painful process. After surgery, you are usually discharged the same day or, for safety, the next day. As the effects of anesthesia wear off, you may feel a mild burning sensation in your throat, a catching sensation when swallowing, or the feeling that there is a lump in your throat. This is completely normal because the surgical area is swollen and the throat muscles were manipulated during the procedure.

During the first 24–48 hours, applying ice intermittently to the neck area without direct contact with the skin greatly helps reduce swelling (edema) and possible bruising. Pain is usually at a level that can be controlled with simple painkillers. You can use your voice; you do not need to go on “voice rest.” However, we strongly recommend that you do not strain your voice in the first few days, especially avoid whispering (because whispering strains the vocal cords more than normal speech), and do not shout. You may have slight roughness or muffling in your voice in the first week; this is due to internal edema and is temporary.

Things to pay attention to during recovery include:

Keeping the head elevated

Avoiding strenuous exercise

Avoiding hot foods

Taking prescribed medications regularly

Drinking plenty of id="Will_There_Be_a_Scar_After_Surgery_and_Is_It_Possible_to_Conceal_the_Scar_in_the_Adams_Apple_Area">Will There Be a Scar After Surgery, and Is It Possible to Conceal the Scar in the Adam’s Apple Area?

The most fundamental truth in aesthetic surgery is this: wherever the skin’s integrity is disrupted and a scalpel touches, a scar (cicatrix) forms. The concept of “scarless” surgery technically does not exist, but “inconspicuous scars” or “hidden scars” do. In Adam’s apple surgery, our goal is to ensure that the scar is faint enough not to be noticed from social distance and that it integrates with the natural structure of the neck.

When making the incision, we use the natural horizontal lines on your neck if they exist. When the incision is hidden within this line, it is almost never noticeable once healing is complete. If you are a young patient and have no lines on your neck, we place it strategically under the shadow of the jaw or at a point corresponding to the uppermost part of the cartilage. The neck skin is one of the best and fastest-healing areas of the body, so we are fortunate in this regard.

In the first weeks, the incision site may be slightly red, pink, and a bit firm. This is the active phase of wound healing. As months pass, this line fades, takes on the skin color, softens, and becomes thinner. To improve scar quality, after the stitches are removed we may recommend silicone-based gels or special tapes. The most critical point is sun protection. Protecting the fresh scar from direct sunlight for the first 6 months is essential to prevent darkening (hyperpigmentation).

Recommended scar care includes:

Silicone scar gels

High-SPF sunscreen

Scar-reducing tapes

Gentle massage applications

Using a sun-protective hat

Does This Procedure Raise the Voice, or Is Adam’s Apple Shaving Only Visual?

This is one of the most commonly misunderstood topics—especially among our trans women patients—and the one with the highest expectations. It must be emphasized that chondrolaryngoplasty (Adam’s apple shaving) is not a voice aesthetic or voice-raising surgery. The primary and only purpose of this procedure is to change the external silhouette and profile appearance of the neck.

Shaving the cartilage from the outside does not change the length, tension, thickness, or mass of the vocal cords inside. These are the factors that determine voice pitch (frequency). Therefore, expecting your voice to change from a male tone to a female tone or become higher solely with this surgery can lead to disappointment. Your voice will remain as it is.

If you want your voice to sound higher and more feminine, that is a completely different procedure called “Voice Feminization Surgery” (Vocal Cord Surgery / Glottoplasty). These two surgeries can sometimes be combined in the same session. However, their techniques, surgical fields, and purposes are completely different. After Adam’s apple surgery, your voice may temporarily sound deeper; this is entirely due to swelling, and as the tissues heal, your voice will return to its previous tone.

Facts about the voice include:

Does not change voice pitch

No intervention on the vocal cords

May cause temporary muffling

Does not replace voice therapy

Only changes appearance

Can Combined Aesthetic Procedures Be Performed Together with Adam’s Apple Surgery?

Absolutely yes, and in fact we often recommend combined approaches to our patients to achieve more dramatic and satisfying results. Because a beautiful and delicate neck is not only about the absence of a cartilage protrusion. The submental angle, skin quality, muscle structure, and fat distribution should be evaluated as a whole.

For example, while reducing the Adam’s apple, if there is fat accumulation under the chin (“double chin”), flattening the cartilage alone may not be sufficient. In that case, we can sharpen and define the jawline by performing neck liposuction in the same session. In older patients or people who have lost a lot of weight, if there is sagging and looseness in the neck skin, we can achieve a much younger-looking neck by combining a neck lift with cartilage correction and removal of excess skin.

In addition, in patients with a retruded chin, profile balance can be improved with a chin implant or filler. If there are vertical bands caused by the neck muscles (platysma), these can also be repaired through the same incision or additional small incisions. This holistic approach does not merely eliminate a protrusion; it reshapes your neck as an aesthetic unit.

Procedures commonly combined include:

Neck liposuction

Chin aesthetics

Neck lift

Facelift

Rhinoplasty

What Are the Risks and Complications in Adam’s Apple Surgery?

Every surgical intervention, no matter how minor it may seem, carries certain risks, and knowing them transparently is your most natural right. Chondrolaryngoplasty is generally a low-risk procedure with high patient satisfaction, but due to the nature of medicine, the possibility of complications is never zero.

The most feared risk is the voice changes detailed above. Permanent hoarseness, vocal fatigue, or unwanted changes in pitch are theoretically possible, although they are seen at very low rates such as 1–2% in experienced hands and with proper technique. Another risk is blood accumulation (hematoma) in the surgical field. The neck area is rich in blood vessels, and postoperative bleeding may require urgent intervention because it can compress the trachea from the outside.

The risk of infection is quite low in surgeries performed under sterile hospital conditions. Sometimes not enough cartilage can be removed (insufficient resection), and the patient may feel that the result is “not flat enough.” In that case, revision surgery may be required. However, it should not be forgotten that the surgeon sometimes must deliberately remain at the safety limit in order to protect the vocal cords.

Possible complications include:

Permanent voice change

Hematoma formation

Wound infection

Hypertrophic scar

Asymmetric appearance

When Do the Results Settle and When Does the Adam’s Apple Appearance Reach Its Final Form?

When you look in the mirror or touch the area right after surgery, you may notice that the sharp protrusion is gone. However, this is not the final result. In the first weeks, due to swelling caused by surgical trauma, the neck area may look thicker and more swollen than it is. Most of this swelling (80–90%) is absorbed by the body and subsides within the first 3 months.

However, the biological healing process continues. It takes time for the skin to adhere to the emptied space underneath, for the soft tissues over the cartilage to adapt, and for the incision scar to mature. Sometimes our patients in the early period (first 1–2 months) may mistake the firmness they feel for “remaining cartilage.” However, these are usually “fibrosis,” healing tissue, and they soften over time with massage.

In particular, the profile becoming clear, the skin thinning, and the neck line achieving the desired elegance can take between 6 months and 1 year. During this process, it is very important to be patient, not make hasty decisions, and not neglect the scar treatments and massages recommended by your doctor.

Factors affecting the process include:

Skin thickness

Patient’s age

Smoking

Genetic healing potential

Postoperative care

Frequently Asked Questions

What is the Adam’s apple and why does it become prominent?

The Adam’s apple is the forward protrusion of the thyroid cartilage in the larynx.
